AbstractThe primary objective of this study was to compare the prognostic performance of the haptoglobin related protein OA-519 with both estrogen and progesterone receptors, as well as with a cohort of 6 non-traditional prognostic markers: pS2, proliferating cell nuclear antigen, epidermal growth factor receptor, c-erbB-2, p53 and DF3. We examined 99 cases of invasive breast cancer using immunohistochemical methods on routine paraffin sections, 68 of which had sufficient demographic data for multivariate analyses. The statistical results indicate that only 1 factor, stage, had significant predictable power for patient survivability (stage 3—hazard ratio = 6.039, p-value =<0.0005: stage 4—hazard ratio = 7.599, p-value =<0.0005). These findings indicate that OA-519 is not any more informative a prognostic marker than the other markers used in this study. (The J Histotechnol19:13, 1996)

AbstractImmunohistochemical staining for the estrogen receptor can be influenced by a variety of technical variables, including degradation of staining with storage of tissue sections on glass slides. The epitope of the commercially available rat monoclonal antibody (H222, ER-ICA) has demonstrated a significant loss of immunoreactivity in tissue sections stored for 8 weeks. The purpose of the present investigation was to determine if the ERlD5 antibody is associated with a similar loss of reactivity. Tissue sections were stored on glass slides at room temperature for short (1–2 days) or long (>365 days) intervals prior to immunostaining with the ER1D5 antibody. Evaluation of immunostaining was performed by manual (microscopic assessment) and automated (image cytometry) methods. No difference in the intensity of the ER1D5 immunoreaction or the number of cells staining was observed by visual inspection. Although the mean summed optical density of ERlD5 staining was less in sections stored for long intervals relative to short intervals, this difference was not statistically significant. These results suggest that the ERlD5 antibody is suitable for staining tissue sections stored on glass slides at room temperature for as long as 1 year. (The J Histotechnol19:23, 1996)

AbstractNuclear organizer Region (NORs) are parts of the nucleoli involved in the synthesis of ribosomes. Silverstainable proteins of the nucleolar organizer regions (AgNORs), which are increased in neoplastic cells, can be localized in tissues embedded in resins. The method was adapted to bone, which needs to be processed undecalcified for histomorphometric purposes. Sections of undecalcified biopsies embedded in methacrylic resins were extensively demineralized in EDTA. They were stained for AgNORs and conterstained with metanil yellow. Sections were flattened with polyvinyl alcohol (a hydrophilic glue), which makes them suitable for light microscopy. The same method was applied for transmission electron microscopy (TEM) by embedding undecalcified blocks in an hydrophilic resin (LR-White). Ultrathin sections of bone mounted on gold grids could be stained for AgNORs after EDTA demineralization. The interest of hydrophilic resins as embedding media was evidenced for AgNORs staining. AbstractA microwave modification of the methenamine silver technique is detailed for the demonstration of Alzheimertype pathology. This method is compared with a microwave modification of the Bielschowsky silver stain and other modifications of the methenamine silver stain as well as with the original Bodian silver stain andβ-amyloid immunohistochemistry for the demonstration of plaque formation. The efficiency of the methenamine silver stain was remarkably improved by microwave heating (reducing the staining procedure time to 15 min on average). In addition, different microwave energy levels and timing in combination with gold-toning mimicked the appearance of either the Bielschowsky silver orβ-amyloid immununohistochemical stains. By utilizing microwaves we have been able to refine the methenamine silver stain to produce a flexible, consistent, time-efficient and cost-effective stain for Alzheimertype pathology. AbstractAbsorbable biomaterial implants are advantageous in that they require no retrieval surgery and they absorb as the affected tissue heals. The nature of the in vivo or in vitro absorption process may be determined by processing, embedding, and sectioning an explanted specimen and surrounding tissue or by directly embedding and sectioning an in vitro specimen. It is important to utilize an embedding medium that will not chemically interact with the polymer, in order to accurately determine the tissue/material absorption interaction.Poly-1-lactide absorbable polymeric films were embedded in 5 different resins to observe the chemical interaction between the resin and the absorbable material and to examine the relative ease of sectioning: methylmethacrylate, hydroxyethyl methacrylate, paraffin, epoxy, and polyester based resins. Paraffin, epoxy, and polyester resins best preserved the surface of the films. The study further suggests that paraffin may be too soft for monitoring materials used in long term absorption studies and that polyester is suitable only if polymerization is initiated before the material is embedded. Hydroxyethyl methacrylate was the most susceptible to environmental conditions. In all cases, the direction of sectioning had a substantial impact on the section quality. AbstractFibroma of tendon sheath (FTS) and tendosynovial giant cell tumor show similarities in site of incidence, benign clinical features, and biologic behavior. Whether FTS and GCT are related histogenetically and biologically has recently been studied immunohistochemically. Factor XIIIa+ dendrophages are dendritic stromal histiocytes that differentiate early in embryogenesis from primitive mesenchyme and persist in fetoplacental and adult collagenous connective tissues. FXIIIa is a fibroblast growth factor that also catalyses cross-linking of matrix protein monomers. CD34, the human progenitor antigen, is a transmembrane glycoprotein involved in signal transduction. CD34 is expressed on widely distributed fibroblast-like progenitor cells as well as endothelium in both the embryo and the adult. To explore whether FXIIIa+ dendrophages and CD34+ mesenchymal cells participate in the morphogenesis of FTS and GCT, we studied 5 FTS and 9 GCT with antibodies to factor XIIIa (FXIIIa), CD34, HHF35 actin, vimentin, and S-100. Six samples of embryonic and fetal mesenchyme were also studied. 5/5 FTS contained numerous FXIIIa+ dendritic cells that were deployed in myxohyaline stroma that also contained numerous CD34+ capillaries and foci of CD34+/actin+myofibroblasts in 4/5 FTS. GCT were cellular, relatively avascular and had minimal stroma. All 9 GCT were variably FXIIIa+. Spindled and histiocytoid mononuclear cells and pseudoadenoid space lining cells in GCT showed negative to often strong FXIIIa reactivity in a random pattern throughout the tumors. Strong reactivity was distributed haphazardly in geographic zones. Actin and CD34 were negative in GCT except for a few vessels. FXIIIa+ cells were numerous also in embryonic dermis, tendosynovial anlagen, mineralizing cartilage anlagen of bones, and the stsoma of chorionic villi suggesting that FXIIIa+ cells are active in stromal morphogenesis and matrix remodeling.We conclude that pleiotropic tendosynovial FXIIIa+ dendrophages participate in the morphogenesis of both FTS and GCT. FTS is a rapidly evolving process that is initially cellular and angioformative and later undergoes myofibroblastic sclerosis. It is rich in strongly staining FXIIIa+ dendrophages at all stages of development. FXIIIa+ dendrophage modulation of phenotypic changes in neighboring tendosynovial CD34+ dendritic progenitor cells may control the circumscribed growth of FTS. GCT appears to also arise from proliferating tendosynovial FXIIIa+ dendrophages that instead undergo macrophage-like cytodifferentiation with little induction of other stromal cells. Thus, FTS and GCT appear to be divergent tissue responses to stress or trauma in tendosynovial tissues, both of which involve FXIIIa+ fibrohistiocytic mesenchymal cells. AbstractIn the first and second articles of this series, the pathophysiology and etiology of cumulative trauma disorders were discussed. These disorders can be prevented by the elimination or minimization of chronic repetitive motion. With the advent of motorized microtomes, elimination of the repetitive motion of microtomy became possible. If motorized microtomy is to be accepted by the histotechnology community, however, it must function as well as, or superiorly, to the conventional microtome. We have systematically assessed the quality of slides prepared with motorized microtomes and compared them to those prepared in rotary microtomes. We also addressed the effects of accurate tissue section thickness on the results of quantitative DNA and immunohistochemistry studies on estrogen receptor assay. (The J Histotechnol19:55, 1996)

AbstractEnvironmental concerns, safety and cost containment have made uS evaluate tissue preservation, processing and recycling. When we randomly reviewed a number of fixatives, Histochoice, Clarke, acetate buffered formalin, zinc formalin and neutral buffered formalin (NBF), we found that all gave good morphological detail on thin sections. However, thicker specimens (over 1 cm) were better preserved in the formalin fixatives. Zinc formalin gave excellent morphological detail but produced coarse nuclear detail for electron microscopy. Acetate buffered formalin was preferable to NBF because it could be neutralized and disposed of down the sink. We also found that the volume of fixative needed for tissue preservation was overestimated, as only enough for complete immersion of the tissue was sufficient. Reducing the number of changes of all solutions on the tissue processor and recycling the containers used in dispensing the formalin fixatives resulted in considerable savings both financially and environmentally.These measures resulted in a saving of 30% for our laboratory. This reduction could be even more dramatic for larger institutions. There would be not only a monetary gain but a reduction in health hazards. (The J Histotechnol19:65, 1996)

AbstractA 62 year old man with a mostly neurotropic, desmoplastic melanoma (DNM) of the lower lip is described. On superficial biopsy this nodular lesion was diagnosed initially as a plexiform neurofibroma with an infiltrative growth pattern. Persistence of growth of the neoplasm led to excisional biopsy 6 mo later, permitting a diagnosis of neurotropic desmoplastic melanoma. There may be distinct differences between those melanomas that are primarily neurotropic and those that are primarily desmoplastic. Immunohistochemical findings support neurotropism or neoplastic replacement of pre-existing nerves rather than neuronal or perineural neoplastic differentiation as the fundamental pathogenesis of this pattern of melanoma in this case.
