We reviewed retrospectively the clinical records of 30 cases of sporadicLegionella pneumophilainfection that occurred in Iowa between FY 1972 and 1978. Cases occurred throughout the year, most between May and December. Twenty-one male patients and 9 female patients ranging in age from 5–80 years were infected. Half the patients smoked or had an underlying illness; five were receiving corticosteroids or immunosuppressive therapy. Occupations and exposures related to hospitals, construction and travel were common; four patients had been exposed to birds. In addition to L.pneumophilainfection, six patients had evidence of infection with a viral, mycoplasmal, bacterial, mycobacteriat or fungal pathogen; three had had preceding dental infections. Twenty-seven cases were pneumonias visible on radiographs. Fever, cough, chills, myalgia and rales occurred in over half the cases. Headache, gastrointestinal symptoms and encephalopathy also were seen. Upper respiratory symptoms were uncommon. Urinalysis and blood studies often suggested renal and hepatic involvement, but other routine laboratory diagnostic tests were not helpful. All but two patients were hospitalized; seven required intensive care. The median duration of hospitalization was 12 days. Two patients who did not receive erythromycin or tetracycline therapy died.

Polymorphonuclear leukocyte chemotaxis was investigated in 35 patients with recurrent polyserositis during attacks and during spontaneous or colchicine-induced remissions. Chemotaxis was found to be unchanged in the attack-free period in untreated patients, increased by about 50% during attacks, and decreased by about 50% during colchicine treatment.

Concentration of cefaclor, a new oral cephalosporin, was measured in prostatic tissue of ten patients undergoing suprapubic prostatectomy. The average prostatic tissue concentrations were 0.51 — 0.22 microgram and 0.74 — 0.67 microgram per gram of tissue following the oral administration of 250-milligram and 500-milligram doses, respectively. The prostate/plasma ratios of cefaclor were approximately 0.7 indicating no evidence of accumulation of the drug in prostatic tissue. Levels of cefaclor achieved in human prostatic disease are equal to or less than the minimum inhibitory concentration of most strains of known facultative bacterial pathogens associated with prostatitis.

The serum levels and urinary excretion of magnesium and copper were studied in 66 patients with either acute myocardial ischemia or myocardial infarction. Serum for magnesium and copper determinations was obtained daily for three days. The initial serum magnesium levels were normal in patients with ischemia but were low in some patients with myocardial infarction. Patients developing ventricular arrhythmias with myocardial infarction showed the lowest levels of serum magnesium. Copper in the serum appeared elevated in patients developing acute congestive heart failure but the elevation was not statistically significant. These data indicate that a decrease in serum magnesium as evaluated may be associated with ventricular arrhythmias in patients with myocardial infarction; thus continued observations of magnesium levels in the myocardial infarction patient is warranted.

The successful management of esophageal perforation requires prompt diagnosis and surgical intervention. The hazards of delayed recognition of esophageal perforation have been recently emphasized. A case of unsuspected esophageal perforation detected by computerized tomography of the chest (CT) is described and illustrated. CT is an effective technique for displaying mediastinal and complex pleural-parenchymal abnormalities and often provides information which is of diagnostic value.

Infections of the cardiovascular system, besides involving both natural and prosthetic heart valves, vascular grafts, and indwelling venous and arterial cannulas, may cause mycotic aneurysms. The latter frequently complicate endocarditis; however, they may occur as isolated phenomena. Enterobacteriaceae are uncommon etiologic agents in vascular infections; however, a patient is reported who presented initially with a bacteremia due to Yersinia enterocolitica biogroup 4 and despite antimicrobial therapy developed a mycotic aneurysm of the left internal carotid artery. Clinical manifestations, pathogenesis, and treatment of yersiniosis are reviewed.

A case report and review of 13 cases ofPasteurelia multocidaempyema in the English literature demonstrate the apparent increased pathogenicity of this organism in patients with chronic lung disease. The infections were marked by their indolence, minimal febrility, blood-tinged pleural fluid, and predilection for the elderly. The was no characteristic roentgenographic pattern. Animal exposure commonly preceded the illnesses. It is postulated that the disease occurs in patients with impaired pulmonary defenses who acquire the organism via the inhalation of infectious droplets aerosolized from the oropharynx of animals whereP multocidais part of the normal flora. Although most isolates were penicillin sensitive, there was significant mortality.

The atypical measles syndrome is a relatively new disease that was first recognized 15 years ago. Initially, it occurred in children who were exposed to wild measles virus several years after they were immunized with killed measles vaccine. It was characterized by a two- to three-day prodrome of high fever, cough, headache, and myalgia followed by a rash that resembled Rocky Mountain spotted fever, scarlet fever, or varicella and associated with roentgenographic evidence of pneumonia with or without pleural effusion. This report highlights three unusual manifestations of this syndrome: 1) transient hepatitis, 2) persistence of pulmonary lesions for several years, and 3) occurrence of excessively high measles hemagglutination-inhibition antibody titers. Today, this syndrome occurs predominantly in adolescents and young adults.
