Abstract:Direct determinations by radioiodinated human serum albumin (RISA) of plasma volume were made in normals, patients with uncomplicated polycythemia vera, and in patients with erythremia associated with chronic lung disease. The results show a moderately severe contraction of plasma volume in both erythremic states, evidently a compensatory consequence of red cell volume expansion. The hematocrit, reflecting both red cell and plasma volumes, may therefore be misleading as to the status of the erythremic condition, and therapeutic maneuvers and responses thereto should be determined on the basis of repeated direct measurements of the red cell volume in erythremia.

Abstract:Procedures currently available to detect bile acid deconjugation are technically difficult and are not routinely performed in clinical practice. We have assessed bile acid deconjugation by measuring14CO2specific activity of breath samples taken at intervals following the oral administration of 1-14C glycocholic acid and of 1-14C taurocholic acid. The test is based on the fact that during deconjugation, the amino acid and cholic acid moieties are split and the labeled carbon is converted to14CO2and exhaled. Controls and a number of patients with various malabsorption syndromes have been studied. Included were patients with bacterial overgrowth syndromes, ileal bypass, gastrocolic fistula, regional ileitis, cirrhosis of the liver, and chronic pancreatitis. The results indicate that breath analysis provides a simple, inexpensive and reliable screening technique for the detection of bile acid deconjugation.

Abstract:The pharmacokinetics of orally administered amoxicillin and ampicillin were compared in a crossover manner among healthy volunteers. All subjects received four one-week regimens consisting of 500 and 250 mg of amoxicillin, each given every eight hours, and 500 and 250 mg of ampicillin administered every six hours. Blood and urine were assayed for antibiotic activity on the first and eighth day of each drug course. In the 250 mg trial, the peak levels of amoxicillin in serum were twice those of ampicillin (4.3 and 5.6 µg/mlvs2.4 and 2.8 µg/ml) and were associated with greater retrieval in the urine of the former drug. At the higher dose level, however, the maximum concentrations in blood of amoxicillin (7.8 and 7.6 µg/ml) were not significantly greater than those of ampicillin (5.9 and 6.3 µg/ml). In addition, their rates of urinary recovery were similar.The antibacterial spectra of the two are found to be quite alike in their markedin vitroactivity against strains of penicillin G-sensitiveS. aureus, Streptococcus pyogenes, D. pneumoniae, H. influenzae, Salmonella,P. mirabilis, N. gonorrheaeand enterococci. Isolates of Klebsiella, indole-positive Proteus species, enterobacter and penicillinase-producingS. aureuswere highly resistant to both compounds.

Abstract:A radiological sign is described in lead poisoning to which very little attention has been given in the medical literature, and, in our opinion, is more reliable for early diagnosis of lead poisoning. This consists of heavy lines of increased density at the iliac crest which is only seen with heavy metal ingestion.

Abstract:A patient with hereditary spherocytosis developed severe anemia associated with a vivax malaria infection. Following recovery, splenectomy was performed and five months later a relapse of vivax malaria occurred. In contrast to the initial episode, anemia did not develop during the course of the relapse. The removal of the spleen which was the site of destruction of the highly vulnerable red blood cells appears to account for the different clinical pictures. This case points up the increased danger of malaria in unsplenectomized individuals with hereditary spherocytosis.
