Cefazolin, a new derivative of 7-aminocephalosporanic acid, was evaluated in 31 patients. Among 18 patients with acute respiratory tract infection and adequate follow-up, 15 were cured, one showed improvement, and two were therapeutic failures. Nine patients with soft tissue infections were cured and two had incomplete healing of infected lesions. Intramuscular administration of cefazolin was well tolerated in 11 of 25 patients and intravenous administration was well tolerated in 10 of 13 patients, Eosinophilia, thrombocytosis, and direct Coombs' positivity developed during therapy in three, seven, and four patients, respectively. Elevations of alkaline phosphatase and serum glutamic pyruvic and oxaloacetic transaminases were observed during therapy in four, five, and five patients, respectively. No other laboratory abnormalities developed. Serum and urine concentrations of cefazolin were similar to or greater than those reported with comparable doses of cephalothin or cephaloridine.

A model system was constructed to study the kinetics of an antibiotic, cephaloridine, in wounds. The antibiotic was administered: (1) by constant infusion with an initial priming dose; (2) in a single rapid injection; and (3) in two rapid injections given six hours apart. The concentration of cephaloridine was measured in plasma and wound fluid and the cumulative amount was determined in the urine. The results of the analysis of data based on a mathematical model and aided by computers show that the transport of cephaloridine between plasma and wound is essentially passive. There is an early advantage in giving large intravascular injections of antibiotic as compared to constant infusions. The implication of the wound size on the antibiotic concentration in the wound is discussed. Estimations of antibiotic concentration in human surgical wounds are made.

The antibacterial activity of the combination of carbenicillin with cephalothin, in a ratio of 4: 1, was studiedin vitroand in volunteers. It was shown that a mixture of 100µg/ml of carbenicillin with 26 µg/ml of cephalothin inhibited 97 per cent of 535 gram-negative rods isolated from patients in a cancer hospital. Sera obtained from patients receiving the combination were inhibitory and bactericidal at a higher dilution than sera obtained from the same patients receiving carbenicillin or cephalothin alone (the geometric means were 1/16 and 1/8, respectively). It is concluded that carbenicillin plus cephalothin might prove to be effective as an empiric therapy for suspected gram-negative sepsis.

An experimental technique for inducing coccidioidal meningitis in rabbits has been developed and a method described for obtaining cerebrospinal fluid (CSF) from these small animals. The CSF changes which occurred simulated the usual clinical tests for meningitis in humans. Two newer diagnostic parameters were evaluated in these animals, the coccidioidin latex agglutination (CM test) and the lactic dehydrogenase (LDH) isoenzyme fractions. These tests are more specific than the usual CSF test and the changes appear to occur early in the disease.

Fifty-three patients with acute falciparum malaria were studied for evidence of disseminated intravascular coagulation (DIC) on admission and during treatment. No one had the classical syndrome of DIC with bleeding. Abnormal coagulation tests sporadically observed were not accompanied by abnormalities of other tests that would establish the presence of DIC. During therapy there was a progressive decline of the mean fibrinogen concentration, and platelet counts rose significantly at the end of hospitalization. One patient who developed cerebral malaria had equivocal evidence for DIC, and he recovered fully without anticoagulant therapy. The abnormalities of the coagulation tests in this study are explained by hypersplenism, drug toxicity, and liver disease. Heparin therapy is discouraged.

Present regimens for determining the bioavailability of tetracycline antibiotics may be inadequate. When blood and urine levels of tetracycline HCI were monitored throughout a 96-hour treatment period, film-coated tetracycline tablets used in this study were found to be inferior to tetracycline capsules. This important difference would have been missed if the usual six-hour single-dose observations had been performed.

A patient is reported who sustained talc microembolization to the brain and retina as a consequence of intravenous injection of unfiltered aqueous suspensions of pulverized oral meperidine tablets. Following a six-month period of injecting three 50 mg oral meperidine tablets four times a day, the patient noted the onset of rapidly progressive visual blurring with blind spots in both visual fields. Associated with this was increasing difficulty with mental concentration and a decline in recent memory. Funduscopic examination revealed numerous punctate and flame-shaped hemorrhages as well as areas of arterial occlusion. Follow-up revealed clearing of the acute organic brain syndrome and gradual improvement in funduscopic findings.

A patient with myoglobinuria and hypernatremia is presented. The effect of hypernatremia on muscle metabolism is discussed, and hypernatremia as a cause of myoglobinuria is suggested.
