Thirteen individuals with Kartagener's syndrome from eight unrelated families have been studied. In view of the co-existence of chronic sinusitis, chronic serous otitis media, and chronic bronchial inflammation, it is apparent that this disease affects the entire respiratory mucosa, as well as normal organ asymmetry. The pathology of the resected lung lobes and sinus mucosa (both of which are normally lined with pseudostratified ciliated columnar epithelium) consists of nonspecific, chronic inflammatory changes; if bronchiectasis is present, it is usually the follicular type. While the primary etiology of the respiratory system involvement has not been found, several contributing factors have been studied, among them transient immunoglobulin deficiencies during childhood. The two youngest patients had transient immunoglobulin deficiencies of different severity within their first two years of life and continue to have low serum IgA levels. While eight of the 13 patients have complete situs inversus, bronchiectasis, and sinusitis, the other five patients represent phenotypic variations of Kartagener's syndrome and demonstrate that the underlying genetic abnormality has a pleiotropic effect. The heredity pattern in the eight families is compatible with an autosomal recessive mode of inheritance.
