A retrospective review of 766,742 hospital admissions was performed between 1966 and 1986 at the University of Iowa Hospital for the diagnosis of congenital syphilis. Although 88 individuals were identified with this diagnosis, adequate treatment was documented in only 33 (38%). Thirty-nine of the 88 individuals identified were initially seen for visual complaints by the ophthalmology department. We recommend that all physicians increase their index of suspicion for this disease, and institute appropriate therapy and follow-up if late congenital syphilis is diagnosed.

Three patients with retinal vasculitis are reported who were found to be seroreactive for Lyme borreliosis. Careful investigation revealed no other apparent etiology for the angiitis, and improvement of the retinal vasculitis on tetracycline therapy was documented by fluorescein angiography in one of them. Two cases of retinal vasculitis were presented at the International Conference on Lyme Borreliosis in Stockholm 18–21 June 1990, and two cases of cerebral vasculitis due toBorrelia burgdorferihave been published. To our knowledge, this is the first published report of retinal vasculitis occurring in patients seroreactive for Lyme borreliosis. Although further investigation will be necessary to prove a cause-and-effect relationship, ophthalmologists encountering patients with otherwise unexplained cases of retinal vasculitis, or Eales disease, are encouraged to study these patients carefully for the possibility ofBorrelia burgdorferiinfection.

Magnetic resonance angiography (MNA) was compared to conventional intra-arterial digital subtraction angiography in a patient with an isolated third nerve palsy caused by a posterior communicating artery aneurysm. The technique of MRA is briefly discussed.

A young woman presented in the third trimester of pregnancy with a rapid onset of bitemporal hemianopia and reduced visual acuity caused by an unusual steroid responsive suprasellar tumor. A computerized tomography scan revealed a 2-cm suprasellar mass which was thought to be a tuberculum sellae meningioma. Surgery was delayed because of pregnancy. A short course of high-dose steroids was given to promote fetal lung maturity. This produced an unexpected and dramatic resolution of the field loss. As the steroid dose was reduced, the visual fields deteriorated, necessitating a craniotomy. The histology revealed lymphocytic hypophysitis, a rare but distinct clinicopathological entity affecting the anterior pituitary lobe. The significance of the suprasellar site and a possible role for the use of steroids in the preoperative management of this condition is discussed.

Histiocytic infiltration of the optic chiasm is rare. We report a patient with a seizure disorder on anticonvulsant therapy, who developed systemic reactive histiocytosis. Treatment with splenectomy, corticosteroids, and anticonvulsant medication change resulted in clinical remission. The patient later developed recurrent lymphohistiocytosis restricted to the optic chiasm. Extensive reevaluation yielded no evidence for infectious or malignant etiology. Radiation therapy and withdrawal of anticonvulsant therapy resulted in clinical remission. We conclude that both phenytoin and phenobarbital may trigger a lymphohistiocytic process and that infiltration of the visual pathway is possible.

The authors describe a case of histiocytosis X of the petrous bone presenting as petrous apex syndrome. The clinical, radiological, and histopathological features are discussed, and the pertinent literature is reviewed. To our knowledge, histiocytosis X of the petrous apex has previously been reported to cause this syndrome in only two cases.

The schneiderian or inverted papilloma is an infrequent and usually benign tumor of the paranasal sinuses. It most commonly manifests as unilateral nasal obstruction. This is a case of an inverted papilloma in a 92-year-old woman who presented with severe proptosis, loss of vision in the left eye, and rhinorrhea. The mass was visible in her left nostril; neuroimaging studies showed that the lesion extended from the left paranasal sinus into the frontal sinuses. In addition, there was secondary spread into the cranial cavity and brain substance.

Meibomian gland carcinomas are rare, slow-growing masses that clinically mimic several benign and malignant processes. We report a patient with sebaceous carcinoma of the meibomian gland with intracranial extension and dural invasion, and discuss the clinical and pathological features of this uncommon entity.

Atrophy of orbital fat is a reported complication of localized scleroderma but not of systemic scleroderma. Here we present a case of systemic scleroderma with unilateral enophthalmos. Orbital fat atrophy was the presumed cause on computerized tomography.

Simultaneous recordings of the pattern visual evoked potential and the pattern electroretinogram were recorded in a case of unilateral optic neuritis following a bee sting on the eye. The patient presented with severe eye pain, apoplectic visual loss, acute optic disc swelling and minimal signs of ocular inflammation and eyelid edema. The vision failed to recover. The electrophysiological recordings initially showed a delay in the P100 wave of the pattern visual evoked potential. On follow-up, a normal latency and amplitude of the P50 wave of the pattern electroretinogram was recorded with the flattening of the N95 component, but the pattern visual evoked potential was absent. The data suggests that the optic nerve was demyelinated acutely, and that subsequently axonal loss and degeneration of retinal ganglion cells occurred.
