Although the central retinal artery and its collateral circulation, especially within the anterior optic nerve, have been extensively studied, there continues to be controversy regarding the existence and significance of anastomoses between the central retinal artery and ciliary circulation. We present the case of a 16-year-old boy who underwent total resection of an optic nerve glioma beginning proximally at the chiasm and ending distally flush with the globe. After resection, heavy bipolar cautery was applied. Postoperatively, normal central retinal artery circulation was documented angiographically, suggesting that, at least in some cases, significant anastomoses can develop between the central retinal arterv and ciliary circulation in the laminar and prelaminar areas.

A 44-year-old man developed a complete homonymous hemianopia 2 days alter imdergoing; Chiropractic cervical manipulation. Thromboembolism from the vertebrobasilar circulation—as the consequence ot trauma to the vertebral arteries by adjacent bones, museles, and ligaments during twisting and extension of the neck—is the probable mechanism for occipital infarction and other strokes in this and previous cases. Other neuro-ophthalmic mamtestations reported include Homer's syndrome and sixth nerve and gaze palsies.

Pathologic and Ultrasonographic feature of cavernous and capillary hemangiomas were correlated with the Computed tomographic (CI ) charaacteristics of these two tumors to show that these hemangiomas can be differentiated by their contrast enhancement characterstics Specifically, all five cases of adult cavernous hemangiomas were enhanced proportionately less than extraocular muscles, whereas capillary hemangiomas were enhanced proportionately more than extraocular muscles. The capillary hemangioma that demonstrated the lowest relative enhancement was in an older child. It may have represented a more mature form of a capillary hemangioma. We correlated these CT findings with the ultrasonographic and pathologic features to explain the differences m these CT characteristics.

A young, healthy woman underwent craniotomv to the resection of a corpus callosum arteriovenous maltormation. She was slow to awaken from anesthesia and was later noted to have a total external ophthalmoplegia. Her mental status quickly improved but the total ophthalmoplegia showed only minimal improvement lor 2 weeks. After ruling out possible cerebrovascular and anesthetic complications, we concluded that the oculomotor impairment was due to postoperative toxic levels of Dilantin. Toxic Dilantin levels are known to rarely produce total ophthalmoplegia. In previously reported cases, this has alwavs resolved with normalization of the serum Dilantin levels. This case represents the first report of Dilantin-induced ophthalmoplegia that took at least 3 months to resolve; it provides a detailed documentation of the course of ocular findings.

A 37-year-old man developed periodic esotropia with 4-dav cycles. He was unique because of his age, negative tamilv history for strabismus, bilateral optic atrophy and absence of light perception in deviating eye. Adilt-onset evelic esotrophia in a blind eye is reported. This atterent detect may lead to secondary changes in the central nervous system and fusional potential is not necessary for cyclic esotropia to occur.

Optic neuropathy secondary to lymphomatous infiltration of the optic nerve developed in a patient who was in complete remission from treatment of a non-Hodgkin's lymphoma. This is the first reported case of an isolated optic neuropathy occurring in a patient in clinical and laboratory remission of non-Hodgkin's lymphoma. Recognition that such a complication can occur is important in that prompt treatment may lead to reversal of visual loss, as it did in this patient.

In its early stages, septic cavernous sinus thrombosis may be difficult to differentiate from orbital cellulitis. We describe a case in which digital subtraction orbital venography proved to be a superior technique for clarifying this clinical dilemma, and we stress the clinical importance of making this differentiation.

The case of a 67-year-old man with acute anterior ischemie optic neuropathy occurring in conjunction with the ocular ischemie syndrome (anterior segment and Hindus manifestations secondary to severe ipsilateral carotid artey obstruction) is presented. Although the association appears to be quite uncommon, occasionally a carotid artery obstructive lesion may predispose a patient to the development of acute ischemie changes in the anterior optic nerve.
