摘要:
To our knowledge, bilateral syphilitic optic neuritis has not been reported in conjunction with AIDS. We document a case of a bisexual man with AIDS whose vision deteriorated overnight from 20/20 in both eyes to total bilateral blindness as a result of syphilitic retrobulbar neuritis. The implications and management of leutic disease in immunocompromised individuals are discussed.

摘要:
Optic perineuritis is characterized by swollen optic discs in the absence of raised intracranial pressure and visual dysfunction. A patient with secondary syphilis who presented with these features is described. The need to recognize the spectrum of ocular and optic nerve involvement in secondary syphilis is emphasized.

摘要:
Symptoms of syphilis have evolved over the ages. In the 16th century, they were essentially cutaneomucous ones. While the intensity of the symptoms decreased localization of the infection in the various organs—mainly those of the cardiovascular and nervous system—gradually appeared. Over a period of years following the introduction of penicillin therapy, the cutaneous and visceral stages became less common. Reminder of the chief serological reactions and of some aspects of the experimental syphilis in the rabbit, similar to certain forms of human syphilis—a strong but late penicillin therapy has been proven ineffective in humans as well as in animals. It is to be regretted that numerous methods of using penicillin (all over the world), had been, from the beginning—except in a few cases—of a purely empirical nature. The authors emphasize—with proof to support their opinion—the failure of antibiotic therapy, which can explain the recent reappearance of hepatic and nervous localizations that had disappeared for 30 years. Despite views to the contrary the authors conclude that penicillin has in no way resolved all the problems raised by the treatment of syphilis.

摘要:
Fourth nerve palsy has been rarely seen following lumbar puncture, myelogram, or spinal anesthesia. We report a case of 4th nerve and 6th nerve palsies following spinal anesthesia. The 4th nerve palsy was best detected by using a Maddox rod. If all 6th nerve palsies occurring after spinal anesthesia were examined with a Maddox rod, more cyclovertical palsies might be discovered.

Three patients are reported who presented with primary position downbeat nystagmus without any other evidence of cerebellar dysfunction. After 2 weeks of abstinence from alcohol, the downbeat nystagmus resolved totally in two cases, and could be elicited only with head hanging in the third. Radiologic evaluation, including computed tomographic (CT) scan of the posterior fossa and craniocervical junction, were normal in each case. These are the first cases reported of reversible downbeat nystagmus secondary to alcohol intake. They suggest that a patient with downbeat nystagmus and a history of recent significant alcohol intoxication should be observed for resolution of this sign after a period of abstinence before extensive radiologic evaluation is undertaken.

摘要:
Congenital ocular motor apraxia is a distinctive disorder that may present initially as apparent cortical blindness, because of the patient's inability to make saccadic eye movements to fixate targets moving in the horizontal plane. Affected children develop a characteristic compensatory head thrust that allows them to fixate and allows the physician to make the diagnosis. Two patients, initially thought to be blind because of a lack of fix or following responses in the horizontal plane, are reported. Although head thrusts were not present because of inadequate head control, the diagnosis of congenital ocular motor apraxia was made because of other diagnostic features. The cases emphasize the need to consider congenital ocular motor apraxia in any child thought to be cortically blind.

An elderly woman with the persistent isolated symptom of monocular and binocular tilting of environmental objects, was found on magnetic resonance imaging to have compression of the lateral medulla by an ectatic vertebral artery. The double Maddox rod test showed no relative torsion of either globe although 5° of right excyclotorsion and left incyclotorsion (rightward ocular counter-rolling) was detected on testing with monocular viewing. There was no accompanying hyperdeviation, nystagmus or other neurologic signs. Symmetrical tilting of each globe associated with head tilt and skew deviation (the ocular tilt reflex) may be caused by lesions involving otolith connections. Isolated environmental tilt may herald the onset of lateral medullary infarction (Wallenberg's syndrome), or may be associated with vertebro-basilar insufficiency. Could compression of otolith connections in the medulla cause persistent ocular counter-rolling without accompanying neurologic signs?

摘要:
A 7-year-old girl presented with bilaterally dilated pupils, nausea, and vomiting 2 days after head trauma. Pilocarpine pupil testing led to the correct diagnosis of pharmacologic pupillary dilation from an unexpected and unusual source of plant poisoning,Solanum dulcamara(blue nightshade). In patients with internal ophthalmoplegia, awareness of the possibility of pharmacologic mydriasis and correct use of topical pilocarpine testing can preclude the necessity for neuroradiologic and invasive diagnostic studies, even in cases with atypical or complex presentations.
