摘要:
A 71-year-old man had sudden vision loss associated with headache. A temporal artery biopsy revealed a typical picture of giant cell arteritis. Subsequent steroid therapy failed to restore sight. A later blood culture contained spirochetes compatible withBorreliaspecies, and a silver stain of the temporal artery biopsy specimen demonstrated a similar spirochete. Treatment with i.v. ceftriaxone sodium led to some limited return of sight. To our knowledge, this is the first case report of a spirochete compatible withBorreliafound in a temporal artery biopsy specimen

A 35-year-old woman with metastatic breast cancer experienced acute monocular visual loss. Her fundus examination was normal. The patient died 48 h later, and autopsy demonstrated hematogenously disseminated aspergillosis. 摘要:
The presumed cause of anterior ischemic optic neuropathy is atherosclerotic vascular changes. Small-vessel occlusive vascular disease most likely causes occlusion of one or more posterior ciliary arteries, compromising blood flow to the optic nerve head and choroid. We present the case of a 59-year-old black man with cholesterol emboli within the retinal vasculature combined with a clinical picture of anterior ischemic optic neuropathy and choroidal nonperfusion demonstrated by i.v. fluorescein angiography. We believe this is evidence that, in a rare case, embolic phenomena may be the cause of anterior ischemic optic neuropathy

Sarcoid optic neuropathy without fundus lesions is unusual. We present a case of optic neuropathy with spontaneous remission in one eye that paralleled the clinical course of demyelinative acute optic neuritis. The fellow eye had disc swelling with normal visual acuity but with a large blind spot and distended optic nerve sheaths seen on ultrasonography. The diagnosis of sarcoidosis was made by characteristic chest roentgenographs findings and confirmed by skin biopsy

In the past, angiography was performed in all patients as part of the initial workup for isolated oculomotor paralysis, except patients older than 40 years with pupillary sparing. The pupil-sparing group was not subjected to angiography because of a low probability of cerebral aneurysm. It is believed that the case reported here constitutes the lower age limit (14 years) for documented, isolated oculomotor paralysis due to aneurysm. It is recommended that an angiogram not be a necessary part of the workup of patients 10 years old or younger

We report a patient with primary systemic amyloidosis and orbital involvement associated with ophthalmoparesis. We support this with the first demonstration of eye movement recordings and magnetic resonance imaging (MRI) of such a patient. The patient presented with a restrictive ophthalmoparesis. Eye movement recordings documented slowed horizontal saccades. MRI of orbital structures suggests that all extraocular muscles were enlarged, with possible involvement of their tendinous insertions

Two young adults, aged 24 and 31, had a long history of small, poorly reactive pupils. There was no history of large pupils, and a review of old photographs confirmed 10 and 5 years, respectively, of miosis. Both were found to have bilateral tonic pupils that were supersensitive to diluted pilocarpine. Although it is possible that they had an unusually early onset of bilateral Adie's syndrome with dilated pupils that was not noticed, it is suggested that some patients might have primary miotic Adie's pupils without ever passing through a mydriatic phase
