An attempt was made to determine whether or not the steroid hormones, aldosterone and betamethasone, increased the Na‐K‐ATPase activity and the surface density of baso‐lateral cell membranes in the juxtamedullary proximal convoluted tubules of young and adult mice. The results showed that aldosterone and betamethasone increase the enzyme activity in young mice but not in adult mice. However, the surface density of the baso‐lateral cell membranes did not increase either in the young or in the adult mice. Further, the induction ratio for the Na‐K‐ATPase activity did not differ between the two hormone‐treated groups in young mice. The prognosis of coronary artery aneurysms complicating Kawasaki disease was assessed by serial coronary angiography in 33 patients. They underwent initial angiography with four months, and one or two follow‐up angiographies between 11 months to 6.3 years after the onset of the illness. 'Ihese patients had 69 coronary aneurysms at the initial study. The maximal diameter of the aneurysms was expressed as a percent of the normal value. Of 44 aneurysms with a diameter less than 300% of normal, 43 were found to have regressed on follow‐up. Of 13 aneurysms with a diameter 300 to 400% of normal, four were proved to have regressed, eight persisted and one stenosed. Of 12 aneurysms with a diameter 400% of normal or more, one was found to have regressed, four persisted, two stenosed and five obstmcted. Plasma concentrations of human atrial natriuretic peptide (hANP) were determined in children with persistent hypertension and in age‐matched normotensive controls. We studied 40 children 6–7 years of age (Group A), and 74 children 13–14 years of age (Group B). There was no significant difference in plasma hANP concentration between hypertensives and normotensives in group A. In group B, the plasma hANP concentration in hypertensives (86.5 ±44.9 pg/ml: mean±SD) was significantly higher than in normotensives (58.8±40.4 pg/ml) (p<0.01). In group B, 24‐hour urinary excretion of sodium and fractional excretion of sodium were not significantly different between hypertensives and normotensives. The plasma hANP correlated significantly with the fractional excretion of sodium (p<0.05)and the 24‐hour urinary excretion of sodium (p<0.01) in normotensives. No such correlation was evident in the case of the hypertensives. The pathogenesis of increased plasma hANP concentration in older children with persistent hypertension remains to be investigated. Effects of postural change on plasma catecholamine levels were examined in 53 children with orthostatic dysregulation (OD). Special interest was focused on plasma dopamine and dopamine‐β‐hydroxylase (DBH) activity, since one patient showed a prominent rise in plasma dopamine from 822 pmol/L to 126 nmol/L in postural change. She had a wide fluctuation of plasma dopamine from 209 pmol/L to 305 nmol/L during 12 hours of observation period, but plasma norepinephrine and epinephrine remained within the normal range as well as urinary excretion of catecholamines. Of 52 children with OD, 11 had a marked increase in plasma dopamine, whereas 11 showed a reduction of it by postural change. Plasma DBH activity was significantly decreased in the former (81±14 nmol/h/ml), while it was elevated in the latter (320±48 nmol/h/ml, P<0.001 vs the former), although DBH activity in individuals was not affected by postural change. A characteristic sinusoidal tcPO2pattern was observed in 17(27%) of 62 patients with congenital heart disease manifested by cyanosis or heart failure during the neonatal period. All of these 17 patients were definitively diagnosed by cardiac catheterization, as having 8 PPA; 3 ToF; 2 TCA+VSD+PA; 2 asplenia with PA; 1 TGA+VSD+PS; and 1 TA (no TCA type). The transcutaneous oxygen pressure pattern of these patients showed a sinusoidal change with a cycle length of 6–20 min with the maximum pressure not exceeding 50 torr and the minimum pressure occasionally close to 0 torr. This sinusoidal tcPO2pattern was associated with various types of ductus‐dependent congenital heart diseases and hence may be of diagnostic value. Treadmill exercise electrocardiography (TE) was recorded in 50 patients to evaluate the incidence of exercise‐induced ventricular arrhythmias (VAs) in a group of patients who underwent corrective surgery of tetralogy of Fallot (TF) more than four years previously. The results of rhythm evaluation were correlated with cardiac catheterization data as well as clinical information such as the age at surgery and the interval from surgery. Of the 50 patients tested, ventricular premature contraction (VPC) was confirmed in 12 patients (24%) on TE, while five patients (10%) demonstrated VPC on the standard ECG. The interval from surgery was significantly longer in those patients with VA (Group 1) than those without VA (Group 11) on TE (p<0.05). There was no significant difference of the age at corrective surgery between the two groups. In group I, the preoperative hemoglobin level was higher (p<0.05) and the right ventricular ejection fraction was lower (p<0.01). Twenty‐eight drugs, including cephem antibiotics and anti‐inflammatory agents currently used or considered potentially useful in neonatal intensive care nurseries in Japan, were examined to determine their influence on albumin and bilirubin interaction by means of a glucose oxidase ‐ peroxidase method, using an automated analyzer (Arrows) for unbound bilirubin (U.B.). The apparent binding constant for drugs to the high‐affinity site on albumin (KD) was determined. Of cephem. antibiotics, latamoxef sodium (LMOX) and cefazolin sodium (CEZ) were found to displace bilirubin from albumin (KD= 5.9 times 103M‐1and 4.5 times 103M‐1, respectively) as strongly as Na salicylate (KD= 6.8 times 103M‐1). Mephenamate and indomethacin, which are used for medical closure of patent ductus arteriosus in premature infants, were also found to be stronger bilirubin displacers (Kn = 1.3 times 105M‐1and 1.2 times 105M‐1, respectively) than sulfisoxazole (KD= 1.6 times 104M‐1). Maximal displacement factors (MDF's) were also estimated in reference to protein binding (%) and effective serum concentration (M) of each drug in human adults. Of these drugs, mephenamate showed a higher risk of bilirubin displacement (MDF = 3.79) than sulfisoxazole (MDF = 2.58) and LMOX had a higher risk of displacement (MDF = 1.97) than Na salicylate (MDF = 1.85). On the other hand, indomethacin and CEZ showed minimal effects on displacement at therapeutic levels (MDF = 1.03 and 1.00, respectively). Neither the hemolytic activity nor the protein level of the seventh component of serum complement (C7) was detectable in an 8‐year‐old girl with nephritis, but in her parents and her brother, they were about half of the normal level. The patient was a homozygote type with a complete deficiency of C7 while her parents and brother were all heterozygote type with a partial deficiency of C7. We studied the immunomodulatory role of growth hormone (GH) treatment in GH‐deficient children. CH potentiated natural killer (NK)cell activity and the PHA and Con‐A lymphocytoproliferative response. The bone mineral status of very low birthweight (VLBW) infants fed exclusively their own mother's milk (group I) was compared with that of VLBW infants fed mother's milk in the initial 4 weeks followed by a 1:1 mixture of mother's milk and preterm formula containing high phosphorus (P) and calcium (Ca) (group II). In both groups, most infants showed a biochemical picture characteristic of phosphorus deficiency syndrome by the fourth week. Thereafter, serum alkaline phosphatase activity (ALP) decreased and serum P increased in all group II infants. Conversely, serum ALP rose and hypophosphatemia persisted in most group I infants. Group II had a significantly higher serum P at weeks 8 and 12 and a significantly lower ALP at week 12 than group I. Furthermore, group II had a lower incidence of severe radiographic abnormalities than group I at week 12. We confumed previous observations that VLBW infants fed exclusively human milk require P and Ca supplementation to prevent metabolic bone disease of prematurity.
