Grommet insertion: some basic questions answeredA very large number of grommets are inserted to improve middle ear ventilation in children. A retrospective survey of 1011 admissions for myringotomy and/or grommet insertion has been used to answer some basic questions about such admissions and the fate of grommets inserted. No attempt has been made to assess the benefits of this mode of treatment. The average number of admissions per child was 1.6, with 64% being admitted only once. A child aged 3–4 years when first listed had the greatest chance of repeated admissions. Nearly 50% of the grommets had been extruded within 6 months and 80% within 1 year. Children put on the waiting list because of middle ear effusions present in summer months had a greater chance of spontaneous resolution in the subsequent few months than those listed in winter months. Significant complications occurred in 2.2% of grommet insertion

Nasolabial cystThe nasolabial cyst is a rare lesion. Eight cases seen at the Birmingham and Midland ENT Hospital over 10 years were studied. Little diagnostic difficulty was encountered and all were successfully treated by surgical excision. A brief review of the literature is included.

The morphogenesis of the tympanic part of the temporal boneThe aim of this report is to describe the relative movement of the tympanic membrane and its supporting structures during embryonic and postnatal life, leading to its final position in a vertical plane. Previous studies were based on ultra thin sections which led to changes in orientation during development being missed. The importance of the present description is to provide a baseline with which abnormal ear development in certain congenital syndromes may be compared.

Mucociliary function in the nasal cavity of children and adolescents with cholesteatomaTo assess the mucociliary function in the nasal cavity of children with middle ear cholesteatoma, 31 children were tested using the saccharin method. As controls, 23 children with chronic otitis media and 29 children with no middle ear pathology were tested. It was found that children with cholesteatoma more often had mucostasis.

The design of phase III palliative chemotherapy trials in head and neck cancerThis paper describes a series of 2447 patients with squamous carcinoma of the head and neck treated personally over 22 years; 276 patients with advanced untreatable disease and 498 with recurrent untreatable disease are analysed. The median survival for the first group was 13 weeks and for the second 11 weeks. The data fitted an exponential curve. Prognostic factors included general condition and N status for the first group, and site of the primary tumour, site of the recurrence and length of time to recurrence for the second group. However, these factors only explained 5% of the variance; the remaining 95% was unexplained, emphasizing the futility of stratification for prognostic factors, and the necessity for large trials. Using the above facts the number of patients required for a trial, and the length of the trial, can be worked out, and tables of these are given.

Unusual polypoidal tumours of the nasal cavity: a clinicopathological review of 18 casesA variety of nasal conditions may present as simple polypi. Clinically, these lesions, some very rare, present a diagnostic problem. Eighteen cases of unusual lesions which were thought to be simple polyps on clinical grounds are reviewed with the relevant literature. The series includes 2 olfactory neuroblastomas, a haemangiopericytoma and a basal encephalocoele. Atypical features from the history and examination are highlighted. It is emphasized that every nasal polyp must be submitted for histological examination and the pathology report consulted when the patient is reviewed. Failure to do so delays appropriate treatment.

A survey of the non‐attendance rate at the ENT clinic of a district general hospitalA survey of the number of patients who failed to keep outpatient appointments at the ENT clinic of a rurally situated district general hospital (Bangour General Hospital, West Lothian) was carried out. The results were compared with non‐attendance rates of the general medical clinic of the same hospital and of the ENT clinic of a nearby urban hospital (the Royal Infirmary in Edinburgh). A particularly high non‐attendance rate was demonstrated for the ENT clinic of the district general hospital. The reasons for this are discussed in the light of currently available literature on the ‘no show’ patient and possible means of improving the situation are

Serum glycoproteins and prognosis in cancer of the head and neckAlpha 1 acid glycoprotein, alpha 1 antitrypsin, haptoglobin, alpha 2 HS glycoprotein and prealbumin were studied in 44 patients with cancer of the head and neck and in a control group. The three primary glycoproteins were considerably elevated in the patients, particularly in stage IV. In the group of patients who died before 30 months after treatment the haptoglobin was significantly elevated compared to those who survived beyond this point. Only values for the alpha 1 acid glycoprotein greater than 150 mg/dl were a prognostic factor of similar precision to that of stage IV. This correlation could not be shown for the other glycoproteins.

Benign lymphoepithelial lesion: a less than benign diseaseThe precise clinical correlate of the benign lymphoepithelial lesion is unclear. Thirty‐six cases of benign lymphoepithelial lesions (BLL), reported to the British Salivary Gland Tumour Panel between 1971 and 1984, have been reviewed. Eighty per cent arose in the parotid gland and 20% were bilateral; 83% were in females and the mean age at presentation was 55.26 years. Only 50% presented with, or developed, symptoms of sicca complex, Sjogren's syndrome or related autoimmune disease. Two cases of BLL had, or went on to develop, extra salivary lymphomas and 5 cases of BLL had lymphomatous change in the initial biopsy. A further case had carcinoma within the benign lymphoepithelial lesion. None of those who developed lymphoma had sicca or Sjogren's syndrome but 3 of them had rheumatoid arthritis. The incidence of lymphomas (salivary or extra‐salivary) in this series is very much higher than that reported in Sjogren's patients and amounted to 20% over
