In a study of the value of ultrasound in staging patients with head and neck malignancies, we performed ultrasound of the neck. The results of this investigation were compared with palpation. A fine needle aspiration biopsy and/or histologic examination was carried out on lymph nodes which were found. One hundred and six patients were included in this study. In 44 of the patients no lymph nodes could be detected, either on palpation or by ultrasound examination. In the other 62 patients all palpable lymph nodes were also demonstrated by ultrasound. However, in 20 patients with negative palpatory findings, ultrasound revealed lymph nodes: 11 metastases and 9 benign nodes. In 40 patients an ultrasound guided fine needle spiration biopsy (UGFNAB) was performed. In 85% of these patients a cytological diagnosis could be made. From these results we conclude that ultrasound and UGFNAB are of considerable value in staging head and neck malignancies.

This study evaluated oral tranexamic acid as an adjunct in controlling epistaxis and preventing or reducing recurrent epistaxis. Patients entered into the trial were randomized in double blind fashion to placebo or tranexamic acid 1 g, 3 times daily. Treatment continued for 10 days. The patients were reviewed daily and any rebleeds categorized into minor, moderate or severe according to length and briskness of bleed and subsequent treatment. Of the 89 patients who completed the course of tablets, 25 (57%) in the placebo group and 21 (47%) in the treatment group had a rebleed. More patients in the placebo group had minor and moderate rebleeds, but the same number of patients in the placebo and treatment groups had severe rebleeds; this difference was not statistically significant. Oral tranexamic acid is, therefore, of no proven value as an adjunct in the treatment of epistaxis in patients requiring hospital admission.

The effects of inhalation of L‐menthol, D‐isomenthol and D‐neomenthol, upon nasal resistance and sensation to airflow were investigated in 40 subjects. A 5‐year longitudinal study into hearing levels of 261 randomly selected elderly people was performed between 1968 and 1973. From 47 survivors, 37 subjects have been traced and their hearing retested with pure tone audiometry. The age range of those followed up was 80–85 years. The results indicate that the rate of hearing loss found after 5 years remained the same over the next 12 years. The deterioration rate is very much greater for the higher frequencies with losses of 1.89 dB annually at 6 KHz compared with 0.35 dB per annum for 1 KHz. This study investigates postoperative hoarseness by comparing the patient's subjective assessment of change in voice following intubation with objective measures made using the laryngograph. Twenty‐five patients admitted for routine surgery were assessed pre and postoperatively. An Fx histogram was recorded on the laryngograph. A linear analogue self‐assessment scale was used to quantifiy hoarseness subjectively. The objective measure of hoarseness (spread or standard deviation of Fx histogram) correlated well with the patients' subjective assessment of hoarseness. Six patients showed a bi‐ or trimodal histogram following intubation. This is known to represent asymmetric muscular activity. It is concluded that change in voice following intubation correlates well with the measures of vocal fold trauma. These measures can be recorded as the change in spread, or standard deviation of the Fx histogram. The transient postoperative hoarseness following intubation is due to laryngeal damage. In 31 patients a symptomatic septal perforation was repaired using an inferior‐turbinate flap originally described by Masing. 1 In approximately 50% of the cases the perforation was closed for two‐thirds or more. Postoperative scarring causing nasal obstruction or alar asymmetry occurred in 21% of the cases. In a study of 26 normal cadaveric temporal bones, a significant correlation was found between the area of the mastoid air spaces assessed from a conventional lateral radiograph and the mastoid volume as determined using high resolution computed tomography. The relationship, which was found to be linear, is of the form V = 0.82 A+ 0.13. The correlation coefficient is 0.95 with a residual standard deviation about the regression of 1.14 cm3.
