SUMMARY.Fifty‐three cases are reported. Thirty‐five were offered psychotherapy but 10 left treatment early. Nine patients refused psychotherapy and nine were unsuitable. Outcome was measured by change in contacts with local health services and general practitioners for an equivalent time period before and after psychotherapy or assessment. Of the treatment group, 17 recovered, three were improved, one was unchanged and four were worse (P<<0.00 1). There was a reduction in general practice use by those who received even brief treatment. Those with anxiety symptoms improved most. The‘refuser’group contained more depressed patients and patients with alcohol or personality problems did less well. Variables not associated with outcome were: duration of treatment, previous treatment, therapist experience, therapist gender, number of cases previously seen and premature termination. SUMMARY.Utilising detailed clinical material from two analytic cases, this paper delineates a search to understand a difficult clinical situation in which the analyst's capacity to think and to analyse during the analytic hour are experienced as being paralysed by virulent, primarily unconscious, sadistic attacks on the part of the patient. The intensity of the counter‐transference experience involving such attacks on the analyst's capacity to think left her feeling that the descriptive explanations of the mental processes involved (i.e. ‘perversion of communication’and‘projective identification’) fell short of helping to provide the understanding needed to overcome the analytic impasse. SUMMARY.This paper describes the hating, demanding, helpless behaviour of a borderline patient, which may be ascribed to the kind of early developmental failures in the primary‘transformational object’which leave the ego too impoverished to contain psychotic anxieties. ABSTRACT.This paper describes short‐term psychotherapeutic intervention with an adolescent girl and her mother by two co‐therapists. Intervention revealed an enmeshed, symbiotic relationship with the daughter's periods of mutism reflecting anger towards others, particularly her uncommunicative mother. The daughter's two‐year history of hearing‘voices’had commenced as her mother's‘voices’had ceased, and seemed to represent a‘magical’attempt to fuse symbiotically with her. In therapy, as the mother started to talk about her own and her daughter's family background, the daughter became more verbally and emotionally communicative, and her‘voices’began to fade. As the daughter came to understand her mother more, and as the mother came to terms with her own past, they began to show some evidence of separating. ABSTRACT.Recent innovations of interventive technologies of fertilisation (AIH, AID, IVF) have brought in their wake a wave of psychological reactions in the consumers. This new syndrome includes psychological symptoms such as derealisation, depersonalisation, hypochondria, low self‐esteem, guilt and neurotic depression; anxiety reactions; phobic avoidance and obsessional reactions with rumination and compulsive rituals. In addition, considerable interpersonal stress is manifested within the marital/intimate relationships of these patients who strive to ‘make’ a baby, rather than being able to just naturally ‘have’ one.This presentation focuses on unconscious phantasies and dynamic processes underlying such symptomatology, including the emotional effects of being unable to treat intercourse as a twoperson generative event or the body as a creative fertile exponent of the self. Data is drawn from an in‐depth study of 19 patients seen in 1–5 per week psychoanalysis or individual/couple therapy and a single case is presented in depth.
