SUMMARYThe pregnant therapist is not often encountered in the psychoanalytic literature, and yet my patients were profoundly affected by my pregnancy. The patient described attempted to recreate in the transference the lost, longed‐for symbiotic mother. Identification with the (fantasised) baby was the main form of defence against separation from the idealised mother and awareness of ambivalence. In the countertransference I developed a corresponding form of primary maternal preoccupation. SUMMARY.The paper introduces a new model of the mind based on psychoanalytic listening which attends carefully to the distinctions between derivative and nonderivative communication, and the three levels of meaning in messages ‐ manifest, implication and encoded. The model forms a basis for revising Freud's theory of primary and secondary process thinking as well as notions of unconscious communication, experience and information processing.The paper then conceptualises a particular subsystem of the model ‐ the transformation subsystem (TSS). The transformation function safeguards processing which takes place in the deep unconscious system at a high level of perceptiveness and intelligence. The information processed is coated with anxiety and terror, and the transformation function assures that the images involved will not gain direct access to awareness. Displacement and symbolisation are the essential components of this processing system.On this basis, patients are classified as transformers or nontransformers, and factors in this aspect of communicative style are presented. The basic flaw in the human mind to which the transformation function contributes, namely, that information derived from the processing which takes place in the deep unconscious system is not available for conscious adaptation, is identified and discussed. Nonetheless, the transformation function is seen as a safeguard against somatisation, acting out and, more fundamentally, as a modulating factor in the human psyche. The means by which the transformation function safeguards against symptom formation is articulated, as is the means by which the therapist may assist the patient in expressing him or herself in this fashion.The paper concludes with a comparison between Bion's Alpha function theory and the present theory of transformations. SUMMARY.Unconscious Family Structure (UFS) is the virtual, symbolic space, the deep level level where lies the meaning which organises the family relations or the manifest level. Within the UFS space there are subspaces: that of the marital couple, later to become the parents, that of the children in relation to the parents, that of the children in relation to each other. Within these subspaces certain places are to be found: the father's place, the mother's place and the child's place. Between the spaces lie the relationships or links: the couple or marital link; the filial link between parents and their children; the consanguineous link between siblings. To these I add a fourth: the avunculus link with the representative of the mother's family.I depict a series of verbal or preverbal stipulations: the unconscious accords, pacts and rules.In addition I describe four UFS configurations: grade a (‘normal’), b (‘neurotic’), c‘perverse’) and d (‘psychotic’).I conclude with some observations about family transference and coun

ABSTRACT.The everyday life of a ward for chronic psychotics was sampled in regular weekly sessions and the observations analysed to discover the unconscious interactions and shared defences that explained its social structure.Outwardly there was boredom, monotony, and an absence of interactions, emotions and events but underneath there were pervasive anxieties defensively subdued especially by the staff. Even tiny moves by patients to alter this atmosphere were instantly nullified. These anxieties had two sources:(a) a massive impotence in face both of chronicity and of identification with the patients as social rejects.(b) lurking fear of madness stagnation and cmptiness.This typical psychiatric facility for chronic patients functions as a defensive system that makes change and development safely impossible and demoralises the staff. Some remedial suggestions are offered.

ABSTRACTEvidence from research in developmental psychology on mother‐infant communication and cognition in the first 18 months is considered in relation to British psychoanalytic theory, and particularly that of D.W. Winnicott. Large overlaps exist between the two disciplines, but there are areas where empirical work indicates modifications of psychoanalytic formulations and, conversely, clinical observations that draw attention to the need for far closer empirical scrutiny of certain developmental issue
