摘要:
The peripheral circulation substantially contributes to the maintenance of blood pressure. The importance of neural reflexes in maintaining vascular tone is discussed. In addition, the role of endothelial-derived vasoactive factors in modulating vascular smooth muscle is reviewed. Both regulatory systems can be compromised by pathologic processes and by anesthetics.

Inter-related and interdependent pathophysiological adaptations combine to create the clinical problem of essential hypertension. This review outlines the continuing work on vascular neuroeffector mechanisms, the use of modern molecular biological techniques, and the role of the endothelium in the regulation and generation of hypertension. Current anti-hypertensive strategies have introduced calcium antagonism and angiotensin-converting enzyme inhibition to the frontline of attention. The demonstration of organ-specific auto-antibodies in dilated cardiomyopathy introduces an exciting era of immuno- and anti-viral chemotherapy.

Perioperative cardiac morbidity remains an important contributor to outcome after non-cardiac surgery, and a plethora of studies have examined factors important in determining outcome in patients with pre-existing cardiovascular disease. This article reviews the most recent work which has examined the role of preoperative clinical assessment to identify subsets of high-risk patients where preoperative testing, special monitoring or therapy may help reduce perioperative morbidity. These data suggest that simple clinical assessment, although not ideal if used alone, when combined with additional non-invasive as well as invasive studies, remains an important element in the comprehensive perioperative management of these patients. Future studies will need to define the optimal method of preoperative risk assessment in clinical practice.

The predictive value of both individual risk factors, particularly prior myocardial infarction and preoperative myocardial ischemia, and multi-variate models for postoperative cardiac morbidity are examined in non-cardiac surgery patients. Preoperative stress and dipyridamole testing help delineate those at greatest risk, although the population studied impacts on the test's predictive value.

Exacerbation of existing or new cardiac arrhythmias in anesthetized patients is attributed to acutely altered normal or abnormal cardiac electrophysiology, the understanding of which is fundamental to effective prevention and management.
