Optometrists need a technique to view the anterior chamber angle in the diagnosis of glaucoma. Present methods require a corneal anesthetic and may damage the cornea. The Brandreth-Saladin goniolens was developed to avoid these objections and to provide the optometrist with a method for binocularly viewing the fundus with the slit lamp. Two additional techniques are mentioned which hold the same promise for optometric gonioscopy as does the Brandreth-Saladin lens. In addition, the anterior chamber angle as seen in gonioscopy is described and pertinent comments about its anatomy are made for the diagnosis of closed and open angle glaucoma.

After establishing the diurnal changes of cholesterol in the tears of a sample of patients, each was fitted with contact lenses. An abrupt, but temporary, decrease in tear cholesterol was observed in all until full wearing time was achieved. Post adaptation levels, however, were found not significantly different from pre-fitted levels. The relationship of tear cholesterol to the adaptation process and the possible contraindications excessive levels may have for wearing success are discussed.

The mechanisms involved include retinal interaction, adaptation at the level of the bipolars in the retina and eye movements. Adaptation at the bipolar layer introduces a non-linear relation between the photoreceptor response and the ganglion cell response. It also sets an upper limit to the response of the bipolars and were it not for eye movements the visual field would be reduced to a uniform brightness and were it not for eye movements the visual field would be reduced to a uniform brightness at high luminance levels. The eye movements prevent a difference in adaptation on the two sides of a border and thus provide for a difference in the response on the two sides of a border. Lateral inhibition has a greater spread than excitation and thus also accentuates the brightness at borders. A special frequency equalizing mechanism is postulated to explain the perceived uniformity of a surface between two borders. Surface contrast and border contrast involve the same mechanisms.

Ghost image intensities were measured for coated and uncoated glass and plastic lenses. Uncoated glass and plastic lenses exhibited similar high ghost image intensities. Magnesium fluoride coating on glass and a commercial coating on plastic (DAR) showed approximately equal reduction in ghost image intensity. A commercial multilayer coating (HEA) on glass effected the greatest reduction of ghost image intensity. Laboratory testing as directed by Military Specification MIL-C-675A revealed the following decreasing order of resistance to abrasion: Uncoated glass. HEA, commercially processed magnesium fluoride, Army-processed magnesium fluoride, DAR, and uncoated plastic.

This study shows how changes of target size and changes of illuminance affect the dimension of the peripheral visual field. The conventional method of testing (kinetic) was done on the tangent screen. Forty-nine subjects were tested. Each subject went from a dark adapted state to varying levels of light adaptation. The size of the visible field for one meridian was measured using five different target sizes and five different levels of brightness.

Following the vision screening of all mental patients in a U.S. Veterans Administration Psychiatric Hospital 39 schizophrenic patients were determined to have severe, long standing uncorrected distance habitual vision (20/70 or worse) of a refractive nature. The nurse evaluation of these patients after a period of wearing spectacle prescriptions shows a tendency toward improved mental health.

The examination of details and lens specifications of 100 persons unable to wear hard contact lenses was studied. Major reoccurring factors in the contact lens failure syndrome included: incomplete or partial blinking, lens design, and lens position. The results and relationships are reported.

A fourteen year old male junior high school student with a history of chronic, avoidance of reading and poor performance in reading had deficient oculomotor skills and a sluggish accommodation. Reading Glasses and combined office and home visual training for 14 weeks resulted in normalized near point visual skills, improved ability in reading, and a new positive attitude toward study and school achievement. This was maintained into the next school year following the cessation of visual training. Both pre- and post-training reading scores and visual analysis findings are presented with a summary of training procedures. Concurrent with and subsequent to the visual training, no other type of therapy was administered.
