摘要:
A report is presented on an 83-year-old female patient with seropositive rheumatoid arthritis who developed a necrotizing scleritis of the right eye. The clinical course could not be influenced by indomethacin, cysteine, and the cytostatic chlorambucil. Local cyclosporin treatment, however, which was initiated after a 4-week therapy-free period, led to considerable improvement within a few days. The disease disappeared after 6 weeks of treatment. There is no recurrence as yet, the follow-up period being 13 months.

摘要:
Epithelium plays a very important structural and functional role in the cornea and conjunctiva. Evaluation of the epithelium is the first step in diagnosing many pathologic states.We have developed a new technique for thein vivostaining of nuclei of corneal and conjunctival epithelium in rabbits and guinea pigs. Several drops of 0.01%, 0.1%, 0.25%, 0.5%, or 1.0% toluidine blue or 1.0% methylene blue were applied to the conjunctival sac to stain epithelial cells. The cells picked up the vital dye within 5 minutes and could be photographed at 30 X with the Keeler-Konan wide-field specular microscope. Cells and nuclei were clearly observable. Photographs could be further enlarged to enhance details. Wash out time was rapid and no toxic effects were observed. This technique adds a new dimension to the study of epithelium in normal and pathologic states in experimental animals. This technique may also be applicable to human eyes for discerning such diseases as carcinoma, herpes simplex, or superior limbic keratoconjunctivitis.

The following is a case report of a 61-year-old woman with a 10-year history of pemphigus vulgaris, successfully treated with steroids and cytotoxic agents. The patient developed severe herpes zoster ophthalmicus, complicated by a staph-indolent corneal ulcer. This case illustrates several of the many unfortunate ophthalmological complications that may develop in the immunocompromised patient.

Corneae of guinea pigs were burned with 200 μl IN-NaOH for 30 seconds. Topical application of 1%, 0.1%, or 0.025% indomethacin (IN) four times daily resulted in a marked delay of corneal re-epithelialization compared to corneae receiving the vehicle only. However, no retardation of corneal wound healing was observed when IN was injected intraperitoneally (5 mg/kg) twice daily, but hyperemia and chemosis were reduced.To study the proportion of retained IN in burned cornea, 0.1% IN was mixed with a tracer dose of [2-14C] IN: 3.4, 1.2, 0.42, and 0.24% of the dose were retained 20, 60, 180, and 360 minutes respectively after topical application of 15 μl. Twenty minutes after application of 15 μl 0.1% IN, the concentration of retained IN in thecorneawas 31 μg/g, which is considerably higher than the average concentration of 0.358 μg/ml1found in human blood plasma after therapeutic doses of IN.Thus, the retardation of corneal wound healing may be explained by a cytotoxic effect of supratherapeutic concentrations of IN.

摘要:
In support of the idea that the HLA system plays a major role in corneal graft rejection, we report these two unique cases. Both had two penetrating grafts in the same eye. One was a regular graft and the second a patch graft to repair a perforation in the initial one. In the first case, the original graft had an allograft reaction and the patch graft remained unaffected. In the second case, an allograft reaction to the patch graft developed while the regular graft remained completely clear. Both grafts in the two cases cleared following medical therapy. The fact that in these two cases rejection reactions involved one transplant only, even though the other graft was subject to the same conditions, implies that the difference in the antigenicity of the two grafts is an important factor in the chain of events leading to rejection.

Five different biotin labeled lectins, Concanavalin-A (Con A), wheat germ agglutinin (WCA),Ricinus communisagglutinin I (RCA1),Ulex europaeusagglutinin (UEA1), and soybean agglutinin (SBA) were used to study lectin receptors on formalin-fixed paraffin embedded human corneas. Con A stained the cytoplasm, cell, and nuclear membranes of the epithelial cells and stained the stroma diffusely. WGA stained the superficial epithelial cells, the epithelial cell membranes, and the keratocytes of the stroma. SBA did not react with any of the corneal layers. RCA1heavily stained the keratocytes but did not stain the epithelium. UEA1lightly stained the epithelial cell cytoplasm and interstitial stroma. All staining reactions could be abolished by omission of the lectin or by the use of the appropriate inhibitory sugar. The lectin binding patterns reported here provide a means for further investigation of carbohydrate structures in the human cornea in both normal and disease states.

Sequential slit-lamp examinations and fluorescein angiograms were performed on adult rabbits to define the morphological and functional maturation of new intracorneal vessels induced by chemical cautery. The vasoproliferation induced by this injury began as vascular sprouts from the preexisting pericorneal vessels and continued until there were well formed vascular loops carrying blood to and from the cautery site. Many vessels, particularly venules, then regressed. As defined by the fluorescein angiograms and microscopic examination of tissue sections, the final loops were formed by muscularized vessels (arterioles) bringing blood to the cautery site, and unmuscularized vessels (venules) carrying blood back to the vessels at the corneoscleral limbus. The angiograms recorded during the evolution of these vessels suggest that intracorneal arterioles form when blood flow or pressure from a preexisting pericorneal arteriole induces muscularization in a previously undifferentiated sprout arising from a capillary or venule.

Three patients undergoing uncomplicated cataract extraction with intraocular lens implantation developed an endothelial line, clinically appearing identical to that seen in allograft rejection following corneal transplantation. Both intracapsular and extracapsular surgery were involved. Three different intraocular lens types were implanted (pupillary supported, anterior chamber, and posterior chamber). Each case was associated with postoperative iritis and glaucoma. They were nonresponsive to steroid therapy and eventually resulted in total corneal decompensation. This clinical entity may represent an autoimmune phenomenon stimulated by chronic inflammation induced by the intraocular lens.

摘要:
Two cases of silver nitrate burn occurred after treatment of superior limbic keratoconjunctivitis with a solid silver nitrate applicator. After medical treatment and several months of observation, visual acuity eventually returned to acceptable levels with minimal corneal scarring. These cases emphasize the fact that silver nitrate should be used very cautiously around the eye and only in a fresh 0.5–1% solution. The use of solid silver nitrate should be prohibited around the eye.
