In a retrospective review of 76 consecutive penetrating and nine lamellar keratoplasties done for herpes simplex keratitis, we studied the incidence of persistent epithelial defects in the graft in the immediate postoperative period. Early postoperative persistent epithelial defects were present in 8% of the penetrating keratoplasties and in 44% of the lamellar keratoplasties. Compared to the incidence of persistent defects in penetrating keratoplasties done for aphakic, pseudophakic, and Fuchs' corneal edema, the frequency of delayed epithelial closure was higher in penetrating keratoplasties done for herpetic keratitis. All keratoplasties done during the active inflammatory stage of herpetic disease developed persistent epithelial defects. This was marked especially in lamellar grafts all of which were done emergently during active inflammation. An additional factor contributing to the high incidence of delayed epithelial closure in lamellar grafts may be the use of donor tissue obtained from whole globes kept in a moist chamber at 4°C in excess of 1 week. Although dry eye conditions were responsible for an increased tendency for persistent epithelial defects, antiviral medications, in contrast, had no significant effect on epithelial wound closure in the immediate postoperative period.

The blood-aqueous barrier of ten keratoconus cases awaiting corneal transplantation was studied and compared to a normal control group of 10 patients by utilizing Amsler and Huber's semiquantitative method. The action of a cycloplegic (1% tropicamide) on this barrier was studied simultaneously both in normal and keratoconus eyes. Our results have shown that keratoconus patients have an abnormal blood-aqueous barrier. The results have shown an average of values with statistically significant higher permeability in keratoconus in all studied times (p < 0.001). The altered blood-aqueous barrier in keratoconus is an indication that this pathology is not only limited to the cornea. This altered barrier could be related to Urrets-Zavalia's syndrome in patients submitted to corneal transplantation for keratoconus.

Corneal endothelial cell borders were stained by alizarin red in 30 intact human cadaver eyes. Estimates of the endothelial cell densities at intraocular pressures of 0 and 40 mm Hg differed by 4.46%. The difference was less than 1% between cell counts at 10 and 20 mm Hg. The corneas were excised, and the endothelial cell densities were determined again. Estimates of the cell densities in intact eyes at 20 mm Hg were 9.7% lower than the estimates in excised corneas.

The technique of photon correlation spectroscopy has been used to measure scattered light intensity fluctuations from the corneal stroma of diabetics and nondia-betics. The fluctuations in light intensity result from Brownian motion of the scattering elements in the cornea. Analysis of the resulting autocorrelation function provides information on these scatterers. Changes in stromal measurements were noted in response to aging. There were also significant changes measured in those diabetics exhibiting proliferative diabetic retinopathy. Patients in the proliferative retinopathy class who had had prior laser photocoagulation therapy showed measurements more characteristic of nondiabetics and diabetics with no or background retinopathy. This technique provides a sensitive, quantitative, and noninvasive method for clinical monitoring of corneal changes at the molecular level.

Penetrating homolateral autokeratoplasties were performed in five adult rhesus monkeys to determine the influence of the length of sutures on the postoperative astigmatism both before and after suture removal. Our purpose was to minimize the effect of wound irregularities on corneal astigmatism by replacing the homolateral button without rotating it. The mean postoperative astigmatism with sutures in place was 10.55 D; suture removal reduced this to 2.37 D. Our results suggest that it is unlikely that sutures induce permanent astigmatism.

A 37-year-old male patient underwent epikeratoplasty for treatment of keratoconus. Over a 38-month postoperative course, visual acuity progressively deteriorated and contact lens intolerance developed, necessitating penetrating keratoplasty. Light microscopic and ultrastructural study of the excised tissue demonstrated epithelial irregularities, repopulation of donor stroma, defects in recipient Bowman's layer, and focal areas of endothelial degeneration.

Eighteen patients (36 eyes) entered a double-blind randomized between-eyes trial comparing 2.0% tolmetin with 0.25% prednisolone. Patients were treated for about 2 weeks (9–21 days) during which time there was a statistically significant improvement in patient symptoms and clinical signs scores in both treatment groups. No significant difference was found in treatment efficacy between tolmetin and prednisolone. Both tolmetin and 0.25% prednisolone have a mild anti-inflammatory effect, which is useful in the treatment of mild to moderate inflammation of the external eye. There was a higher incidence of discomfort in the tolmetin-treated eyes.

The adherence of a strain ofPseudomonas aeruginosato hydrophilic contact lenses was quantitatively determined using scanning electron microscopy. Soft contact lenses were incubated for 72 h in either phosphate-buffered saline (PBS), a 500-μg/ml solution of human serum albumin (HSA), a 500-μg/ml solution of bovine submaxillary gland mucin, or a solution containing 500 μg/ml of both HSA and mucin. Lenses were then incubated for one hour in a 107-ml suspension ofP. aeruginosa.The number of adherent bacteria per field at × 8,000 magnification was determined. Only 1.96 ± 2.52 bacteria/field were found on lenses incubated in saline, whereas 5.23 ± 5.30 bacteria/ field were adherent to lenses incubated in HSA, 23.42 ± 31.01 to lenses incubated in mucin, and 26.76 ± 35.12 to lenses incubated in a solution containing both HSA and mucin. Contact lens coatings, especially mucin, facilitate the adherence off.aeruginosato soft contact lenses and may therefore play a role in the pathogenesis of contact lens-associatedpseudomonalkeratitis.
