We did a specular microscopic study with computer-assisted morphometric analysis of individual cells on the corneal endothelia of 69 Japanese diabetics and 53 age-matched nondiabetics. No statistically significant differences were seen in the mean cell areas of diabetics and nondiabetics. However, the coefficient of variation of cell area was significantly higher and the percentage of hexagonal cells was significantly lower in diabetics than in nondiabetics. Although racial differences of the corneal endothelia exist between Japanese and Americans, diabetic changes are quite similar in the two populations.

A 45-year-old man who had uneventful excision of bilateral pingueculae developed bilateral membranous lesions involving the bulbar conjunctivae and corneas. Histologically, the membranes were composed mainly of large fibrinous deposits intermixed with acute and chronic inflammatory cells with areas of fibroblastic and capillary proliferation resembling granulation tissue. By electron-microscopy the amorphous acidophilic masses were composed of electron-dense, fibrillar material with a periodicity of 10–12 mm, which was consistent with fibrin. Despite mechanical removal of the membranes, they continued to recur rapidly over a period of several months. The lesions apparently responded slowly to topical enzymatic therapy that consisted of hyaluronidase (175 U/ml) and α-chymotrypsin (1:5000) drops. Follow-up examination, ∼1 year after surgery, revealed that the patient was asymptomatic. Ocular examination disclosed slight persistence of gelatinous membranes on the bulbar conjunctivae, most prominent in the left eye.

A simple, semiquantitative, histochemical method using periodic acid-Schiff (PAS) staining was developed to measure the mucinlike glycoprotein content of bathing solutions exposed to rabbit conjunctiva. Rabbit eyes were bathed in vivo in solutions of various tonicity (160, 260, 300, 330, and 363 mOsm/L). Eyes bathed in hypotonic or isotonic solutions (160, 260, and 300 mOsm/L) exhibited a steady-state glycoprotein secretion (50–100 μg/h). In hypertonic solutions (330 and 363 mOsm/L), glycoprotein discharge increased to 150–360 μg/h when the solutions were exchanged hourly for 6 h, but glycoprotein discharge increased to a much lower rate (100–167 μg/h) after prolonged bathing for 6 h without solution exchange. Periodic acid-Schiff-positive glycoprotein aggregates were noted more frequently on the filters of hypertonic solutions than on those of other tonicity. After eyes were bathed in hypertonic solutions, increased disruption of the surface epithelium, increased abnormal discharge of mucin granules, and decreased goblet cell density were noted in the biopsied conjunctiva. These data suggest that mucinlike glycoprotein secretion from the conjunctival goblet cells can be enhanced by the tonicity of the bathing solution, and that nonphysiological hypertonic solutions can also affect the conjunctival epithelium.

Bacterial viability in bacterial adherence to new soft contact lenses (SCLs) was studied by exposing polymacon and lidofilcon A lenses toPseudomonas aeruginosa:L, live, untreated bacteria; H, bacteria heated for 1 h at 100°C; and P, bacteria treated with 3% hydrogen peroxide for 1 h. Cultures of groups L, H, and P showed heavy, no, and minimal bacterial growth, respectively. Scanning electron microscopy (SEM) showed significantly less bacteria on new SCLs in groups H and P. Transmission electron microscopy (TEM) showed that bacterial shape was mostly preserved in all groups, with marked intracel-lular and cell membrane changes and breaks (groups H and P), margination of intracellular electron-dense material (group H), and central and peripheral vacuolation (group P). Adherence to new SCLs appears to be an active process, as killed or altered bacteria adhere less.

The clinical results of penetrating keratoplasty for iridocorneal endothelial (ICE) syndrome are presented. Nine cases of penetrating keratoplasty for ICE syndrome were reviewed retrospectively. All grafts remain clear, with posterior corneal changes occurring in only one case, after a mean follow-up of 43 months. Penetrating keratoplasty produced visual improvement in 7 of the 9 cases, relief of pain in 6 of 6 cases, and prevention of further corneal infection in 1 case. We believe that penetrating keratoplasty has a place in the treatment regimen of this disease, not only to achieve improvement in vision, but also for relief of pain, and avoidance of recurrent corneal infections, and to allow a clear media for careful monitoring of optic disc and field changes in the associated glaucoma.

A 3-month course of chronic keratitis with persistent epithelial defect resolved in 48 h following the use of topical trimethoprim-sulfamethoxazole.

Epidermal growth factor (EGF) is known to promote corneal epithelial wound healing in experimental scrape and keratectomy models. We studied its efficacy in treating alkali-burned epithelial ulceration. EGF induced hyper-plasia and cell proliferation to resurface the denuded and denatured corneal stroma, but it did not prevent recurrent erosions. A combination of EGF with fibronectin (Fn) was noted to enhance epithelial defect closure after alkali burns of the cornea and was seen to prevent recurrent erosions. Histopathologic examination of these corneas revealed marked leukocytic infiltration of the alkali-burned corneal stroma. To find ways to retard this inflammatory response, we studied the role of topical steroids and their efficacy when used with EGF, Fn, and laminin (Ln) in the management of alkali-burned corneas. Use of steroids decreased the incidence of recurrent erosions and corneal perforations. Histologically, steroids markedly decreased the leukocytic infiltration of stromal tissue, thereby retarding the collagenolysis. Topical steroids used with EGF and fibronectin were seen to promote epithelial wound closure and to prevent recurrent erosions in alkali-burned corneas. Combinations of EGF, fibronectin, and steroids may have a place in the treatment of clinical corneal alkali burns.

We evaluated the potential ocular surface toxicity of a recombinant interferon-a (IFNα) preparation on rabbit eyes. Topical application of this interferon (1 x 106 U/day in each eye) for 6 weeks produced no discernible con-junctival or corneal abnormalities on clinical, histopatho-logic, or ultrastructural evaluations. We believe that this recombinant IFNα preparation is not toxic to the ocular surface of rabbits.
