A long-term study of 114 penetrating keratoplasties in 106 patients using donor corneas stored at 37°C in organ culture incubation for an average of 16.5 days prior to transplantation is reported. A total of 80.7% of the grafts remained clear after a minimum of 6 months' follow-up time. Compared with other published studies using 4°C refrigeration and M-K medium, there appears to be no difference in the success rate or cause of graft failure between these three methods. Except for an increased incidence of wound separation using organ cultured corneas the postoperative complications are similar. There is no apparent modification of immune graft rejection. Based on the results of this study, 37°C organ culture appears to be a safe and efficacious method of medium to long-term corneal storage prior to penetrating keratoplasty.

From 1974 to 1978 the Medical Eye Bank, Inc. of Baltimore, Maryland, supplied the donor material for keratoplasties in 410 cases of keratoconus. Follow-up information of six months or more was obtained in 390 cases. In 322 cases, the recipient eye was phakic without previous keratoplasties on the recipient eye and a penetrating keratoplasty was done solely for the improvement of vision. Graft clarity was examined in terms of donor and recipient variables. Overall graft clarity was 93% and it was equally good using donor tissue from whole eyes preserved in moist chamber compared to donor tissue from the Medical Examiner preserved in McCarey-Kaufman medium. Thus, the Medical Examiner's office has proved to be a safe, reliable source of excellent donor tissue.

Twenty patients with severe recurrent stromal herpetic keratitis and keratouveitis resistant to IDU and Vira-A were treated with topical F3T, steroids, and cycloplegics in a controlled manner of initiation and withdrawal. The timely use of adjunctive systemic steroids is indicated in cases of severe anterior segment inflammation to ameliorate the antigenic and cell mediated anterior chamber response. Antibiotics, acetazolamide, and bandage lenses are used to treat various complications of herpetic keratouveitis. Optimum management controls viral replication and the severe inflammatory reaction in the anterior segment within 4–5 weeks, but does not prevent recurrence of the disease.

A 23-year-old woman had bilateral keratoconus requiring keratoplasty, and her identical twin sister had normal corneas. Red blood cell antigen and biochemical marker typing supported monozygosity. Differences between the twins in eye-rubbing and hormonal status may have contributed to the development of keratoconus in one and not the other. Since the second twin may yet develop keratoconus, these findings do not negate the importance of heredity in the disease; they do suggest, however, that nongenetic factors can influence the manifestation of keratoconus in susceptible individuals.

This study was designed to assess the effects of topically applied hyperosmotic glycerin on the human corneal endothelium. Ten human subjects with one eye legally blind due to noncorneal causes were studied. Anhydrous glycerin was applied to one eye and normal saline to the other eye. Both eyes were studied with specular microscopy.Frequent application of topical glycerin resulted in extensive changes in the specular microscopic appearance of the corneal endothelium; these changes consisted of blurring of the endothelial mosaic, phase reversal of the cell borders, and light and dark intracellular and intercellular bodies. Most of these changes reversed by 1–1 1/2 hours with the exception of some cellular “events.” In contrast, the corneas treated with saline alone showed no significant change.It is suggested that at least transient and perhaps some permanent changes in the corneal endothelium can be induced by the topical application of anhydrous glycerin.

Penetrating sclerokeratoplasty is a technique of total corneal transplantation including a rim of sclera which was introduced in 1956, but which gave only temporarily clear grafts. The technique has been revived and with newer surgical techniques, the protection of a bandage contact lens, and long-term local and systemic steroids, has given encouraging results in severely damaged anterior segments. Clear grafts were determined in nine of 18 (50%) eyes for from 1.2 to 3.5 years, average 2.7 years. Three of 18 (17%) grafts were partially clear and six of 18 (33%) grafts were opaque. Complications included postoperative hemorrhage (2), graft rejection (2), glaucoma (2), temporary epithelial loss (2), and phthisis bulbi (2).

Epikeratophakia is a new form of lamellar refractive surgery. These grafts are theoretically applicable for permanent refractive correction of many refractive problems. However, the present availability of human donor corneal tissue is probably inadequate to meet this potential demand. We have used porcine corneal tissue for hyperopic epikeratophakia grafts in non-human primates. These grafts appear to be successful, to remain optically clear, and to have a stable anterior surface.

The procedures for correction of excessive corneal astigmatism following penetrating keratoplasty—corneal wedge resection and corneal relaxing incisions—have been well established as effective in reducing approximately 80% of high astigmatic errors. Recent refinements in the original techniques are presented with particular reference to the use of the corneal suction ring, the double-bladed diamond knife, the TG-6-C corneal needle, 10–0 nylon and 11–0 polypropylene sutures, and clinical and surgical keratometry. The improved accuracy afforded by these techniques promises to improve the results and to reduce the standard deviation of previously published techniques.
