Thirty-eight eyes of 22 patients with corneal endothelial guttata were examined by slit lamp, tonometry, tonography, and wide-field specular microscopy. The mean value for facility of outflow (C55) was 0.23, and it was not statistically different from reported values for a normal population. Also, there was no correlation between extent of guttata as seen with wide-field specular microscopy and facility of outflow. Thus, contrary to a previous report, endothelial guttata does not seem to predispose to impaired facility of outflow.

摘要:
The technique of conjunctival transplantation has proven very successful in reestablishing an intact ocular surface in patients with severe ocular surface disease in whom conventional treatment has failed. We present follow-up on the histology of the transplanted conjunctival tissue in four alkali-burned patients who underwent penetrating keratoplasty 3–28 months following conjunctival transplantation. The corneal button was reepithelialized in all patients. The fate of the transplanted tissue agreed with experimental observations made in the rabbit model, i.e., under areas of vascularization and inflammation, the transplanted tissue resembled conjunctiva with numerous goblet cells and 3–4 cell layers of nonkeratinized, stratified epithelium. In one patient, a definite “transition zone” from conjunctival to corneal appearing epithelium was seen in an area with minimal inflammation and vascularization, again confirming the animal results showing that transdifferentiation of conjunctival to corneal epithelium can occur.

A series of 51 patients with bilateral penetrating corneal grafts was followed for 1–12 years to assess the effect of the second eye corneal graft on the incidence of allograft rejection. In 33 patients (66 eyes) who had avascular grafts, there was no significant increase in the incidence of allograft rejection reaction after corneal transplantation in the second eye. In 18 patients (36 eyes) who had vascularized corneas, there was a twofold increase in the incidence of allograft rejection in the second as well as in the first corneal graft. Based on this observation and other reports cited in the literature, we offer contraindications for grafting of the second eye.

摘要:
Six cases are described ofPseudomonas aeruginosaulcerative keratitis in which antibiotic sensitivity studies demonstrate organism resistance to gentamicin sulfate but sensitivity to other aminoglycosides such as tobra-mycin and amikacin. In four cases, community-acquired infections represent the source of these ulcers. This paper documents the emergence of aminoglycoside resistance amongPseudomonas aeruginosakeratitis within the general community.

BL-P1654 is a new semisynthetic penicillin that possesses broad spectrum antimicrobial activity against many gram-positive and gram-negative bacteria, includingPseudomonas aeruginosa. Anin vitroprofile of BL-P1654 was established against strains ofStaphylococcus aureus, Streptococcus pneumoniae, andPseudomonas aeruginosa, three bacteria frequently associated with infections of the eye. The effectiveness of BL-P1654 in preventing the development of experimentally-induced keratitis by each of these bacteria was determined. The results of these experiments show BL-P1654 to be more effective than gentamicin and support further evaluation of the semisynthetic penicillin for ophthalmic indications.

Rabbits underwent implantation of either lyophilized or fresh porcine lenticules into the central or peripheral cornea. All animals were followed for up to four months by slit-lamp examination and macrophotography to determine implant rejection. Serum antibody levels to soluble porcine cornea extract were determined by an enzyme-linked immunosorbant assay. Only those animals receiving lenticules into the peripheral cornea experienced a rejection and developed an antibody response to the porcine cornea extract. The production of antibody preceded the appearance of vascularization of the implanted lenticules. Thus, the site of lenticule implantation, not the type of tissue preparation, determined the outcome of the graft.

摘要:
Morphological changes were examined in the corneal endothelium of rabbits after administration of several drugs widely used in ophthalmology. After administration of epinephrine and dipivalyl epinephrine eye drops, swelling of corneal endothelial cells developed; however, these changes were reversible. No pathological changes were observed in the corneal endothelial layer following the administration of timolol, betamethasone, and gentamicin eye drops. Morphological changes were observed in the corneal endothelial layer after the subconjunctival injection of timolol, betamethasone, and gentamicin.

Nine corneal specimens were obtained after penetrating keratoplasty for ultrastructural analysis. All the corneas had earlier suffered from herpetic infections and scarring. Six corneas presented vascularized interstitial keratitis and three corneas showed chronic keratitis. Five specimens with interstitial keratitis presented stromal herpes virus-like particles. All the corneas containing virus particles showed stromal lymphocytic infiltration, and in four, macrophages were present. In two cases, lymphocytes were in close contact with affected keratocytes, suggesting that cell-mediated immunity plays a role in herpetic interstitial keratitis. The corneas with disciform keratitis were free of virus particles and leukocytic infiltration. The observed ultrastructural findings suggest that retrocorneal ridges, a form of posterior corneal scarring, has its origin in a granulomatous reaction between the stroma and Descemet's membrane.

We performed four aphakic and two myopic epikeratophakia procedures in a non-human primate model using two fresh and three prefrozen corneas and one glycerin-preserved cornea. The experiments were terminated 4 to 5 months postoperatively, and the eyes examined with correlative microscopy. The glycerin-preserved cornea sloughed and was not available for analysis. All but one prefrozen specimen demonstrated donor keratocyte repopulation. Active keratocytes were noted lining the optical interface. Focal areas of missing Bowman's layer were present in three recipient corneas. Epithelial interface ingrowth occurred in two specimens. The recipient corneas were otherwise normal.
