Clinical guidelines for the diagnosis of pigmented lesions of the skin are not always reliable. Surface microscopy (SM) represents an interesting approach to this problem. For this in vivo investigation, a stereomicroscope, a glass slide, and immersion oil are used. In the present study, the various criteria of SM that cannot be discerned by the naked eye are correlated with the histopathological structures in step-sectioned specimens. Characteristic SM features such as the pigment network, “black dots,” or “irregular extensions” were found to correspond to particular histological findings. SM opens a new dimension of clinical morphology for the diagnosis and differential diagnosis of malignant melanomas, dysplastic nevi, or nonmelanocytic pigmented neoplasms, and facilitates a more reliable preoperative assessment of these lesions.

Between 1982 and 1985, a total of 39 congenital pigmented nevi (CN) were removed from patients in the Oklahoma Children's Memorial Hospital. Multiple sections were examined from each of the lesions removed. The histologic patterns were recorded with special note of those features traditionally associated with CN. These include nevus cells in the lower reticular dermis and subcutaneous tissue, association of nevus cells with and in appendageal structures, and infiltration of nevus cells between collagen bundles. These features were primarily observed in larger CN (>3 cm). Most small nevi do not show these features. The histologic patterns were consistent throughout multiple sections of the nevi. It is concluded that the histology traditionally associated with CN relates primarily to thesizeof the nevus rather than to the time of its appearance.

We performed ultrastructural studies of apoptosis (previously referred to as “malignant, dyskeratosis”) in a case of genital Bowen's carcinoma in which human papillomavirus (HPV) type 33 genome was identified and in two cases of cutaneous Bowen's disease with no detectable viral DNA; herein we present the sequential stages in the development of apoptotic bodies. The apoptotic process in the HPV-containing genital Bowen's disease was similar to that in the cutaneous lesions with no detectable HPV. The presence of a large number of apoptotic bodies in Bowen's disease may be responsible for the slow progression and noninvasive growth of this carcinoma in situ.

The clinical, immunohistochemical, and electron-microscopic features of 13 consecutive patients with gold dermatitis were analyzed: 12 developed an eczematous dermatitis and one a lichenoid dermatosis. The patients had received intramuscular sodium aurothiomalate therapy from 1 month to 4 years before the dermatitis broke out. After cessation of gold therapy, the dermatitis persisted for 1–11 months. A relatively sparse perivascular mononuclear cell infiltrate was found in the affected skin in all cases. With immunoperoxidase staining, most of the infiltrating cells were shown to be OKT-4-positive T-helper lymphocytes. A majority of the infiltrating cells were Ia, i.e., HLA class II antigen, positive. Clearly increased numbers of dermal OKT-6-positive Langerhans' cells were also seen. In epidermis, on the contrary, the expression of both OKT-6 and Ia markers on dendritic cells was decreased. However, electron-microscopic examination revealed large numbers of macrophage-like, cells and the Langerhans cells were activated, often in apposition to mononuclear cells within the epidermis. No correlation was observed between the immunohistological findings and the amount of gold received, the duration of gold therapy, and the interval between the last gold injection and biopsy, respectively, although peripheral blood eosinophilia was more common during 5–10 months of gold therapy. There were no specific findings in the patients in whom dermatitis lasted several months after discontinuation of the therapy. Our findings support the view that immunological mechanisms operate in the development of gold dermatitis, although the exact mechanisms remain unknown.

Cryostat sections were incubated with two different monoclonal antibodies directed either against two different cells or against the same cell. For visualization of the first antibody, the alkaline phosphatase-antialkaline phosphatase (APAAP) technique was used, which rendered a bright red reaction product. For visualization of the second monoclonal antibody, the immunogold-silver-staining (IGSS) technique was used, which resulted in a black reaction product. The combination of an enzyme technique with a technique involving metals (gold and silver) seemed ideal since the reaction product of either technique could be detected easily.

To establish the character of microvascular changes in psoriatic skin and their specificity, 29 skin biopsies of psoriatics (20 in exacerbation and 9 in a stationary stage) were investigated using histologic, histochemical, immunomorphologic, electron-microscopic, and morphometric methods. Five biopsies of uninvolved skin in scleroderma and five of diabetes mellitus patients were studied with the same technique for comparison. The results showed that structural changes depend on disease stage and the clinical appearance of lesions. Microvascular changes precede papule appearance during exacerbation and gradually increase during papule development. They comprise vascular dilatation, bridged fenestrations and gaps in endothelium, edematous areas in the cytoplasm of endotheliocytes, myocytes and pericytes, basement-membrane-zone thickening, and cell extravasation—signs of increased vascular permeability. Immunoglobulin G deposits in vascular walls, degranulation of mast cells, and extravasation of lymphocytes and neutrophils indicate that inflammation is a basic process during exacerbation and that immune mechanisms play an important role in the pathogenesis of inflammation. Microvascular changes in scleroderma and diabetes mellitus are different in nature and do not resemble those in psoriasis.

While he was being treated for Waldenstrom's macroglobulinemia, a 75-year-old man developed an enlarging forearm skin nodule. On biopsy, the lesion appeared to be a malignant lymphoma. The tumor cells were negative for immunoglobulins but positive for lysozyme and alpha-1-antitrypsin. Therefore, the lesion was diagnosed as histiocytic lymphoma. Nine months later, an ipsilateral axillary lymph node biopsy revealed a small focus of tumor identical to that of the skin lesion. Three months after the lymph node biopsy, the patient developed acute myeloid leukemia. A reevaluation of the electron micrographs of the skin and lymph node lesion showed primary lysosomal granules within the tumor cell cytoplasm consistent with a diagnosis of leukemic infiltrates (granulocytic sarcoma); additionally, the naphthol AS-D chloracetate estarase activity of the skin lesion was positive, supporting the diagnosis of granulocytic sarcoma. This report shows that if not suspected, granulocytic sarcoma is difficult to diagnose in nonleukemia patients.

A case of a solitary mesenchymal hamartoma occurring on the chin of a newborn infant is presented. The skin nodule exhibited an abnormal arrangement of dermal mesenchymal elements and, in particular, there was prominence of skeletal muscle. This lesion, which appears to be unique, has been termed rhabdomyomatous mesenchymal hamartoma of skin.

Malignant endovascular papillary angioendothelioma is a rare vascular tumor of childhood that was first defined by Dabska in 1969. Microscopically, this tumor is composed of anastomosing vascular channels, some of which contain papillary projections or tuft-like structures sometimes resembling renal glomeruli. Although cases have been reported with metastases to regional lymph nodes, the long term prognosis for patients with these tumors is generally good. We have studied this tumor by light and electron microscopy as well as immunohistochemical techniques in a 10-year-old boy. A review of the literature on this rare entity is presented.

The pathophysiological processes responsible for the histological and clinical appearance of skin diseases characterized by inflammatory infiltrates of dermis and epidermis remain obscure. The recent explosion of knowledge about cytokines with immune and inflammatory activities, as well as the identification of many of these cytokines as products of activated keratinocytes, fibroblasts, and endothelial cells, lends a novel and exciting perspective on these diseases. In this article, paradigms that incorporate what is known about epidermal and dermal cell cytokine production and the effects of these cytokines on mature immune and inflammatory cells are discussed.
