Computed tomography (CT) and radionuclide imaging of the central nervous system (CNS) are important modalities in both the diagnosis and management of CNS complications fromHemophilus influenzaemeningitis. Both examinations are sensitive indicators of CNS abnormalities associated with bacterial meningeal infections and are important determinants of the therapy and ultimate prognosis of such complications. Representative findings from CT and radionuclide imaging of children with complications fromH. influenzaeinfection of the meninges are presented.

The findings of radionuclide brain scans and computerized tomography (CT) were compared in 25 consecutive patients with intracranial infectious disease. Seventeen of the 25 patients had diffuse disease and eight had brain abscesses. The radionuclide scans showed abnormalities in 13 of the 17 patients with diffuse inflammatory disease, whereas the CT scans showed abnormalities compatible with inflammatory disease in five. All eight abscesses were demonstrated on the radionuclide scans, whereas the CT scans indicated abnormalities in seven cases. The authors conclude that the radionuclide brain scan is more sensitive than the CT scan for the demonstration of diffuse intracranial infectious disease.

To determine the accuracy of the posterior cerebral radionuclide angiogram (CRAG) in the detection of carotid occlusive disease, a retrospective study of 186 patients who underwent both CRAG and contrast cerebral angiography within a period of 30 days was carried out. Asymmetry of neck activity was highly specific (97%) but insensitive (27%) for detection of carotid disease. When asymmetry of cerebral perfusion was also considered, the sensitivity rose to 61%, but the specificity fell to 77%. For the entire group, including carotid and intracerebral disease, the sensitivity of the posterior CRAG was 49% and the specificity 89%. From these data and a review of the literature, we conclude that the CRAG is too insensitive to be used as a screening test for carotid occlusive disease.

摘要:
The cause of jugular venous reflux during a radionuclide cerebral angiogram is uncertain. The authors describe a patient in whom physiologic respiratory maneuvers had a profound influence on the jugular venous reflux during cerebral radionuclide angiography. The massive jugular reflux was abolished by intravenous injection of a radiotracer bolus during inspiration. When brain flow data are difficult to interpret because of reflux, the authors suggest that radiotracer be administered intravenously during inspiration.

摘要:
Cerebrospinal fluid (CSF) leaks may occur following spinal surgery and are commonly associated with infection. These leaks can be successfully evaluated with retrograde myeloscintigraphy using the same techniques applied in diagnosing CSF otorrhea and rhinorrhea. This report describes the utility of this procedure in differentiating a process due solely to postoperative infection from one that involves both an inflammatory site and a fistulous tract with the subarachnoid space.

摘要:
Osteonecrosis of the skull produced by an electrical burn presented as a dramatic “cold” bone lesion on an MDP bone scan, despite normal skull radiographs. Four months later the skull radiograph showed marked bone resorption, and the three-phase bone scan confirmed healing and new osteoblastic activity. The pathophysiology of high-voltage injuries is outlined.

A case of right atrial visualization on Tl-201 imaging in a patient with marked right atrial enlargement secondary to atrial septal defect is described. The misinterpretation of Tl-201 and gated cardiac studies on initial inspection, due to this unusual finding, and the importance of obtaining special lateral and oblique views for correct identification of cardiac chambers are discussed.

摘要:
It has been reported that the tentorial confluence sign and/or hockey stick pattern on static imaging are useful in evaluating posterior cerebral artery (PCA) infarctions. In 16 cases with PCA infarction, the tentorial confluence sign was found in all cases, but a hockey stick pattern was seen in only half of these patients. The abnormal accumulations may be classified into three types. These three types seem to be consistent with the territory of the blood supply from the cortical, temporal, and/or occipital branches of the posterior cerebral artery. Emission computed tomography images may also demonstrate this consistency.
