Myocardial studies with99mTc-labeled phosphate agents were obtained in 190 patients (208 myocardial scintigrams). Thirty-eight of 39 patients free of cardiovascular disease had no myocardial activity. Forty-two patients who met the clinical criteria for stable arteriosclerotic heart disease showed no myocardial activity. Thirty-seven patients with unstable arteriosclerotic heart disease showed ill-defined, faint-to-mild, increased activity in the region of the anterior to inferior aspect of the left ventricle. Sixty-three patients met the clinical criteria for acute myocardial infarction. Of 50 patients with transmural infarctions, 49 demonstrated focal accumulation of increased activity corresponding to the ECG localization of the infarction. Of 13 patients with subendocardial infarction, 11 demonstrated increased activity in the region of the left ventricle similar but somewhat more focal than that seen in unstable arteriosclerotic heart disease. Differentiation between these two conditions by scintigraphy alone is not always possible. Six of seven patients with myocardiopathy demonstrated faint-to-marked, diffuse, increased activity throughout the conines of a markedly enlarged cardiac silhouette. Assessment of the distribution and intensity of myocardial activity in conjunction with other clinical considerations aids in differentiation between these disease processes. Myocardial imaging with99mTc-labeled phosphate is a simple, safe, non-invasive technique which is both practical and worthwhile in a community hospital setting.

Intravenous diffusion hepatography with99mTc-pertechnetate was used to evaluate the diffusion patterns of solitary hepatic masses which had little or no perfusion demonstrable by the113mIn perfusion study. The scintiphotos of 21 proven cases were analyzed. The results indicate that the diffusion study may help to differentiate hepatic abscesses, especially amebic abscesses, from congenital solitary cysts. This simple and innocuous approach would be another step toward increasing the specificity and accuracy of the nuclear diagnosis of hepatic mass lesions.

Serial bone scans and uptake measurements with99mTc labeled diphosphonate were performed on patients with osseous metastases from primary breast cancer. Eleven of 18 patients showed a transient worsening of the scan appearance shortly after cyclical combination chemotherapy was commenced. The appearance of this “flare” effect could not be correlated significantly with the patients' response to treatment. It is stressed that an apparently deteriorating99mTc diphosphonate bone scan is not an adequate basis on which to terminate a particular therapeutic regime. We have previously reported similar results for a series in which87mSr was the scanning agent.

Two hundred forty-one99mTc-pyrophosphate camera images in 181 patients with trauma were compared with roentgenograms taken on the same day. Forty-eight of 53 fractures determined by roentgenography were detected by bone imaging. Initial roentgenographic studies missed 13 fractures that were detected by imaging and confirmed by repeat roentgenography. Forty-four additional cases of positive bone imaging with negative roentgenograms were found. Bone imaging was especially useful in detection of fractures in chest, feet, hands, and skull, areas that are difficult to define by routine roentgenography. The combination of imaging and roentgenography may detect many more traumatic fractures than roentgenography alone.
