A case is presented in which right ventricular hypertrophy resulted in an ECG study suggestive of myocardial infarction and201Tl-myocardial imaging with unexpectedly high count rates and a confusing morphology. Once the anomaly in the anatomical presentation was understood, the case was easily resolved.

Cerebral blood flow studies and brain imaging performed on a middle-aged Mexican man showed decreased arterial flow to the right hemisphere and decreased radioactivity in the right front-parietal region with a crescent area of increased activity in the parieto-temporal region. The scintigraphic findings were compatible with a large cystic lesion in the right parieto-frontal area. Computerized axial tomography revealed multiple mass lesions of decreased density in the right frontal, perietal and temporal regions. A large cyst and several small cysts were removed surgically, and the pathologic findings indicated cerebral cysticercosis. This is the first reported case of radionuclide imaging in cerebral cysticercosis and suggests that cerebral cysticercosis be included in the differential diagnoses in patients who are from regions in which this disease is endemic.

A patient with a rare mixture of osteosarcoma and adenocarcinoma of the breast is described. Intense uptake by the tumor of99mTc-labelecl diphosphonate was noted on the bone scan. Bone trabeculae and osteoid tissue were noted in the tumor tissue. It is implied that these structures were labeled with the radiopharmaceutical and were responsible for the intense uptake by the tumor. Bone scans can be of help in an occasional breast tumor.

An adrenal pseudocyst was visualized as a persistent “cold” suprarenal mass on a radionuclide renal angiography study using99mTc-Sn-DTPA. A99mTcpertechnetate blood pool study performed shortly after the renal study again showed an avascular mass, and visualization of vascular abdominal structures was striking. This prolonged and excellent visualization of blood pools is attributable to pertechnetate labeling of red cells from the residual small amounts of circulating Sn (II) derived from the99mTc-Sn-DTPA given shortly before. Experiments confirmed that over 95% of pertechnetate may label red cells when as little as .001 mg Sn per kg of body weight is given (here, in the DTPA preparation) 45 minutes prior to pertechnetate administration. This in vivo labeling of red cells may be turned to advantage in visualizing blood pools during the study of abdominal masses.

Four children with discitis were studied with99mTcpolyphosphate bone scintigraphy. All showed narrowing of the intravertebral disc space with accentuated nuclide accumulation in adjacent vertebral bodies. This image pattern may be unique to discitis and when occurring in a young child with a painful back should strongly suggest that diagnosis.
