摘要:
This paper sets forth generic guidelines for the clinician to observe in building and practicing a personal integrative method in family therapy. Principles are articulated around five foci: (a) the need for a personal paradigm, (b) the assimilation of aspects of scholastic approaches, (c) the role of the person of the therapist, (d) the adaptation of the model to specific cases, and (e) the pathways toward the development of a personal integrative model. An informed view of integrative practice is emphasized, in which a theoretical base is created, strategies and techniques are related to this paradigm, and the treatment of specific cases is linked to the general model.

In working with families with symptomatic children, family therapists may be so focused on the systematic aspects of the problem that they inadvertently neglect the child's individual issues. Some family therapists, following Bateson, use “epistemology” in a peculiar, non‐traditional way. Nothing is gained, and much appears to be lost, by this practice. This paper argues that family therapy is failing to attend to the contexts in which family mental health services are provided and, therefore, is losing touch with the realities of family services in communities. We present a model for describing the institutional contexts of family mental health treatment in North America, and explore how these contexts influence family treatment. The model proposes that family mental health care can be categorized into three levels, analogous to the levels of the health care delivery system: (a) primary, (b) secondary, and (c) tertiary care. These levels represent systematically different contexts for family treatment; each has unique advantages and limitations. Translating treatment methods across levels can be hazardous because of differences in contexts. We argue that delineating the contextual levels of family mental health care can encourage more fruitful and respectful collaboration among the diverse professional groups working with families.

The Family Health Scale is an instrument designed to quantify the quality of family functioning from the perspective of an external clinical observer. Rating may be based on whatever information is available on the family but, ideally, should be derived from a valid standardized method of direct observation like the Family Task Interview (Kinston and Loader, 1986). Clinical judgement must be exercised in rating, and new methods to enhance its quality have been devised. The scale may be used with nonlabelled, as well as psychiatrically labelled, families. This study concerned the extent to which systemic and nonsystemic clinicians differ in their diagnostic processes. Experienced therapists, identified either with a family systems or a psychodynamic orientation, viewed a stimulus film of a family session and individual interviews with each parent and identified patient. Subjects listed “clinically relevant factors” and formulated hypotheses. Content analyses showed that the systemic clinicians, in contrast to the nonsystemic ones, identified as relevant a greater number of different subsystems, more triads, and fewer monads. One of the difficulties in conducting research that compares violent to nonviolent marriages, is determining exactly which couples are violent and what is the direction of that violence: husband‐to‐wife, wife‐to‐husband, or bi‐directional. To illustrate this problem, data on the incidence of violence in 103 couples are presented, based on wife's report only, husband's report only, or both spouses' reports. Incidence figures based on the report of either spouse were approximately 100% higher than the incidence based on agreements between spouses. Although slightly more wives than husbands reported severe husband‐to‐wife aggression, other forms of aggression were reported by more husbands than wives. This paper describes four interventions frequently used by experiential therapists during the trial of labor to foster greater intimacy in the therapy relationship. A brief overview of the trial of labor stage of the therapy process is given, followed by a description of the four interventions. Case examples are included for clarification.
