How processes of family functioning are conceptualized influences a therapist's understanding and use of immediate experiential phenomena in the therapy setting. This article outlines two basic formulations that generate different conceptual perspectives of immediate phenomena. One recognizes that the family system has a time‐enduring stability, while the second postulates that the family system has an immediate and temporary organization. The latter position is developed as an appropriate and useful application of systems thinking. Integrating systems thinking and Gestalt Therapy provides a framework for this application, one that recognizes individual experience as organized and as embedded in a mutual causal, self‐maintaining system of the whole. A method of therapy for families with an acting out adolescent or young adult is presented, as is a rationale for the method. The aim of the approach is to alter the underlying problem of immaturity that is hypothesized to exist in these families. Emphasis is placed on working with the parents in a systematic way over an extended period of time. As progress is made by the parents, an emotional crisis is created in the adolescent that can then be used by him as the basis for extensive change. The therapy of the parents proceeds in phases: 1) preparation for a long‐term effort; 2) rethinking and objectively evaluating the nature of the relationship with the adolescent in specific areas; 3) translation of the newly gained perspective into action positions vis‐à‐vis the adolescent; and 4) non‐child‐focus

The initial, evaluation phase of family therapy is of decisive importance within the systemic perspective. Contacting a therapist is always a complex action for a family. The therapist's evaluation must include the possibility that continued contact may influence the family negatively and limit their potential for change. If there appears to be a logical context in which continued therapy can be justified, it will be necessary to use the first three or four sessions to collect essential data and to organize them meaningfully. It is essential to clarify the circular interaction that is maintaining the symptom, as well as the presence of specific family structures and traditions, and the kind of relationships the family attempts to establish with the therapist. On the basis of this information, the therapist can build models of the family's functioning. These models, according to general system theory, make possible logical intervention in complicated, living systems, without distorting them by artificially splitting them into their component parts. This paper describes a class of paradoxical interventions the author calls parental reversals which involve inducing parents to reverse either their injunctions or their position regarding a problem with a child. A taxonomy, as well as guidelines for the selection and implementation of these paradoxical interventions, is presented.

Theorists in family therapy have written about the difficulties in making attempts to move from a presenting problem of a child to a focus on marital tension. Difficulties include the presence of resistance and the risk of alienation between family and therapist. Proposed in this article is a technique using paradoxical prescription with families to successfully move away from the identified patient to an emphasis on family and marital dynamics. Three clinical illustrations are presented to demonstrate the procedure. Some research questions are posed regarding the use of this technique and the need for further understanding and refinement.

The relationships between child vulnerability to stress, birth order, and stage of family development were assessed. Four stages were identified: Stage I involves 2 parents and their 2 or 3 natural children; Stage II, following divorce, involves mother and her 2 or 3 natural children; Stage III following remarriage, involves mother, stepfather, and mother's 2 or 3 natural children; and Stage IV involves mother, stepfather, mother's 2 or 3 natural children from her first marriage, and the single child from her current marriage. In general, vulnerability of first borns and last borns shifted across stages in a complementary fashion. First borns were most vulnerable in Stage III, least vulnerable in Stage IV, and at intermediate levels in Stages I and II. Last borns were most vulnerable in Stage IV and least vulnerable in Stage HI. The results were interpreted in terms of a linkage between emotional symptoms, gains and losses in privileged positions in the family, and changes in mother‐child closenes

This paper details the structure and content of an introductory family therapy course offered in an academic department. The parallel processes between teaching and therapy are used as a metaphor for the presentation of the course. Joining, restructuring, and consolidation are defined as the three stages of the course, each having distinct goals and methods. Finally, generic issues raised by the proposed teaching model are presented for trainers to consider.

This article describes ways in which a family‐oriented psychologist contributed to a reconceptualization of the appropriate role for the physical therapist in patient care. The article identifies aspects of the changing role of the physical therapist, specifically its expansion to include skilled psychosocial interaction with patient and family for the purposes of reassurance, support and instruction. A primary shift involved changing from focus on the individual and his or her disability to focus on the patient in the context of his or her family. This article, which is based on in‐depth interviews of gay men who had been married, focuses on the nature of the spousal relationship and the almost inevitable marital disruption. It describes the conditions which bring about the man's disclosure of his homosexuality, the means of disclosure he uses, the wife's response, and the interactional effects the disclosure and response have on the marriage relationship. Most of the marriages ended in divorce.
