SummaryWound healing is part of the normal general repair process of the body. Its efficient completion depends on many factors, some physical, eg, pH, oxygen tension and tissue tension, and some biological, eg, cell‐cell interaction and feedback of extracellular matrix on to the cells which produce it. The factors which affect healing are discussed and failures and anomalies in the process described. New techniques which may improve the quality of healing in specialised tissues are explained.Recent research on wound healing has concentrated on the role of the different cell types in the process and also on the effects of micro‐environment on the enhancement or inhibition of cellular activity. The wound is a situation in which cells are apparently operating at the limits of their powers of survival and very slight modification of the micro‐environment can render the tissue either unduly hostile to the cell or can enhance cellular activity. While it is not possible to increase the rate of healing of a clean surgical wound to any useful degree, an understanding of the mechanics of wound healing is essential to a rational treatment of simple or indolent wounds. It is especially important that surgical or medical treatment should complement, rather than compete with, the natural process. Tight bandages, which compress circulation, or excessive washing, which may remove macrophages and dilute bacteriostatic or chemotaxic agents, should be avoided.RésuméLa cicatrisation des plaies est un aspect du processus général de réparation de lórganisme. Son achèvement dépend de facteurs nombreux, les uns physiques (pH, pression d'Oxygène, tension des tissus), les autres biologiques (interaction cellulaires, phénomène de feed back de la matrice extracellulaire sur les cellules qui la produise). Les facteurs qui agissent sur la cicatrisation sont étudiés, les échecs et les anomalies sont décrits. De nouvelles techniques qui peuvent améliorer la qualité de la cicatrisation dans les tissus spécialisés sont expliquées.Une recherche récente sur la cicatrisation des plaies a mis l'accent sur le rôle des différents types cellulaires dans le processus et indiqué les effets du microenvironnement sur la stimulation ou sur l'inhibition de l'activité cellulaire. La plaie est une situation où les cellules travaillent apparemment à la limite de leur pou voir de survie; de très légères modifications de l'environnement peuvent tendre les tissus hostiles aux cellules ou au contraire favorables à l'activité cellulaire. Bien qu'on ne puisse pas accroître la vitesse de cicatrisation d'une plaie chirurgicale de manière utile, une compréhension du mécanisme de la cicatrisation est essentielle pour le traitement des plaies simples et atones. Le traitement chirurgical ou médical devrait compléter pluotot que rivaliser avec le processus naturel. On doit éviter les bandages serrés, comprimant la circulation, les lavages excessifs qui peuvent déplacer les macrophages, diluer les bacteririostatiques et les agents chimiotaxiques.ZussammenfassungDie Wundheilung ist ein Teil der normalen Reparaturvorgänge im Körper. Ihre wirksame Vollendung hängt von manchen Faktoren ab, einige physikalischer (pH, Sauerstoffspannung, Gewebsspannung), andere biologischer Natur, wie zelluläre Interaktion und Feedback extrazellulärer Matrix zu den Zellen, die jene produzieren. Die Faktoren, die die Heilung beeinflussen, werden diskutiert und Versager sowie Anomalien im ganzen Vorgang beschrieben. Neue Techniken, die die Qualität der Wundheilung in spezialisiertem Gewebe beeinflussen, werden erklärt.Neuere Forschungsarbeiten auf dem Gebiet der Wundheilung haben sich konzentriert auf die Rolle verschiedener Zelltypen und auf die Auswirkungen der Mikro‐umgebung auf Förderung oder Hemmung der zelulären Aktivität. Die Wunde repräsentiert eine Situation, in der die Zellen offenbar an der Grenze ihrer Ueberlebensfähigkeit funktionieren und sehr geringe Veränderungen in der Mikroumgebung vermögen das Gewebe entweder der Zelle ungebührlich feindlich zu gestalten oder auch die zelluläre Aktivität zu fördem. Wiewohl es unmöglich ist, die Wundheilungsgeschwindigkeit einer sauberen, chircurgischen Wunde in nützlichem Mass zu steigern, ist das Verständnis für die Mechanismen der Wundheilung wesentlich für eine rationale Behandlung einfacher oder indolenter Wundan. SummaryIn general, 4 types of hypersensitivity reactions can be defined according to their immunological basis and clinical appearance. The differing mechanisms of these responses are described with particular reference to chemical mediators which through their pharmacological actions contribute to the clinical manifestations of hypersensitivity. Chemical mediators may exert their influence locally or systemically through their action on effector, tissues or organs and in addition, may be involved in the recruitment of cells of specific type to the site of the reaction. The possible role of these mediators in hypersensitivity in the equine species is discussed to provide a background for a subsequent paper which will be concerned with the therapy of hypersensitivity conditions.RésuméOn peut d'une manière générale définir quatre types de réactions d'hypersensibilité en se basant sur leurs fondements immunologiques et sur leurs apparences cliniques. Les différents mécanismes de ces réactions sont décrits avec une attention particulière pour les médiateurs chimiques; ceux‐ci, par leur action pharmacologique contribuent aux manifestations cliniques de l'hypersensibilité. Les médiateurs chimiques peuvent exercer leur influence localement ou généralement, en agissant sur les effecteurs, sur les tissus, sur les organes. En outre, ils peuvent participer à la mobilisation de cellules spécifiques au site de la réaction.Le rôle possible de ces médiateurs dans l'hypersensibilitééquine est discuté et prépare le champ à un prochain article traitant du traitement des manifestations de cette hypersensibilité.ZussammenfassungVier verschiedene Typen von Ueberempfindlichkeitsreaktionen können definiert werden, je nach ihrer immunologischen Grundlage und den klinischen Erscheinungen. Die sich unterscheidenden Mechanismen dieser Reaktionen werden beschrieben, wobei besonders Bezug genommen wird auf chemische Mediatoren, die durch ihre pharmakologische Wirkung zu den klinischen Aeusserungen einer Ueberempfindlichkeit beitragen. Chemische Mediatoren können ihren Einfluss lokal oder systemisch geltend machen über ihre Beeinflussung von Effektoren, Gewebe oder Organen, und dazu können sie an der Mobilisation von spezifischen Zellen an der Reaktionsstelle beteiligt sein. SummaryA radiographic technique is described for the equine stifle joint with the horse in the standing position or under general anaesthesia. The method with the animal anaesthetised in dorsal recumbency and the leg extended was preferred because it gave greater flexibility with a better range of views and greatly reduced the safety hazards. In the standing position a useful practical tip for the lateral view was to raise and extend the limb caudally. This provides some flexion and ventral movement of the stifle allowing improved access for the cassette, a more accurate lateral view of the joint and a reduction in exposure.Some of the features of radiographic anatomy, from birth to adulthood, of this rather complex joint are described to form a basis for radiological interpretation in cases of suspected stifle lameness.RésuméUne technique pour la radiographic du grasset du cheval sur l'animal debout ou placé sous anesthésie générale est décrite. La méthode s'appliquant au cheval sous anesthésie générale et placé en decubitus dorsal avec extension de la jambe est préférée: On dispose ainsi d'une plus grande diversité d'angles pour les prises de vue et les risques d'irradiation sont réduits de beaucoup. Pour la radiographic sur l'animal debout, il apparut utile de lever et d'étendre vers l'arrière le membre étudié. Ainsi obtinton une certaine flexion et un déplacement du grasset. Le mouvement permit une meilleure position de la cassette, une meilleure image latérale de l'articulation et une réduction du temps d'exposition.Quelques particularités de l'anatomie radiographique de cette articulation complexe sont décrites, depuis la naissance jusqu'à l'âge adulte. Ceci constitue une base de référence pour l'inter‐prétation radiologique des cas portant suspicion de boiterie du grasset.ZusammenfassungDie Röntgentechnik zur Abbildung des Kniegelenks am stehenden oder narkotisierten Pferd wird beschrieben. Es wird eine Methode bevorzugt, bei der das narkotisierte Pferd auf dem Rücken liegt, das Bein wird gestreckt. Dieses Vorgehen ist flexibel, erlaubt einen Zugang von mehreren Seiten und es reduziert die Gefahr. Beim stehenden Pferd wird für die laterale Aufnahme empfohlen, das Bein anzuheben und nach hinten zu strecken. Dabei kommt es zu einer gewissen Flexion und einer Verlagerung des Gelenks nach unten, was das Placieren der Kassette einfacher macht, die laterale Aufnahme kann präziser und mit reduzierter Exposition angefertigt werden.Einige Eigenschaften der Röntgenanatomie dieses komplizierten Gelenks werden in Abhängigkeit vom Alter der Pferde beschrieben.
