SummaryTreatment of degenerative joint disease (osteoarthritis) is discussed in relationship to pathogenesis of five clinical entities, defined to facilitate discussion:‐ 1) occurring in high motion joints and associated with synovitis; 2) associated with low motion joints; 3) “non‐progressive” articular cartilage erosion; 4) secondary to other identified problems (intra‐articular fractures, ligamentous damage, wounds, septic arthritis, osteochondrosis); and 5) chondromalacia of the patella. In addition to direct damage to articular cartilage, synovitis and capsulitis and depletion of matrical glycosaminoglycans and proteoglycans play important roles. The natural healing response in damaged articular cartilage is inadequate. The principles of treatment are divided into: 1) prevention or treatment of primary causes. These include the effects of track surface, shoeing, and the appropriate resolution of intra‐articular fractures, septic arthritis and osteochondritis dissecans. 2) treatment of active soft tissue disease contributing to articular cartilage degeneration, including rest, physical therapy, synovectomy and administration of anti‐inflammatory drugs, sodium hyaluronate and polysulfated glycosaminoglycans. 3) attempts at treatment of articular cartilage loss or degeneration, including articular cartilage curettage, subchondral bone drilling, osteophyte removal, articular cartilage grafting an

SummaryThe medical records of 20 neonatal foals in which exploratory celiotomies were performed for gastrointestinal disease were reviewed. In all 20 foals, persistent pain and/or progressive abdominal distension were the primary clinical findings influencing the decision to operate. However, ancilliary laboratory data were important to the proper medical management of these foals during anaesthesia and following surgery. Surgical diagnoses of the 20 foals included ileus (nine foals; 45 per cent), small colon obstruction (five foals; 25 per cent), large colon displacement (three foals; 15 per cent), small intestinal displacement (two foals; 10 per cent), and perforated gastric ulcer (one foal; 5 per cent). Seventeen foals were recovered from anaesthesia, 13 of these were discharged from the hospital, seven were alive six months or more following discharge. Sepsis was the cause of death in six of the 10 foals that died following recovery.

SummaryClinical experience continues to expand our understanding of the pathophysiology of pain and injury within the carpal joints. The anatomical and physiological capacity of the equine carpus to neutralise loading stress plays a significant role in the prevention of carpal injury. The ability of joint surfaces to dissipate axial force by transfer to the interosseous ligaments is the principal means by which carpal injury is avoided. In the medial aspect of the intercarpal joint and the dorsal aspect of the radial carpal joint, however, the effects of chronic maximal stress and isolated acute supraphysiological loads cannot be neutralised. Progressive osseous remodelling and stress induced changes within the bone, which outstrip its ability to accommodate increasing loads, lead to the loss of structural integrity and fracture in horses in training.

SummaryThe medical records of 18 neonatal foals, in which exploratory celiotomies were performed for suspected urinary tract lesions, were reviewed. Despite clinical signs and laboratory values indicative of disruption of the urinary tract, three foals did not have a site of urinary tract leakage at surgery. Eight foals had ruptured bladders and seven foals had urachal lesions. Ultrasonography was used as a pre‐operative diagnostic procedure in eight foals to evaluate the presence of free peritoneal fluid and urinary tract integrity.
