The intent of the study described in this article was to determine (a) if patients with chronic obstructive pulmonary disease (COPD) who regularly participate in less vigorous exercise programs, such as the program of systematic movement sponsored by the American Lung Association, differ from those who do not participate; and (b) whether these differences, if they exist, persist over time. Two convenience samples of COPD patients—those who practice systematic movement by participating in a pulmonary exercise program (PEP) and those who do not—were obtained through the American Lung Association of Maryland. Demographic information and health history were obtained from the participants and their physicians. They all completed the vertical visual analogue dyspnea scale (VADS) and the Beck Depression Inventory (BDI) on two occasions 8 weeks apart and described the strategies they used in dealing with their illness. Differences were found between the groups on the BDI and VADS at both measurement times. Those who participated in the program had lower depression (5.9 versus 17.75) and lower dyspnea (58.9 versus 78.8). These findings persisted 8 weeks later, with exercisers having depression levels of 5.5 versus 19 for the nonexercisers and dyspnea levels of 55.2 versus 79.4 for the nonexercisers. Dyspnea, the sensation of uncomfortable breathing, is the primary activity‐limiting symptom leading to reduced functional ability in chronic obstructive pulmonary disease (COPD). Patients with severe COPD report a marked increase in the sensation of dyspnea with routine tasks that require arm use, especially activities necessitating unsupported arm elevation. Dyspnea is associated with alterations in respiratory muscle function, such as an increase in muscle force requirement, a reduction in respiratory muscle strength and endurance, and an increase in the recruitment of the rib cage and accessory muscles. Unsupported arm exercise (UAE) further compromises respiratory muscle capacity for ventilation because it requires the muscles' concomitant recruitment in the maintenance of chest wall stabilization. The purpose of this study was to determine the degree of loneliness felt by young and middle‐aged adults in a long‐term rehabilitation center and the relationship among such variables as loneliness, age, and length of institutionalization. A convenience sample was taken from a long‐term rehabilitation center in Vancouver, BC, Canada. Findings indicate that loneliness is present among young and middle‐aged adults in this long‐term rehabilitation center and is more prevalent among those who have been admitted more recently. The nursing department of a 154‐bed acute rehabilitation facility, cognizant of the changing trends in health care and responding to feedback from staff, developed and implemented a comprehensive documentation system. The previous system had been fragmented, inconsistent, and inefficient. The development of the new system focused on the complex needs of the rehabilitation client and the equally complex standards required by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO), the Commission on Accreditation of Rehabilitation Facilities (CARF), and insurance carriers. The final product, which was based on the nursing process and functional health patterns, encompassed the following areas from admission to discharge: providing feedback on clients' functional abilities and progress toward goals, satisfying requirements of the 1990 JCAHO standards, and, finally, using a flow sheet that saves nursing time and increases objectivity.
