SUMMARY1The effects of intravenous (standard apnoeic) and intra‐arterial doses of succinylcholinc chloride (SCh) on peripheral circulation (forearm, hand, skeletal musculature, skin, mucosa of the sigmoid colon), arterial blood pressure and heart rate were studied in 15 unanaesthetized and 17 anaesthetized subjects.2Standard apnoeic doses of SCh injected intravenously in anaesthetized subjects produced circulatory changes usually of a biphasic character, a sudden initial decrease in peripheral blood flow and heart rate followed by an increase throughout The apnoeic period; there was also a rise in arterial blood pressure. The initial decrease occurred synchronously with the preparalytic activity of the skeletal musculature (twitchings), but unlike the latter also after repeated injections of SCh. The magnitude of the initial decrease was, on an average, 16 per cent for the peripheral circulation, and 7 per cent for heart rate. The subsequent average increase amounted to 16 per cent above the initial control value for the peripheral circulation, 8 per cent (5 mm Hg) for the systolic, 21 per cent (6 mm Hg) for the diastolic and 9 per cent (4 mm Hg) for the mean arterial blood pressures.3Doses of SCh which caused local muscular relaxation on continuous infusion into the brachial artery of unanaesthetized subjects also caused an average blood flow increase in forearm and hand of 43 per cent. This blood flow response remained only during the first few minutes, and then gradually decreased to the control level during the course of the next 10–15 minutes.4After the second or third repeated intravenous injection of SCh, but not after further injections, a sudden and marked bradycardia sometimes occurred, with a duration of up to 3 minutes.5With repeated intravenous injections of SCh in a standard apnoeic dose, the size and duration of both the circulatory reactions and the duration of the apnoeic period gradually decreased (‘tachyphylaxis’).6The individual sensitivity to SCh was rather variable. This was particularly evident in patients with a labile and hyperkinetic circulation (‘vasoregulatory asthenia’) in whom the average blood flow increase during continuous intra‐arterial infusion of SCh was considerably higher than in the control group.7The circulatory changes after an intravenous injection of an apnoeic SCh dose should be evaluated in relation to the various interfering factors, such as anaesthesia, atropine premedication, endotracheal intubation, positive‐pressure ventilation, hypoxia, hypercapnia, hypothermia during anaesthesia, and acidosis.8The possible mechanisms by which SCh exerts its circulatory effects are discussed, especially the direct effects on the vessels and heart and the indirect effects via nervous or humoral mediators. It is concluded that SCh seems to be a general vasodilator substance. A review of the literature shows that the vasodilator effect of SCh also exists in circulatory areas of the body not directly studied in the present investigation. The vasodilatation is partially caused by a local action of SCh on the peripheral circulation, as demonstrated by the effect of intra‐arterial infusion. It is suggested as a possible explanation that SCh may cause initial stimulation (contraction) and subsequent relaxation not only of skeletal, but also of smooth muscle, especially in blood vessels.9It is concluded that the circulatory effects of SCh are of clinical significance.ZUSAMMENFASSUNG1Bei 15 nicht anästhesierten und 17 anästhesierten Personen wurden die Wirkungen von Succinylcholinchlorid (SCh) auf die Durchblutung in Unterarm, Hand, Skeletmuskulatur und Haut des Unterarmes sowie Schleim‐haut des Colon sigmoideum, auf den arteriellen Blutdruck sowie auf die Herzfrequenz untersucht. Bei den anästhesierten Personen wurde SCh intravenös in klinisch üblichen apnoeischen Dosen injiziert, bei den nicht anästhesierten Personen erfolgten Infusionen von skeletmuskelrelaxierenden SCh‐Dosen in die A. brachialis.2Intravenöse Injektion apnoeischer SCh‐Dosen bewirkte gewöhnlich biphasische Kreislaufreaktionen. Diese bestanden aus einem initialen Abfall von peripherer Durchblutung und Herzfrequenz mit nachfolgendem, während der Apnoeperiode anhaltenden Wiederanstieg dieser Grössen und des arteriellen Blutdruckes über die Ausgangswerte hinaus. Der initiale Abfall erfolgte gleichzeitig mit der präparalytischen Skeletmuskelaktivität (Faszikulationen), jedoch im Unterschied dazu auch nach wiederholten i.v. SCh‐Dosen; seine Grösse betrug durchschnittlich 16 % für die peripheren Durchblutungswerte und 7 % für die Herzfrequenz. Der nachfolgende Anstieg über die Ausgangs‐werte hinaus betrug durchschnittlich 16 % für die peripheren Durchblutungswerte, 8 % (5 mm Hg) für den systolischen, 21 % (6 mm Hg) für den diastolischen und 9 % (4 mm Hg) für den Mitteldruck.3Infusion muskelrelaxierender SCh‐Dosen in die A. brachialis nicht anästhesierter Personen bewirkte einen Durchblutungsanstieg in Unterarm und Hand um durchschnittlich 43 %. Dieser hielt nur während der ersten Infusionsminuten an; 10–15 min nach Infusionsbeginn waren ‐ trotz fortdauernder SCh‐Infusion ‐ die Durchblutungswerte gewöhnlich wieder auf die Ausgangs‐werte abgefallen.4Nach der 2. und 3., aber nicht nach noch weiteren wiederholten i.v. SCh‐Injektionen, kam es gelegentlich zu plötzlichen, bis zu 3 min langen Perioden mit ausgeprägter Bradykardie.5Bei wiederholten i.v. Injektionen klinisch üblicher apnoeischer SCh‐Dosen nahmen sowohl Grösse und Dauer der Kreislaufreaktionen wie auch die Dauer der Apnoeperioden allmählich ab.6Die individuelle Empfindlichkeit für SCh ist unterschiedlich. Dies war besonders deutlich bei Patienten mit labiler und hyperkinetischer Zirkulation (‘vasoregulativer Asthenie’), die auf SCh‐Infusionen in die A. brachialis mit stärkeren Durchblutungsanstiegen in Unterarm und Hand reagierten als die Personen in der Kontrollgruppe.7Die Kreislaufreaktionen, die bei anästhesierten Personen nach i.v. SCh‐Injektion erfolgen, können nicht alle mit Sicherheit nur auf das SCh bezogen werden. Es ist nicht auszuschliessen, dass sie, zumindestens teilweise, auch durch andere Faktoren ausgelöst werden, wie vor allem die Anästhesie, aber eventuell auch die Atropinprämedikation, endotracheale Intubation, Über‐druckbeatmung, eine mögliche Hypoxie, Hyperkapnie, geringfügige Hypothermie während der Anästhesie oder Azidose.8Die möglichen Wirkungsmechanismen für die Kreislaufeffekte des SCh werden näher diskutiert, besonders im Hinblick auf direkt oder indirekt (nervös oder humoral) übermittelte Effekte auf Herz und Gefässe. SCh ist wahrscheinlich eine allgemein vasodilatierende Substanz. Literaturangaben zeigen, dass SCh auch noch in anderen als den hier untersuchten Kreislaufgebieten vasodilatorisch wirkt. Durchblutungszunahmen bei intraarteriellen SCh‐Infusionen deuten an, dass der vasodilatorische Effekt zumindestens teilweise durch eine direkte Gefässwirkung des SCh erfolgt. Es wird vermutet, dass SCh nicht nur bei der Skeletmuskulatur, sondern auch bei der glatten Muskulatur (besonders derjenigen der Blutgefässe) zu initialer Stimulation (Kontraktion) und nachfolge

SUMMARYA total of 1163 patients were premedicated with haloperidol. In 94% of the patients, 5 mg haloperidol given intramuscularly produced excellent results. This consisted of a vegetative stability and a condition of psychic indifference and removal from emotional and sensory stimuli. This condition, advantageous for the patient, could be extended to last throughout the entire day of operation thanks to the prolonged effect of the drug.The need for analgesics postoperatively was significantly decreased.Haloperidol has a very powerful antiemetic effect. Only 10 patients out of the entire series vomited and only three experienced nausea.The “inner anxiety” appearing in 4% of the patients probably depends on the patients' psychomotory basic attitude, expressing itself in a depressed and tense mood. Such patients should not be premedicated with haloperidol. In patients with increased muscle tonus and organic disorders of the central nervous system, haloperidol is contra‐indicated.The unforeseen development of extrapyramidal symptoms in two patients is the only disadvantage that has appeared in our investigation on the use of haloperidol as a premedicant.ZUSAMMENFASSUNG1163 Patienten sind mit Haloperidol (Haldol®) prämediziert worden. 5 mg Haloperidol, i.m. verabreicht, hatte in 94% der Fälle einen befriedigenden Erfolg, der sich in einer vegetativen Stabilität und in einem von psychischer Indifferenz und einer Distanz einem emotionellen und sensorischen Stimulus gegenüber, manifestiert. Dieser für den Patienten günstige Zustand konnte, infolge des langen Effektes des Haloperidols, auf den ganzen Operationstag erstreckt werden. Der postoperative Analgeticabedarf ist deutlich herabgesetzt. Haloperidol ist antiemetisch sehr wirksam. Insgesamt haben nur 10 Patienten erbrochen und 3 an Übelkeit gelitten.Die in 4% der Fälle entstandene “innere Unruhe” beruht wahrscheinlich auf der psychomotorischen Grundkonstellation der Patienten, die sich in einer deprimierten und gespannten Stimmung zeigt. Deshalb sollten solche Patienten mit Haloperidol nicht prämediziert werden. Bei Patienten mit gesteigertem Muskeltonus und organischen Krankheiten des ZNS ist Haloperidol kontra‐indiziert.
