摘要:
Intestinal transplantation offers hope to children with intestinal failure and life-threatening complications of parenteral nutrition (PN). As more transplant centers are adding intestinal transplantation to their existing programs, new challenges are presented to nursing professionals. This three-part series will provide information for nursing professionals regarding evaluation for intestinal transplantation, donor preparation, the surgical procedure, immediate postoperative care of the transplant recipient, complications of intestinal transplantation, and long-term care of the intestinal transplant patient. Part 1 will discuss the indications for intestinal transplantation, the evaluation of patients for intestinal transplantation, the process of listing patients for intestinal transplantation, and the waiting time for donor organs for intestinal transplantation. The information presented represents the approach taken at University of Nebraska Medical Center.

In May 1998, we celebrated the 25th anniversary of the Society of Gastroenterology Nurses & Associates (SGNA). It was in 1974 that a group of dedicated people met in San Francisco under the leadership of Marna Harmon Schirmer to found an organization dedicated to the professional needs of health care providers working in the gastroenterology diagnostic and treatment setting. Through the efforts of these dedicated people and those who followed them, this organization has grown into the recognized specialty SGNA is today. SGNA has its roots in the historical development of clinical gastroenterology practice and the diagnostic tools made possible through advances in technology. Throughout the years, these changes continue to affect not only the delivery of patient care but also the specialty of gastroenterology nursing. This article explores the history of GI nurses-how we began, where we are now, and what we can expect in the future.

Approved clinical trials involving gene therapy have been taking place for less than 10 years. As more knowledge is gained about genetics and disease, nurses must have a fundamental understanding of what genes do, how gene therapy is administered, and the risks and potential benefits of treatment. New methods for the administration of adenoviral vectors for cancer treatment in the endoscopy lab are currently being developed. By staying abreast of new treatment methods using gene or viral therapy, the gastrointestinal nurse can actively participate in clinical trials. A basic background of genetics and gene therapy in cancer is discussed. Safety issues and nursing implications regarding the administration and recovery of patients following gene therapy in the GI endoscopy lab are also addressed. An actual case study of a patient who participated in a study utilizing an attenuated adenovirus as investigational therapy for pancreatic adenocarcinoma is presented.

A Study of Alternative Health Care Use for Gastrointestinal Disorders |

The discomfort and frustration often experienced by patients with gastrointestinal (GI) disorders may lead many to seek alternative health care (AHC). This study was conducted to describe AHC use by patients with GI disorders in a convenience sample (N= 73) from a tertiary hospital in Florida. AHC was explored within social exchange theory. Measurement instruments included the Alternative Health Care Gastrointestinal Sociodemographic Questionnaire, Alternative Health Care Use Questionnaire, and Alternative Health Care Advantages/Disadvantages Questionnaire. The study indicated that 32 subjects (43%) had used AHC for their GI disorders during the past 2 years. Most frequently used AHC included relaxation therapy, herbs, lifestyle diets, megavitamins, massage, and home remedies. There was a greater use of AHC by young persons (t= 2.39,p= .02) and by those not retired (χ2= 4.58,p= .03). AHC was associated with perceived rewards (r= .38,p= .03) and perceived profits (r= .38,p= .03). AHC was not associated with type or duration of GI disorder or other demographic variables. Subjects specifically cited benefits with relaxation therapy, vegetarian diets, spiritual healing, fish oil for inflammatory bowel disease, and use of milk thistle for hepatitis.

Most nurses will care for and provide health information about cancer to a patient at some point in their careers. Cancer care will change dramatically in the coming years as a result of the translation of information gained from the Human Genome Project into clinical practice and the enhanced understanding of cancer at a molecular level. A select group of cancers, known as hereditary cancers, result from mutations in the germline that confer a greatly increased lifetime risk of developing cancer. Advances in technology and discoveries stemming from the Human Genome Project now provide the means to test individuals for the presence of mutations associated with some known hereditary cancer syndromes. Of particular importance to gastroenterology nurses are hereditary colorectal cancer syndromes. Although many ethical, legal, and psychosocial issues associated with testing remain unresolved, predisposition genetic testing is having a significant impact on health care. Nurses will have vital roles in the future assessing patients and their family members for increased cancer risk, educating them about the availability of testing, making referrals for cancer genetic counseling and risk assessment, and providing follow-up care in the community for patients found to be at increased risk.
