Percutaneous endoscopic gastrostomy is offered to many elderly, ill, debilitated patients. Nurses are key individuals in helping patients and families decide if this is the appropriate choice given the prognosis, goals of therapy, and patient wishes. In this article the author presents a review of the literature on percutaneous endoscopic gastrostomy including indications, contraindications, outcomes, and complications. Special risk factors in the aged, such as aspiration pneumonia and morbidity due to underlying diseases, are addressed. The decision to start, continue, or stop enteral feedings is difficult. Advance directives as well as open discussion with all involved parties are helpful. An educated prediction of lifespan, given underlying pathology, and patient/family wishes needs to be considered by knowledgeable practitioners before offering a percutaneous endoscopic gastrostomy as a therapeutic option.

Patients with chronic atrophic gastritis intestinal metaplasia (IM) are suspected of being at increased risk for the intestinal type of gastric cancer. Diet is one of several etiologic components that may contribute to the development of the IM lesion. One hundred patients (50 with IM and 50 without IM) were evaluated to determine their dietary intake of nitrosating agents and vitamin C and their smoking habits. Patients with IM reported a higher consumption of two foods rich in nitrosating agents, bacon (.28 vs. .10,p= .02) and sausages (.16 vs. .02,p= .01), compared to patients without IM. Smoking and vitamin C intake were similar in both the IM-positive and the IM-negative groups.

During the past 2 years, the staff in the Endoscopy Department at Georgetown University Hospital noticed an increase in the number of procedures being performed in the x-ray department. The nursing staff were becoming concerned with the amount of x-ray exposure they were receiving. The purpose of this article is to describe the process in which the nursing staff are equally rotated through radiology based on a daily log of fluoroscopy exposure for each nurse. At the completion of each x-ray procedure, the nurses chart the amount of x-ray and fluoroscopy exposure. Each nurse wears a total body film badge while a second badge is worn at the collar. Monthly dosimetry reports are made available for each nurse to review. After 1 year of monitoring by this method, it was found that fluoroscopy exposure was evenly distributed to each nurse.

After failure of medical therapy, children with refractory inflammatory bowel disease may require surgical intervention. In this article the authors describe one child in whom, after traditional therapies for refractory ulcerative colitis failed, full remission of the disease occurred after the addition of 6-mercaptopurine and cyclosporine A to her medical regimen. 6-Mercaptopurine has been used with increasing frequency in refractory inflammatory bowel disease; the use of cyclosporine A for this condition is still under investigation.
