Constipation is a common problem and is frequently taken for granted by both the lay and medical populations. Although sometimes easily resolved it can prove to be a lifelong, debilitating problem, both medically and sociologically. Most frustrating is the category of idiopathic chronic constipation, a problem without identifiable etiology and diagnosed by exclusion. This paper takes an in-depth view of chronic and more specifically, idiopathic chronic constipation, discussing etiology, diagnosis and treatment. Two case histories are presented illustrating opposite ends of the spectrum in terms of severity. The objective of this paper is to enhance the reader's knowledge about a “common” problem and to increase the future appreciation for patients experiencing this alteration in bowel functioning.

New technology has combined the endoscope with ultrasound in an effort to enhance the visualization of the gastrointestinal tract. With a modified standard endoscope that has an ultrasound transducer built into the tip, high frequency ultrasonic beams can be targeted in close proximity to existing lesions. This results in better quality resolution which enhances the evaluation of the targeted lesion. In addition, esophageal wall thickness can be evaluated and assessed as to its role in esophageal function.

Bloody diarrhea is a common problem in children. It is important to distinguish bloody diarrhea from other causes of rectal bleeding. Bacterial infections and parasitic infestations are responsible for most of the cases. Milk allergy is a frequent cause in young infants. Chronic inflammatory bowel disease occurs in older children. The cause is most often apparent after a thorough history and physical assessment is performed and the stool is examined for pathogens. Proctosigmoidoscopy with biopsy is the procedure of choice if the stool tests for pathogens are negative. A few children require further diagnostic evaluation by colonoscopy and/or radiographic procedures.

Pancreatitis is a recognized serious complication of endoscopic retrograde cholangiopancreatography (ERCP). The pressure with which contrast media are injected may be a risk factor for developing pancreatitis. We, therefore, designed a study to determine if there were differences in pressures generated by experienced and inexperienced gastroenterology nurses and how much training would be required to instruct individuals to inject to a specific pressure. With the use of manometrically guided practice sessions, it was determined that inexperienced operators initially inject with significantly high pressure but can learn to inject accurately to a specific pressure as rapidly as experienced operators. Injection training sessions may be beneficial in minimizing the potential risk factor of hydrostatic pressure induced endoscopic retrograde cholangiopancreatography-associated pancreatitis.

The purpose of this paper is to provide the novice nurse author with guidelines to follow for preparing and writing a manuscript. Nurses are responsible for the education of other nurses, and writing and publishing play a major role in providing current knowledge within the profession. By illustrating the mechanics of the writing process, it is hoped that more nurses will approach the task of sharing their ideas on paper.

Developing and nurturing professional relationships with support departments enhance the function of any nursing area. Building mutual goals and sharing common knowledge of patient cases can create improved patient care in the endoscopy suite and the inpatient unit. This article discusses possible methods to achieve team spirit and a sense of common purpose for the gastrointestinal nurses and associates who share a client in the procedure area and the ward.

Rowasa (mesalamine) rectal suspension enema, a new product by Reid-Rowell, represents a major breakthrough in the treatment of distal ulcerative colitis. It provides patients with an alternative therapy to the current oral and enema preparations. This treatment reduces the incidence of unwanted side effects and toxicity associated with oral therapy.
