Summary:Although rare, complications of rotator cuff repair surgery can be severe. The most severe complications include deltoid detachment, axillary nerve injury, anterior–superior instability, and infection. With the possible exception of infection, all of these complications can be avoided with proper adherence to surgical principles. Treatment with revision procedures is technically demanding but can bring significant pain relief. Functional gains also can occur but are less predictable.

Summary:Uses of shoulder arthroscopy for both diagnostic and therapeutic purposes have greatly expanded in recent years. As applications of this procedure increase, so do potential problems and complications associated with its use. General pitfalls associated with preoperative diagnoses, positioning, traction, optics, irrigation, and cauterization are discussed. Procedure-specific complications are reviewed, and the relevant literature is summarized. Known postoperative complications of shoulder arthroscopy are also cited, illustrated by pertinent references. Discussion of each of these issues has been expanded to include appropriate preventative strategies.

Summary:The use of metal implants for the surgical management of problems about the shoulder has become a common and effective part of our treatment armamentarium. The anatomy of the shoulder, however, makes it less tolerant of rigid metallic implants. Therefore, extreme care should be exercised in their use. This should include adequate exposure or arthroscopic visualization during insertion, intraoperative radiographs to document accurate placement, and follow-up radiographs to identify potential problems related to migration, loosening or breakage. Finally, to avoid some of the complications associated with the use of metallic implants, nonmetallic alternatives should be utilized for the treatment of many of the common clinical problems encountered. Procedure selection should depend primarily on the risk/benefit ratio, not the ease with which the procedure can be performed.
