SummaryRegenerative injection therapy (prolotherapy) is a treatment modality that can help cure chronic pain when it is the result of specific musculoskeletal causes. Chronic pain has no physiologic or protective role in humans and becomes a disease if not relieved during its acute stage. It therefore demands a change in treatment strategy and requires physicians to focus on the pain as a chronic disease and not an acute illness. Modifying the underlying physiology is often the only way to modify this pain. By recruiting the body’s own healing forces, new connective tissue can be generated at specific locations that can change the natural history of the chronic pain. Regenerative injection therapy is a treatment that has helped thousands of patients overcome their chronic pain.

SummaryRarely has the spinal pathology been understood through the eyes of dynamic transition. This is especially true when contemplating the etiology of low back pain. The back is dynamic. To consider the spine independent of the integrated neurologic, fascia, and muscular attachments is like eating spaghetti without the sauce. You can do it, but you are missing the best part. Making no apology for the revolutionary renaming of the lumbosacral articulation based on function, this article is meant to shake up experts who regard the spine as static, and back disease as the outgrowth of a pickled product from the anatomy laboratory. To understand the spine is to go beyond spinal biomechanics and enter the realm of osteopathy where Alice knows it is not enough to catch the white rabbit. It is more important to find truth. That is magic.

SummaryCarpal tunnel syndrome is the most common entrapment neuropathy. Diagnosis is made on the basis of the history and physical examination, including provocative testing and neurodiagnostic studies. Other underlying conditions should be ruled out first or, if present, treated accordingly. Although surgical release of the transverse carpal ligament is considered the gold standard for treatment, nonsurgical treatment of early carpal tunnel syndrome often can be effective. The author outlines the diagnosis and nonsurgical treatment of carpal tunnel syndrome.

SummaryThe objective of this study is to review research on acupuncture in the treatment of osteoarthritis of the knee, to document adverse effects, and to identify patient parameters associated with outcomes. A literature search was completed using Alt Med., CINAHL, MEDLINE, and the Cochrane database, since 1977, using the key words acupuncture, knee, osteoarthritis, and pain. Studies were excluded that involved electrical stimulation of acupuncture needles, injection therapies, and other knee pathologies including patellar femoral pain, rheumatoid arthritis, and gonarthritic pain. Five trials representing 205 patients with osteoarthritis of the knee were identified. There are rare but mild side effects from acupuncture that include bruising and nausea. The evidence suggests that acupuncture reduces pain in osteoarthritis of the knee. The best results for reducing pain in osteoarthritis of the knee with acupuncture treatments occurs in patients who report the highest pain levels. Preliminary results suggest that long-term continuous treatment with acupuncture sustains the effectiveness in pain reduction. Also, there is sustained pain reduction 4 months after an initial 6-week treatment regimen was discontinued.

SummaryAmerican consumers have clearly expressed their desire to incorporate complementary and alternative therapies, including herbal medicine, into their health care regimes. The use of herbs is rooted in history and has formed the foundation of medicine. Not so established is the use of herbs in combination with modern pharmaceutical medicines. A lack of federal regulation in the herbal industry, variations in herbal product constituents, and failure of patients to report herb use to their physicians have contributed to the dearth of information. Interactions between herbs and drugs may be additive, synergistic, or antagonistic and therefore may be beneficial or adverse. It is not within the scope of this article to touch on all the known herb-drug interactions, but rather to provide an overview of the evidence for popular herbs including St. John’s wort, kava, ginkgo, ginger, and ginseng so that the practitioner can gain a realistic perspective on herb-drug interactions and make the necessary recommendations to ensure the safety of their patients.

SummaryEnergy medicine is a field of complementary therapy based on the interactions of the human energy field with other energy fields (human or other). The human body is surrounded by and emits various kinds of energy: electrical, magnetic, light, and subtle. Health and disease appear to be related to the flow or lack of flow of our energetic systems. Modern quantum physics provides the framework for energy medicine. Einstein demonstrated that matter and energy are interchangeable and therefore our bodies are energy. Matter, then, does not exist in certain places with certainty, but shows tendencies to exist. A particle can also act as a wave, which becomes a probability wave, and the interactions between waves (particles) are probabilities of interconnections. In this view, the universe becomes a dynamic web of inseparable energy patterns, and the classic idea of analyzing the world by separate, independent parts becomes less plausible. Therapeutic touch, healing touch, Reiki are probably the most commonly used energy healing modalities, and all of these involve a practitioner who assesses the client’s energy field and then rebalances it using a variety of techniques. A form of energy medicine has been used for many years in orthopaedics for healing nonunion fractures: the pulsed electromagnetic field. This device creates an external magnetic field that induces an electrical current in bone that has a similar waveform as the piezoelectric current induced in bone with mechanical stimulation. Data also support the emission of a biomagnetic field from the hands of therapeutic touch and QiGong practitioners. These fields are similar in magnitude and quality to pulsated electromagnetic fields that have been used to heal chronic nonunion fractures. There is evidence for the use of energy medicine in ligament and tendon injuries and for wound healing.

This Law of Similars is one of the fundamental principles on which this extensive and most comprehensive medical discipline is based. This article provides a general overview and historical perspective of homeopathic medicine, as well as some practical applications for the orthopaedic patient.
