Within the same family erythrocytosis occurred in the mother, in three of five children and possibly in one child of an affected daughter. The occurrence of the anomaly in successive generations as well as the distribution of affected and healthy persons in the offspring of affected individuals are in excellent agreement with autosomal dominant inheritance.Hemoglobin function (P50) and red cell function (2,3‐diphosphoglycerate) were normal. Plasma erythropoitin activity in plasma of affected individuals was undetectable when measured by cell culture assay. Only low levels of activity, comparable to that occurring in normal plasma, were detected using59Fe incorporation in erythrocytes of exhypoxic, polycythemic mice and this activity did not increase upon phlebotomy of 15–20% of the blood volume. A case of previously undiagnosed congenital spherocytosis that became manifest during lymph node tubercolosis is described. It is suggested that mild forms of congenital spherocytosis are common, but may pass unnoticed unless revealed by an intercurrent infection.

Sixteen patients with solitary plasmacytomas of bone (SPB) have been reviewed with regard to prognosis, response to treatment, and protein findings.Three patients developed multiple myeloma (MM), and 2 of the patients died due to dissemination. Local recurrence was not seen. Moreover, this study confirms other reports as for the good prognosis of SPB.Intracytoplasmatic immunoglobulin (Ig) has been characterized in tumour tissue from 10 cases by the peroxidase‐anti‐peroxidase (PAP) method. The tumour tissue stained positively with monoclonal Ig in 8 out of 10 cases. In 111 patients, the amount of stainable iron was compared in bone marrow smears and marrow needle biopsies. The correlation between the two methods was good (r = 0,73, p<0,001), but with a higher frequency of false negative results in the marrow biopsies (5,4%) than the marrow smears (1,8%). As both methods may occasionally fail, it may be desirable to perform iron staining in both smears and biopsies. If it is necessary to choose one of the methods, we would recommend bone marrow smears for the routine determination of stainable marrow iron stores.

Platelet aggregation in response to ADP, collagen, ristocetin and polymerizing des‐AA fibrin monomers was studied in six patients in chronic, stable, renal failure on maintenance haemodialysis. A case report is presented concerning a 39 years old man with 8 days history of impending myocardial infarction with transitory ECG changes, but no rise in serum enzymes.Selective coronary angiography revealed single vessel coronary artery disease with occlusion of the LAD in the middle 1/3.Streptokinase was then given selectively into the left coronary artery by continuous infusion for 140 min. At the end of the infusion continuous flow was noted in a narrowed LAD.Clinically he was later fully normalized including a normal ergometer bicycle exercise test.Control angiography after 7 weeks revealed an almost fully normalized LAD, significantly improved from just after the streptokinase treatment.In the discussion of this successful case, special attention is paid to the duration of the streptokinase infusion, as well as to the improvement achieved in the post‐infusion perio

In a prospective study 99 consecutive patients admitted for elective aorto‐coronary by‐pass surgery were subjected to estimation of their fibrinolytic capacity.Reevaluation after 16 months included shunt angiography and repeated testing of the fibrinolytic capacity.In the total material consisting of 244 grafts (2.7 per patient) 76 (31%) were occluded. In the subgroup of 24 patients with reduced fibrinolytic capacity, 25 (38%) of the 66 grafts were occluded as compared to 53 (30%) of the 178 grafts in patients with normal fibrinolytic capacity (p = 0.2). The number of grafts per patient and the values of the peroperatively measured blood flow in the grafts were similar in the 2 groups.Looking into the subgroup of grafts with “poor flow values” as measured peroperatively (≤ 35 ml/min), 14 (78%) of 18 grafts were occluded in patients with reduced fibrinolytic capacity, whereas 28 (53%) of 53 grafts were occluded in patients with normal fibrinolytic capacity (p = 0.06).In conclusion, the present study is thought to indicate that reduced fibrinolytic capacity might be an additional risk factor for graft occlusion in patients undergoing aorto‐coronary by‐pass surgery, particularly so in grafts where low blood flow values
