A fecal or urinary diversion is frequently recommended for a variety of patient conditions. This article will review some of the history of stoma creation, discuss current practices in stoma care, and summarize the current literature regarding stomal complications. Outcomes of fecal and urinary diversion procedures will be depicted, and postoperative quality of life will be described. An update of current ostomy equipment will be presented, along with the crucial role of the WOC nurse in stoma care rehabilitation. Areas for future research in the ongoing drive for evidence-based practice will be highlighted.

In today's rapidly evolving health care environment in which quality, cost-effectiveness, and outcomes are among the most valued health care products, significant issues must be addressed regarding ostomy products and how persons with stomas use them. The lack of evidence-based data has a direct impact on the pocketbooks of many patients with ostomies in the United States and their quality of life. Regulators, accrediting agencies, and payers look to WOC nurses and other providers to provide a sound scientific base upon which they can develop standards and regulations that will improve the life of Americans with stomas.

ObjectiveThe objective was to collect data related to individual dietary recommendations and restrictions for people with ostomies.DesignA descriptive survey was used that asked people about food choices/avoidances that occurred because of their ostomy.SubjectsSubjects were members of the United Ostomy Association who had an ostomy and were living at home.MethodsA 14-item questionnaire was mailed to the United Ostomy Association membership (approximately 40,000 current members) in their quarterly newsletter.ResultsSix hundred and four questionnaires were returned. The most frequent reason for ostomy surgery among the respondents was colon cancer (71%; n = 424), and the most common type of ostomy was a colostomy. The majority of respondents (88.4%) stated that they were not following a special diet because of the ostomy; 11.8% (n = 69) stated that they had modified their diet. Of the people who had modified their diet, 55% stated that they were using the special diet indefinitely and were adhering to it very well. More than half of the participants had received dietary instructions, provided mainly by a WOC nurse, at the time of surgery. The foods avoided because of the ostomy included fresh fruits, nuts, coconut, and vegetables such as corn, popcorn, cabbage, beans, and onions. The major problems reported were blockage and increased gas.ConclusionMany respondents reported problems with certain foods. Preoperative teaching and postoperative follow-up must include anticipatory guidance on food selection.

The modern era of ileal pouch anal anastomosis began just over 20 years ago. Several areas remain controversial, including patient selection, surgical technique, use of a temporary diverting ileostomy, aging sphincters, complications, and postoperative management. This article reviews current controversies concerning ileal pouch anal anastomoses and our approach to managing patients who undergo this complicated procedure.

Gross painless hematuria often prompts patients to seek medical attention. For some persons, especially those aged 55 to 80 years, this hematuria will prove to be the result of bladder cancer. Patients found to have muscle-invasive disease commonly undergo radical cystectomy and urinary diversion. The decision about which urinary diversion will be best suited to the individual patient is complex and depends on multiple factors relating to the patient and the tumor as well as a clear understanding about the risks and benefits of each diversion. This article will discuss issues such as cancer control, preoperative and postoperative care, management of the condition at home, and quality of life associated with the various urinary diversion options, with a special emphasis on orthotopic neobladder reconstruction. Awareness of the special issues associated with this procedure is needed to safely counsel and care for patients with these forms of bladder substitution.
