AbstractThe literature indicates that Doppler demonstration of pulsatile flow in the portal vein suggests heart disease, and that retrograde transsinusoidal transmission of atrial pulsations is the mechanism. We noninvasively investigated portal vein pulsatility (PVP) in normal subjects and in patients with cardiac and liver disease, and performed invasive studies in cirrhotic humans and normal pigs. We found that accentuated PVP occurred in some normal subjects and in some patients with cirrhosis, and that mechanisms other than transsinusoidal transmission of atrial pulsations contributed to PVP. Determinants of PVP may include pulsatile portal inflow, transmission of pulsations from the vena cava (IVC) and location of the Doppler sample volume relative to the IVC. © 1995 John Wiley&Sons, Inc

AbstractWe evaluated intrarenal arterial waveforms with image‐directed color Doppler ultrasonography in 21 patients with systemic lupus erythematosus (SLE). The systolic–diastolic ratio, resistive index, and pulsatility index were compared with serum creatinine levels, creatinine clearance, and quantitation of urinary protein excretion, as well as with histopathologic scores of specimens obtained from 9 patients who underwent renal biopsy. Doppler parameters were in the normal ranges in all patients, without showing significant correlation with any of the histopathologic scores or laboratory parameters except creatinine levels. We conclude that image‐directed color Doppler ultrasonography is of no practical value in the evaluation of lupus nephritis during the early stages of the disease. © 1995 John Wiley&Son

AbstractUnlike conventional transcranial Doppler (TCD), transcranial color‐coded sonography (TCCS) enables imaging of the basal cerebral arteries using color‐flow ultrasonography and correction for the angle of insonation when determining blood flow velocities. We present hemodynamic data from 20 normal subjects, each studied with TCD and TCCS.Velocities derived using TCCS with angle correction were significantly greater than those derived using TCD in all vessels (mean velocities [cm/sec; mean with 95% confidence intervals])—anterior cerebral artery: TCD 48 (45–50), TCCS 62 (58–66),p<.0001; middle cerebral artery: TCD 61 (58–64), TCCS 70 (66–74),p<.0001; posterior cerebral artery: TCD 43 (41–46), TCCS 54 (50–57),p<.0001; basilar artery: TCD 40 (34–45), TCCS 45 (38–52),p<.01. Pulsatility index values were significantly greater in all arteries when determined by TCCS, and resistance index values were significantly greater except in the basilar artery. Correcting for the angle of insonation using TCCS may enable estimation of blood flow velocities closer to the “true” values than those derived using conventional TCD. ©

AbstractSeventy‐four low birth weight infants underwent serial measurements of cerebral blood flow velocity (CBFV) using Doppler ultrasound together with ultrasound imaging of the brain. Surviving infants were examined and assessed using the Bayley scales of mental and motor development at 18 months post‐term. There were no significant differences in mean CBFV between normal and impaired infants. Infants with abnormal neurological signs (n= 6) or developmental delay (n= 3) did not show the usual steady rise in CBFV during the first few days of life. Four of nine had a rise, then a fall in CBFV compared with 4 of 31 with complete data in the normal group. This difference is significant (χ2= 4.3,p= 0.03). The magnitude of the percentage increase between day 1 and day 3 was also smaller in the abnormal group (median 0% vs. 39%, Mann–Whitneyp= 0.03). A structural abnormality seen on the cranial ultrasound image was a better predictor of adverse outcome than an abnormal CBFV pattern, with a better sensitivity and specificity (66% and 97% for imaging compared to 44% and 87% for CBFV). The addition of serial ultrasound Doppler measurements of CBFV did not improve the prediction of outcome obtained using ultrasound imaging alone. © 1995 John Wiley&So

AbstractThe purpose of this study was to test the hypothesis that the acceleration time (AT) of the fetal umbilical artery remains constant at term for normal pregnancies. In addition, we also examined whether the AT has any correlation with umbilical cord blood hematocrit (Hct). In total, 539 normal‐term fetuses with menstrual ages (MA) ranging from 37 weeks to 42 weeks were enrolled in a cross‐sectional design. The AT of the fetal umbilical artery was measured prior to delivery using a high‐resolution, real‐time Doppler scanner. All were delivered within 2 days of the ultrasound examination. At parturition, blood from the umbilical vein was collected and the red cell indices, including the Hct, were determined. The results indicated that the mean values of AT of the umbilical artery remained constant during normal‐term pregnancies from 37 weeks to 42 weeks, menstrual age (mean: 0.104 sec, SE: 0.001 sec,n= 539). Although the AT was thought to be affected by the Hct, the AT had no correlation with umbilical venous Hct (n= 539,r= 0.002,p>0.05). This constant value of the AT may be used as a reference for fetal physiology and perinatal medicine. © 1995 John Wiley
