AbstractThe diagnostic and prognostic value of ultrasound, β‐HCG subunit, and progesterone are evaluated in a prospective study in 169 patients with threatened abortion. Ultrasound diagnosis had just been introduced at the hospital when the study began and clinicians accepted the patients for treatment without receiving the ultrasound, HCG, or progesterone results. With ultrasound, a correct diagnosis of the in utero situation (true positive and true negative) was made at admission in 93% and after 1 week in 99% of the cases; in 1% of the cases intact pregnancies were mistaken for abortions (false positive). AbstractThe results of various diagnostic procedures performed on six cases with solitary and minimal (<2 cm) hepatocellular carcinoma (HCC) that was later surgically resected were comparatively reviewed. The tumor was detected in 6/6 patients by ultrasonography, 1/6 by scintigraphy, 2/6 by CT, and 4/6 by angiography, and hepatocellular carcinoma was diagnosed in 3/6 by angiography and 6/6 by ultrasonically guided fineneedle aspiration biopsy. AbstractSonographic detection of fetal growth retardation was evaluated on a sample of 778 patients routinely scanned over a 16‐month period. From the 29th week to term, the sensitivity of abdominal circumference measurement progressively increased from 41% to 88% (P<0.01). Higher rates were achieved by using the biparietal diameter but false‐positive diagnoses were twice as frequent as with the abdominal measurements. When the limitations in precision of abdominal circumference were considered, the screening rate of this parameter increased about 16%. Moreover, 50% of the falsenegative diagnoses were made on fetuses weighing between the fifth and 10th centiles at birth. These diagnostic indices for biparietal diameter and abdominal circumference evaluated on a retrospective series proved to be lower than those reported for the prospective selected series. However, the results yielded by routine ultrasound were better than those obtained with clinical or traditional biochemical methods. AbstractThe sonographic findings in eight cases of splenic infarct at their onset and at different phases of their development have been retrospectively analyzed. A wide range of appearances was seen (single or multiple, rounded or wedge‐shaped, echo‐free, hypoechoic, and hyperechoic lesions). In our opinion, such variable appearances are related to the age of the infarct (hypoechoic or echo‐free in the earlier stages, hyperechoic in healed infarcts). A presumptive diagnosis is possible in the earlier stages since the detection of changes (in echogenicity and/or in size) over a period of time strongly suggests an infarct. AbstractFifty consecutive cystic renal masses in 43 patients were evaluated by both static scanning and real‐time scanning techniques. Various parameters were evaluated and compared to ascertain whether the strict static scan cystic criteria were always present in comparable real‐time evaluations. Four (8%) of the 50 masses were not classic cysts on real‐time examinations, although the criteria were present on the static scans. Two had minimal acoustic enhancement and two had low‐intensity internal echoes. Possible reasons for this difference were detailed and discussed. The potential problems in this observation were enumerated. AbstractNoninvasive monitoring of stroke volume by pulsed Doppler echocardiography is an important recent advance in neonatal care. Using this technique, we assessed stroke volume in 112 healthy neonates (birth weight 780–5350 g, menstrual age 27–42 weeks). Stroke volume increased with advancing birth weight (r= +0.93) and gestational age (r= +0.90). Mean stroke volume per kilogram body weight (1.77 ± 0.28 ml/kg) was constant throughout the study population. Doppler‐derived stroke volumes correlated linearly with left ventricular dimensions in diastole and in systole. AbstractThe sonographic appearance of the normal adrenal gland in children varies with age. In newborns, the cortex is large and hypoechoic, whereas the medulla is relatively small and hyperechoic.
