61 patients with seminoma and 113 with nonseminomatous germ cell tumors of the testis were treated according to the histology, stage of disease, and serum levels of tumor markers (CEA, AFP, hCG, hPL and SP1). 33 were stage I, 63 stage II, and 78 stage III patients. Most patients with seminoma, mature teratoma, immature teratoma, and ‘pure type’ embryonal carcinoma, as well as the latter three types with seminomatous admixture, had normal serum levels of the markers. Sometimes, slightly elevated levels of hCG suggested the presence of metastases. But, serial measurements of the markers were seldom useful in monitoring therapy. The 5-year tumor-free survival rates were favorable: 100% for stage I and II disease; and 57 or 44% for, respectively, stage III seminoma or the other tumors amounting to 10% of the nonseminomatous group. The role of the five markers was significant in patients with teratoma with malignant transformation, choriocarcinoma, endodermal sinus tumor (EST), and embryonal carcinoma or teratocarcinoma with an admixture of EST or choriocarcinoma or both. Elevation of a marker was a grave prognostic sign. The 5-year survival rates were 100, 16, and 4% for stages I, II and III disease, respectively. An elevated level of one or more of the markers assayed was always useful for monitoring therapy. Decreasing level indicated regression. However, return of an elevated level to normal did not indicate eradication of all tumor and called for diagnosis by imaging modalities. Constantly elevated or increasing marker levels during treatment indicated resistance to therapy. An increasing level from any nadir during remission indicated recurrence. Vincristine and prednisolone are important drugs for induction therapy in acute lymphoblastic leukemia. In our experience quite a number of patients relapse during maintenance therapy after induction. At this stage some of the patients are resistant to vincristine and prednisolone. In the present study we attempted to explore whether vincristine could be used together with cytosine arabinoside. Vincristine being a phase-specific agent is known to synchronize the leukemic cells in vitro and a drug like cytosine arabinoside added after vincristine at the appropriate time is expected to have added cell kill. In vitro studies with leukemic cells confirmed the expected increase in cell kill with the addition of cytosine arabinoside 6 h after vincristine treatment. Simultaneously, the same approach was explored in the clinic. Acute lymphoblastic leukemia patients who were resistant to vincristine and prednisolone for reinduction, remitted with this strategy, i.e. with the addition of cytosine arabinoside 6h after the vincristine treatment.

Ten chemotherapeutic agents, mostly phase I and II drugs, were tested for activity against two human lymphomas heterotransplanted in nude mice. Three of these agents have been tested in phase II trials in patients with lymphoma and found to lack activity; a corresponding lack of activity was found in lymphoma-bearing nude mice. Apart from cyclophosphamide, which is known to have activity against lymphoma and was used as a positive control, only dianhydrogalactitol (DAG) had antitumor activity in the lymphoma-bearing nude mice. Tumor regressions induced by DAG in a heterotransplanted diffuse histiocytic lymphoma were determined to be significant using a statistical method designed for such studies. These data suggest there should be further study of DAG in patients with lymphoma.

A specific anti-carcinoembryonic antigen (CEA) antiserum was used to identify CEA-positive tumor cells in peritoneal and pleural effusions obtained from patients with various malignant neoplasia. In 7 out of 10 fluids in which tumor cells were detected by cytological examinations, cytoplasmic CEA-positive cells were also detected by an indirect immunofluorescence test. In addition, out of 11 fluid samples cytologically negative for tumor cells, CEA-positive cells were found in 8 cases. When both staining for cytoplasmic CEA and soluble fluid CEA levels were considered in combination, 81% of the samples were found to be positive by either one or both of these markers, whereas only 54% were positive by using cytological criteria. The data suggest that CEA marker may be used to identify tumor cells and to assess malignancy in pleural and peritoneal effusions in patients with certain types of cancer. The CEA marker was also used for identifying tumor cells grown in tissue cultures and for separating viable CEA-positive and CEA-negative cells from a mixed cell population using the fluorescence-activated cell sorter

A rabbit antiserum prepared to 2nd-trimester fetal organ extracts and absorbed with adult tissue (anti-STFa) was used to detect antigens common to 2nd-trimester fetal large bowel, normal adult stomach, several colon carcinoma cells (in primary and established cultures) and epithelial cells cultured from benign colonic polyps. The frequency of anti-STFa-positive cells was highest in cultures derived from benign tumors known to be associated with a greater degree of premalignancy. Thus, the percentage of antigen-positive cells increased from 18 and 43% to 70% of cultures from tubular, villotubular and villous adenomas, respectively. Considerable heterogeneity in the distribution of antigen-containing cells was evident within any given area of a positive culture. Absorption experiments, using a spectrum of fetal and normal adult tissue extracts, indicated that the adenoma-carcinoma specificity resides in fetal, but not adult, large bowel and normal adult stomach.

Pretreatment total serum protein of 0.6 mg% predicted a bad prognosis for patients with metastatic gastric cancer. If two of these objective laboratory tests predicted a good prognosis, partially nonambulatory patients lived an additional 20 weeks (median survival 27.9 vs. 8.7, p < 0.001) and patients with no clinically recognized liver metastases lived an additional 22 weeks (median survival 34.2 vs. 12.4, p < 0.001). Patients with liver metastases lived an additional 11 weeks (median survival 20.9 vs. 9.6, p < 0.001). These objective laboratory tests improve the assessment of patients in clinical trials. The model corrected a false assessment of a poor prognosis for 38% of all patients and 64% of patients with liver metastases.

The effect of combination chemotherapy (bleomycin, actinomycin D, vindesine and DTIC) on taste sensation in patients with malignant melanoma was evaluated. Five concentrations of 4 basic tastes (sweet, bitter, sour and salt) were tested. Lowest concentrations of all tastes were subjectively rated more intense after chemotherapy than before. This change was significant for sweet, sour and salt. The highest concentration of sweet was rated significantly less intense following chemotherapy. The discrimination between highest and lowest concentration was diminished for sweet, sour and bitter and marginally for salt. The changes in taste sensation following chemotherapy could attribute to anorexia in cancer patients treated with cytostatic agents.

Kinetic studies of leukemic blast cells (LBC) and erythroblasts were carried out in 20 cases of acute myeloblasts leukemia (AML), and the results were compared with those observed in dividing granulocytic precursors (DGP) and erythroblasts in a control group of 16 normal subjects. The parameters studied were: mitotic index (MI), labelling index (LI-3HTDR), MI/LI ratio and mitotic time (MT). The results showed that (1) LBC of AML do not proliferate more quickly than normal DGP, on the contrary, they replicate more slowly; (2) LBC obey some ‘ecological laws’ of normal hemopoiesis as a response to the control of the proliferative activity and release through the ‘marrow-blood barrier’; (3) the difference between hyperleukocytemic and subleukemic AML is not related to the multiplicative activity of marrow LBC, but rather to other unknown biological factors; and (4) the kinetic behavior of erythroblastic population in AML is consistent, at least in some cases, with its ‘leukemi

The effects of cimetidine were studied in mice bearing a transplantable syngeneic fibrosarcoma. Both tumor size and extent of lung metastasis were enhanced by cimetidine when drug treatment was commenced prior to but not 2 weeks after inoculation of tumor cells on the hind feet.

Infection with herpes simplex virus (HSV) enhances the growth of Lewis lung tumor (LLT). Infection with HSV-1 enhances as efficiently as HSV-2. A significant acceleration of primary tumor formation was obtained. In mice, lung metastases were increased in number and size above the controls. Enhancement occurred only with high doses of infectious virus and was also obtained only when the virus infection was at a site close to that of the tumor implantation. This finding, in addition to the lack of antigenicity of LLT, is interpreted as arguing against an immunologic mechanism of the phenomenon.
