The registry of digestive cancer set up for the French department of Côte d’Or (455, 727) enables a morbidity study about esophageal cancer to be made among residents. The crude annual incidence rate was 16.7/100,000 for males and 1.2/100,000 for females. Corresponding age-standardized rates were 13.2 and 0.5. The sex ratio is extremely high. Among other cancer registries Côte d’Or is in the intermediate range for males, and in the low range for females. 40% of tumors were in the middle third, 37% in the lower third, and 23% in the upper third. Most cancers were squamous cell carinomas (94%). The risk of an associated aerodigestive tract tumor is important (16%). 10% of patients were operated upon for cure, 53% had radiotherapy. The drug combinations of dianhydrogalactitol and VP-16 and dianhydrogalactitol, VP-16, and triazinate were used in patients with primary brain tumors, principally astrocytoma, recurrent following cranial irradiation. Tumor regressions were noted in 40% of patients treated with the 2-drug regimen and in 33% of patients treated with the 3-drug regimens. Regressions were noted in all grades of tumor. Poor performance score on the patients’ part did not seem to effect regression rates. Myelosuppression was the principal toxicity encountered. The combination of pyrazofurin (PF) and 5-azacytidine (5-aza-CR) was evaluated in 12 patients with neoplastic diseases who had failed conventional therapies. They included 6 patients with acute myelocytic leukemia (AML) and 6 with various carcinomas. PF was given by i.v. bolus at doses ranging from 25 to 200 mg/m2 followed by 5-aza-CR given by continuous 24-hour infusion at doses ranging from 30 to 120 mg/m2/24 h during 40–120 h. Dose-limiting toxicity was mucocutaneous, which appeared in 11/17 courses. Skin rash was similar to that produced by PF alone, but was present after much lower doses of PF when followed by 5-aza-CR. Moderate and reversible thrombocytopenia and leukopenia occurred in 3/6 patients with carcinoma. No objective response was seen in patients with carcinoma, and no marrow remission occurred in patients with AML. 24-Hour urinary excretion of orotidine and oroticacid were measured during 11 courses in 8 patients, and were not much different than those obtained previously with PF alone. The reactivities of leukocytes from gastric cancer and noncancer patients to gastric tumor and normal tissue extracts were tested by the leukocyte adherence inhibition (LAI) microtest, assessing cell-mediated immunoreactivity to adenocarcinoma of the stomach. The reactivities were expressed with the LAI index. All leukocyte preparations showed low reactivities, a LAI index of less than 20%, to normal tissue extracts and only the preparations of leukocytes from cancer patients displayed high reactivities, a LAI index of more than 20%, to tumor extracts. Assuming that a patient is sensitized to gastric tumor antigen if his leukocytes respond to at least one tumor extract with a LAI index of more than 20%, approximately half of the cancer patients tested in the present study are considered to have positive cell-mediated immunoreactivity to gastric tumor antigen. Thus, the LAI microtest appears to be a simple, rapid and specific method for demonstrating cell-mediated immunity to tumor.

Male Sprague-Dawley rats were given 10 weekly intravenous injections of acetoxymethyl-methyl-nitrosamine (AMMN = N-nitroso-acetoxymethyl-methylamine). One group was also treated with disulfiram (DSF) by gavage twice daily five times per week. DSF markedly reduced the rate of heart and lung tumors but not of tumors at other organ sites. In animals treated with the combination of DSF and AMMN a significant incidence of neurogenic tumors was observed. No neurogenic neoplasms occurred in animals treated with AMMN alone. During the observation period DSF showed no carcinogenic potential.

Neoplasms of the liver were induced in 18 cases out of 44 in the Egyptian toad, Bufo regularis, by injection with 20-methylcholanthrene. Dose level was 10 mg/50 g body weight once a week. However, the injection of the same dose level of this hydrocarbon together with vitamin A palmitate on the level of 20,000 IU/50 g body weight once a week was found to develop hepatomas in the liver in only 1 of 45 cases. Results of this study indicate that vitamin A palmitate inhibited the carcinogenic effect of 20-methylcholanthrene.

Serial determinations of plasma carcinoembryonic antigen (CEA) levels were assayed for 34 patients with advanced gastrointestinal cancer undergoing 5-fluorouracil and mitomycin C combination chemotherapy. 21 patients (62%) had an objective response and the CEA level was decreased in 12. After chemotherapy was initiated a rise in CEA level occurred on the average 6.5 months later, while the mean time to clinical relapse was 8 months. 5 patients (15%) had stable disease; the CEA level was decreased in 2. In 8 patients (24%) with progressive disease, the mean time for a rising CEA level was 3 months. In our view, a change in the CEA level reflects the effectiveness of combination chemotherapy in patients with advanced gastrointestinal cancer.

A combined treatment modality including surgical excision and daily injections of high molecular levan seemed to be effective against Lewis lung carcinoma in C57BL mice. The inhibitory effect of levan on tumor growth in mice treated by resection of the primary tumor was demonstrated as follows: (1) recurrence of primary tumors was less in levan-treated mice compared to untreated animals; (2) metastases were less numerous in the levan-treated animals; (3) levan treatment partly prevented loss in body weight, and (4) it increased the survival rate. Recurrence of primary tumors appeared to be more efficiently inhibited by local levan treatment, while systemic (i.p.) administration was more effective than local treatment in preventing metastases. The mechanism of inhibition of tumor growth was related to accumulation of macrophages around the metastases.

Different schedules of thoracic irradiation (1,000 rad single dose) and adriamycin (3.0 mg/kg/day for 3 consecutive days given i.v.) were used to treat mice bearing pulmonary microcolonies of the Dunn osteosarcoma. Lung colony numbers and volumes were scored in mice sacrificed 21–25 days after tumor cell inoculation. Significantly greater levels of tumor control were achieved when the 2 agents were given concurrently or when the irradiation preceded adriamycin by 7 days, compared to lower levels of tumor control when the adriamycin preceded the irradiation by 7 days. The preterminal intraperitoneal implanted rat leukemia L 5222 was used to test the chemotherapeutic activity of 10 newly synthesized nitrosoureas: ethylmethanesulfonato-CNU, acetamido-CNU, dihydroxy-propyl-CNU, carboxyethyl-CNU, cyanoethyl-CNU, morpholino-CNU, piperidino-CNU, methylene-dioxybenzyl-CNU, methylene-3-pyridyl-CNU and methylene-4-pyridyl-CNU; their activity was compared against BCNU, CCNU, MeCCNU and hydroxyethyl-CNU. All compounds tested showed a more or less pronounced chemo-therapeutic activity. MeCCNU was superior to all other compounds investigated. The predictive value for human tumors of results obtained from a model which is highly sensitive to nitrosoureas is discussed.
