75 patients with various solid tumors treated with combined 5-fiuorouracil (5-FU) and Velban, were compared to those patients treated with 5-FU or Velban alone. Drug toxicities caused by the combined therapy were significantly reduced as compared to those caused by a single drug, while the response of gastrointestinal tumors was identical when treated with combined therapy nd 5-FU alone. We conclude that combination chemotherapy with 5-FU and vinblastine offers some advantage over single drug therapy for solid tumors. We found that by combining these drugs in lesser dosage we were able to reduce the toxic side effects of the therapy without reducing its effectiveness.

Small lesions of breast or colorectal origin have been eradicated with 5-fluorouracil (5-FU) therapy and did not recur after the chemotherapy was discontinued. This has not occurred in larger lesions. It is preferable to treat patients with known remaining disease early rather than to wait for them to become symptomatic which often renders them poor risk for effective chemotherapy. In patients having curative resections for gastrointestinal, breast, or testicular cancers that are high risk for developing recurrence, adjuvant chemotherapy appears promising.

Certain compounds capable of blocking the growth of cultured animal cells or causing them to lose viability upon prolonged exposure may have no apparent effect upon tumor growth in vivo, if administered by injection at widely spaced intervals, due to rapid inactivation, breakdown or excretion by organs of the mammalian host. Continuous infusion or repetitive infusion at shortly-spaced intervals of compounds of this sort into arterial blood supplying the tumor region holds out the prospect of achieving a prolonged localized concentration capable of arresting tumor cell multiplication, while dilution of the inhibitor upon entry into the general circulation and its rapid clearance from the blood via metabolic conversion or excretion prevents accumulation to toxic or immunosuppressive levels in the body of the host. A novel use of bradykinin to induce inhibitor trapping and thereby augment the concentration of inhibitor in the interstitial fluid of the tumor region is described. Examples are presented of compounds potentially useful in this mode of chemotherapy.

The rationale for freezing carcinoma adjunctively with chemotherapy is synthesized from published data on tumor cytokinetics, regional chemotherapy, cryopathology, and the microvascular hemodynamics of proliferating tumors. Acute cryolesions in the dog’s tongue, arterially infused with methylene blue solution demonstrated early central ischemia, increasing porosity of marginal circulation and extravascular trapping of dye for 5 h. The dog intestine tolerated transmural cryonecrosis without perforation. 27 patients with cancer of the oral cavity received Methotrexate arterial infusion with cryosurgery without complications since May 1969. Studies on the composition of newly-synthesized nucleolar and ribosomal RNA of the Novikoff rat hepatoma have demonstrated that a portion of these molecules differs from the corresponding portion of similar RNA molecules of normal liver These studies suggest that the genetic defect in cancer cells may be mirrored in aberrations of nucleolar RNA. Accordingly, the possibility exists that modification on this RNA by uptake of specific oligonucleotides and localization on the genes involved in its synthesis may provide a mechanism of inhibition of growth of tumor cells. To test the possibility that tumor cells can take up intact oligonucleotides or RNA molecules, labeled tRNA was incubated with tumor cells in vitro and significant cell uptake of exoge-neous tRNA occurred. Accordingly, it seems possible that one approach to cancer chemotherapy may be the uptake of normal RNA molecules or specifically defined oligonucleotides.

A case of hemangiopericytoma of subcutaneous tissue with multiple metastases to the lungs is presented ombined treatment with methotrexate, cyclophosphamide and vincristine sulfate inhibited temporarily the growth of the metastases in the lungs, as shown in growth curves of 3 metastases whose relative volume was estimated from chest roentgenograms. No other treatment employed inhibited the tumor progression. As hemangiopericytoma is considered to be resistant to cytostatic drugs, combination chemotherapy in the treatment of a diffuse metastatic disease is suggested.

The treatment of advanced breast cancer patients has been clarified through the efforts of prospective, randomized studies on large populations. Treatment is palliative, using single agents in a sequential manner, with close follow-up of results. Premenopausal patients undergo castration with subsequent treatment consisting of hormones or chemotherapy in castration failures or further endocrine ablation in castration responders. Postmenopausal females undergo treatment with androgens, estrogens or a combination of corticosteroids and triiodothyronine with endocrine ablation reserved for hormonal failures. Radiotherapy is used for treatment of localized disease, bone pain or impending fracture.

Isolated perfusion is of established effectiveness in Melanoma the treatment of recurrent melanoma, particularly of the extremities. 105 perfusion operations were performed on 92 patients in the past 12 years. 44 of these patients had recurrent melanoma of the extremities and 10 (22%) are free of disease 7 to 124 months postoperative. In this group the five-year survival is 35%. Perfusion was employed as an adjunct to surgical excision therapy for primary melanoma in 39 patients, 27 (69%) are free of disease with a five-year survival rate of 73%. However, no conclusion is reached as to the value of this procedure as an adjuvant to surgical excision.

Chemotherapy in 400 patients during a 34-month period utilized 603 courses of therapy with 30 different agents. In 222 patients with measurable lesions, a 30-percent response rate was observed. Approximately ¼ of the responsive patients had complete responses, lasting up to 24 months, with a median complete response duration of 8 months. Of the 374 evaluable drug courses, 74 (20%) produced responses. Serious complications occurred in 45 patients, but only 3 drug-related deaths occurred. Of the 178 non-evaluable drug courses, 33 were administered prophylactically, as an adjuvant in patients with a poor prognosis. 82% of these patients remained alive and free of disease. 92 non-evaluable patients were treated for known but unmeasurable lesions; 34% of these patients remained alive. The other 49 non-evaluable patients received courses of therapy which were too short to permit evaluation. Of the common tumors, colo-rectal carcinoma and carcinoma of the breast were the most responsive to chemotherapy. The drugs which produced the highest percentage of responses in evaluable patients were 5-FU (24%), MTX (22%), ThioTEPA (21%) and DTIC (21 %), which appeared to be uniquely effective against malignant melanoma
