摘要:
A case of dermatofibrosarcoma protuberans seen at the West Virginia University Medical Center is reviewed. Seven inadequate local resections of the primary tumor resulted in a solitary pulmonary metastasis and two intrathoracic pleural wall recurrences. These events necessitated two thoracotomies for removal of the pulmonary lesion and one chest wall recurrence. The second chest mass one year later was unresectable. Treatment with cobalt therapy, Methotrexate, Cytoxan and Actinomycin D is planned.

摘要:
A serum protein that is present in normal rat serum, and can be detected if serum is fractionated on polyacrylamide gel electrophoresis, disappears from the serum of hepatoma-bearing animals as the hepatomas increase in size. The protein has been purified and its molecular weight has been determined. It binds hemoglobin and could be a haptoglobin or hemopexin. The protein also disappears from the serum of rats kept for 3 weeks on a low protein diet. Such rats support growth of hepatomas poorly. It is postulated that the protein is preferentially used by hepatoma cells as a growth factor or surface effector.

A method of short-term organ culture in chemically-defined media of human mammary carcinoma is described. Hormonally dependent proliferation over a 48-hour period is demonstrable in some human breast carcinomas, and uniform maintenance of the explants was obtained over the 48-hour period of incubation. The method permits routine explanting of an individual patient’s carcinoma in the organ culture system described, and may eventually provide useful information for management of advanced breast carcinoma with appropriate antihormonal therap

33 patients with advanced adenocarcinoma of the colon or rectum were treated with isophosphamide given intravenously at a dose of 4 g/m2 every 3 weeks. Patients receiving the full dose in a single infusion experienced severe nausea and vomiting and hemorrhagic icystitis. Those receiving two infusions of 2 g/m2 each 24 h apart, had only mild gastrontestinal and urinary symptoms. Hematologic toxicity was similar for both regimens, consisting of mild and transient leukopenia in approximately 30% of patients. Alopecia occurred in 60% of patients. Central nervous system toxicity manifested as somnolence and disorientation occurred in three patients. Only one objective tumor response lasting 9 weeks was noted in 33 patients treated with isophosphamide.

摘要:
A review of 76 patients with thyroid carcinoma, in which residual disease was left behind, has been presented. Of the 76 patients, 29 received postoperative irradiation, and 47 received no postoperative irradiation. The median survival for the group receiving postoperative irradiation was 16 years. In the 47 patients with thyroid carcinoma who received no postoperative irradiation, the median survival was 9 years. The treatment of thyroid carcinoma is basically surgical in nature. Patientswith evidence of infiltrative residual disease following surgery may benefit from postoperative irradiation.

摘要:
The efficacy of restoration of cellular immunity in therapy of patients with certain chronic in-infectious or neoplastic diseases is under evaluation in several centers. Theoretically, a substance such as transfer factor is an ideal candidate for this therapy. Transfer factor, a dialyzable substance from blood leukocytes, selectively endows recipients with the cellular immune responses of the leukocyte donors, but does not induce antibody synthesis. Moreover, it does not contain intact leukocytes and therefore does not sensitize the recipient to HL-A antigens and cannot induce graft versus hostdisease. This review summarizes the current status of transfer factor therapy and points out the need for controlled clinical trials.

In order to determine the type of relationship which exists between gluconeogenesis inhibition at the phosphoenolpyruvate carboxykinase (PEP CK) reaction and tumor inhibition, the tumor-inhibiting effects of L-tryptophan, hydrazine sulfate and pyridine-2,3-dihydrazide – all known PEP CK inhibitors – were compared separately with their effects on: (a) the in vitro enzyme PEP CK; (b) gluconeogenesis in the isolated perfused rat liver, and (c) HeLa cell growth in culture. All substances were found to inhibit gluconeogenesis in the isolated perfused rat liver; the site of inhibition of l-tryptophan and hydrazine sulfate being PEP CK, as determined by previous cross-over studies. Both pyridine-2,3-dihydrazide and hydrazine sulfate inhibited the enzyme itself; l-tryptophan has no direct effect (its antigluconeogenic properties are due to metabolites). All three substances had virtually no effect on HeLa cells in culture, indicating a mechanism of in vivo antitumor action other than direct cytotoxicity. These data are consistent with a causal relationship existing between inhibition of gluconeogenesis at the PEP CK reaction and tumor inhibition. The overall mechanism of action is consistent with an inhibition of the energy loss expected from normal host cells of the body by virtue of recycling of many precursors to glucose via gluconeogenesis in the liver and kidney cortex. During a cytogenetic study of the aged population (80 years or older) in a Veterans Administration Domiciliary, we found a 90-year-old man whose karyotype showed a 13/14 translocation, evidenced by radioautographic examinations. His karyotype: 45, XY, 13-, 14-, t(13q14q) + was found in blood and skin cultures and in direct bone marrow studies.
