The experimental and theoretical bases for the use of Lonidamine in cancer therapy are reviewed and discussed. In murine tumors Lonidamine has a narrow spectrum of antitumor effects. It lacks the characteristic properties of antiproliferative drugs, as well as other important pharmacological actions, with the exception of antispermatogenic and embryotoxic effects which are closely related to the antitumor ones. Lonidamine appears not to affect the cell division processes. Available data instead show that it specifically affects the condensed or oxidized mitochondrion; since tumor cells have a mitochondrially bound hexokinase, both respiration and glycolysis are decreased. There is some evidence that hyperthermia, X rays and some chemotherapeutic agents, when used in combination with Lonidamine, increase the response of tumor cells or systems to this drug.

Pulmonary function tests, including TlCO, and renal function tests were performed and GFR and ERPF measured in 18 patients with disseminated testicular non-seminoma before and after remission induction according to the Einhorn regimen. The in vitro inhibitory activity of Lonidamine on the aerobic glycolysis of normal as well as leukemic lymphocytes has been investigated. The extent of the impairment of lactate production induced by Lonidamine on normal thymus (T)- and bone marrow (B)-derived lymphocytes was found to be dependent on their source of origin, i.e. residing or circulating pool. Among leukemia tested ‘null’ and B cell leukemias appeared the most affected metabolically by the compound. Administration in vivo of the drug to the patient with B cell chronic leukemia resulted in a decrease of lactate production by leukemic cells comparable to that induced in vitro. 21 human renal cell carcinomas (RCC) were xenotransplanted into artificially immunosuppressed mice. 4 tumors grew successfully retaining some characteristics of the primary tumors (according to morphology and karyotype analysis), but losing metastatic capacity. One of the serially transplantable tumors (HT 40) with hyperdiploid cellular DNA content and estrogen receptor positivity failed to respond to the single maximally tolerated dose of several cytotoxic agents.

The availability of [125I]-16α-iodo-3,17β-estradiol ([125I]-E2) with binding characteristics similar to estrogen receptor (ER) enabled us to establish a double-labeling assay for the simultaneous determination of ER and progesterone receptors (PgR) using 125I-E2 and [3H]-R5020. The criteria for the establishment of such a double-labeling assay are described. 150 human mammary tumor cytosols have been investigated with the standard routine receptor assay for ER as well as PgR and the results were compared to those obtained by the double-labeling assay. ER: a coefficient of correlation of 0.691 was obtained, the parameters of the regression line were Y = 1.025 x X-0.036. When referring to the standard assay, 3 determinations were false-positive and 4 false-negative. PgR: a correlation coefficient of 0.984 was found, the parameters of the regression line were Y = 0.960 × X + 12.16. One value was false-negative and 1 false-positive. An equivalence of the two methods could be demonstrated. Lonidamine was studied in HeLa cell cultures and BALB/c mice with Meth-A fibrosarcoma. Lonidamine is a hyperthermia sensitizer, both in vivo and in vivo. In vitro the potentiating effects were higher at low pH conditions.
