摘要:
There is a growing number of medically complex children residing in hospitals who could benefit from home care This paper describes a unique federally funded grant project designed to develop alternative home resources for those medically complex children whose parents are unable to provide the requisite home care Building on an innovative partnership between a pediatric hospital and a community child welfare agency, this project recruited trained, and supported foster parents to care for those children Problems, solutions, and strategies for developing alternative home care resources for this population are presented.

A total of 44 adolescents wIth moderate to severe physical disabilities, between 11 and 14 years of age completed an intervention designed to facilitate the emergence of autonomous behaviors The intervention included an adolescent interpretive interview, a teaching physical exam, individual and group health education, social skills development (assertiveness training, role-playing, and peer activities), and structured experiences with disabled adults from a local independent resource center.

摘要:
The problems of children with chronic illness generally extend beyond immediate medical concerns Some of these children experience concomitant psychosocial problems This article describes a family-focused social skills development model which has been used in the children's mental health field to improve children's social interaction, impulse control, and social problem-solving skills In this model, skill training can be focused directly on the child, the family, or both, depending on the resources available, the individual family situation, and whether the child is receiving inpatient or outpatient services Possible preventative and remedial applications of this social skills development model for children and families with chronic illness are discussed.

This paper provides a brief overview of the bone marrow transplantation procedure and examines the potential family disruption that takes place when the family and patient come to the transplantation center for treatment A family stress model developed by McCubbin and Patterson is used to describe the family adjustment and adaptation that occurs during medical crisis The clinical implications of two internal strategies for defining the transplantation procedure are discussed in terms of their possible effects upon the family's attitude toward treatment and recovery.

This is a report of the development and initial testing of a "disease-specific" family behavior scale When related to medical outcome, such scales may be useful in examining specific family behaviors that are harmful or helpful in adjustment to and management of disease In the future, behavioral interventions might be developed for compliance problems An example of such a scale that was designed for use with families of children and adolescents with diabetes is described.

This study compared sibling perception of changes in feehngs and behaviors to parental perception of sibling changes during a pediatric hospitalization The data showed a low level of agreement on a Perceived Change Scale, with siblings reporting a significantly higher number of changes than did their parents Further, the only significant main effect found on seven-way analysis of variance was sibling age, with a lower percentage of agreement for young siblings and their parents than for older siblings Similarly, one significant two-way interaction revealed that the percentage of agreement increased for older siblings when they saw their parents more frequently, but it did not increase for younger siblings In addition, siblings reported the worst parts of the experience and what could be done to help them Implications of these findings for clinicians and researchers are presented.

There is a paucity of information about different methods used in helping families, as a system, adapt to having a child with diabetes This article presents an initial effort with a multiple family group approach during an overnight retreat Discussion focused on diabetes-specific issues but evolved into broader area of communication and family relationships An informal evaluation suggested that the opportunity to meet with other families who face similar challenges was viewed as most beneficial Implications for the continued use of multiple family therapy with these families are discussed.

Providing adequate psychosocial support for hospitalized pediatric patients and their families is sometimes difficult An interdisciplinary team can help caregivers to assess needs and develop strategics for working with difficult patients and families This paper describes the development of a pediatric family care team that has been effective in one hospital outlining the general steps followed in establishing the team A review of practical considerations related to team membership, costs, and procedures is followed by a discussion of the problems encountered A case study demonstrates how the team helped meet thc psychosocial needs of one pediatric patient.
