Severe fetal distress was produced in 16 fetal dogs by successive and/or prolonged occlusion of the maternal abdominal aorta in an acute surgical preparation, and was characterized in fetal arterial blood of 7.06 for pH, 10 mm Hg for O2, 127 mm Hg for CO2, and fetal heart rate decelerations. Five fetuses recovered spontaneously in utero and delivered normally; 3 needed maternal oxygenation before delivery; 3 suddenly died in utero; 5 fetuses were delivered while still alive in utero when the tpH reached 6.85, and eventually expired. Intrauterine recovery and resuscitation was effective in some fetuses.

It is recognised that there is a non-reactive status in the maternal immune system during normal pregnancy as it fails to recognise the fetus which is an allograft. The present study was aimed to understand the nature of cell-mediated immune response and the presence of immunosuppressive factors during human pregnancy. The functional reactivity of lymphocytes was found to be reduced during pregnancy, particularly in the third trimester, as phytohaemagglutinin- and concanavalin-A-induced lymphocyte transformation was affected. It was also observed that in primigravidae and pregnant women with a history of repeated abortions the lymphocyte reactivity was not reduced during the first and second trimesters, but there was a marked reduction in the third trimester. However, in the case of multigravidae there was no change in lymphocyte transformation between the trimesters. When pregnancy serum from various trimesters was tested for its suppressive activity, it was found to suppress the mitogen-induced proliferation of lymphocytes from normal donors. Sera from primigravidae and patients with a history of abortion showed the highest suppressive activity in the third trimester, whereas sera from multigravidae showed high suppression in all the trimesters.

External cephalic version (ECV) using tocolysis is a widely recognized low-risk intervention in cases of breech presentation, particularly in areas where resources are scarce and the perinatal mortality is high in vaginal breech deliveries. In such a setting, pregnant women were trained to make regular self-assessments of the presenting fetal part after ECV in order to detect spontaneous reversions to breech presentation. It was found that such maternal involvement could significantly improve the final results of ECV.

In order to evaluate the influence of the time interval on the second twin in twin deliveries, we have used more precise criteria than have been used in the literature to date. The following parameters of the twins were analyzed: normal CTG of the second twin recorded continuously during labor and pH value of the umbilical artery blood after delivery as well as clinical state according to the modified Apgar score. We could not find a general influence of the time interval on pH and clinical status of the second twin. Our results indicate that in cases of uncomplicated twin delivery with a normal cardiotocogram there is no necessity for the second twin to be born as soon as possible after the birth of the first twin.

The occurrence of antinuclear antibodies (ANA) in an unselected population of pregnant women was studied by an indirect immunofluorescence method. Six women out of 116 were ANA-positive (5.2%), 2 of them having a moderately high titre (1:160 and 1:320) and 1 complement-fixing ANA. The anti-genic specificity of ANA was anti-single-stranded DNA in 2 cases and RNAase-sensitive anti-extractable nuclear antigen in 1 case. Two of the ANA-positive women had pre-eclampsia levis and 1 had a premature delivery. None of the infants of the ANA-positive women had symptoms of connective tissue disease. The occurrence of ANA among pregnant women was not significantly higher than among 70 non-pregnant female control subjects (7.1 %).

Ten plates of normal fetal ultrasonography (at 16-20 weeks’ gestation) showing fluid, liver, lung, bone and intestine were submitted to electro-optical transmission surface densitometry. A distinct echogenic gradient has been found and used as an ‘internal standard’ to evaluate intestinal echogenicity. Intestine had significantly different echogenic properties from bone, liver and fluid but similar to that of lung. Using this echogenicity scale for fetal bowel, observers were correct in 46%, overscored in 32.6% and underscored in 21.4%. These poor results may be due to the significant inconsistency in the interpretation of each plate compared to other plates (about 65%). Although the densitometry-derived grading scale seems valid at these gestational ages, interobserver variation is too large and precludes a simple ultrasonic diagnosis of the hyperechogenic fetal intestine. Densitometric evaluation may be indicated in suspected cases when fetal bowel seems to have increased echogenicity.

The postpartum period is characterized by high levels of circulating steroids which condition hypothalamo-pituitary activities. In pregnancy growth hormone (GH) levels are greatly increased and lack pulsatility. In order to investigate the behavior of GH during the postpartum period, the GH response to GH-releasing hormone stimulation (50 and 100 μg), to amphetamine, a dopamine receptor agonist, and to FK 33-824, an opiate receptor agonist, was investigated in women during the first 5 days after delivery. In all groups GH responses were significantly lower (p < 0.01) than in normal women studied during the early follicular phase. FK-33-824-induced GH release was similar in postpartum and control women. These results demonstrate reduced pituitary GH response to GH-releasing hormone and to amphetamine in women during the postpartum period, confirming the peculiarity of the hypothalamo-pituitary component.

Gonadotropin-releasing hormone (Gn-RH) stimulates phosphoinositide turnover by binding to its specific receptor and suppresses gonadotropin-dependent maturation and steroidogenesis in granulosa cells. This study was undertaken to determine whether persistent receptor occupancy was necessary for Gn-RH to exert such actions on rat granulosa cells, or whether Gn-RH actions were continued by a first and transient stimulation by Gn-RH, using a competitive antagonist, antide. Gn-RH stimulated [32P]phosphate incorporation into phosphatidylinositol (Ptdlns), which could be terminated by displacement of previously bound Gn-RH from its receptor by antide and restarted by reoccupying the receptors with Gn-RH. Antide could prevent Gn-RH-stimulated Ptdlns radiolabelling whenever it was added to incubations. An identical effect of antide was observed also in the antifollicle-stimulating hormone (FSH) action of Gn-RH. FSH markedly stimulated aromatase activity, and Gn-RH caused a time- and dose-dependent inhibition of FSH action. Estrogen production was quenched by Gn-RH and restarted at a time when Gn-RH was removed from its receptor by antide. These two responses associated with the occupancy of Gn-RH receptor provide the evidence in favor of a tight coupling of stimulated Ptdlns turnover to suppression of aromatase activation. These data of required continued activation of receptor might exclude the possibility that hypothalamic Gn-RH participated in the control of steroidogenesis in the ovary.
