Thromboplastin activity in amniotic fluid from 39 patients pregnant in weeks 14–40 has been studied. Amniotic fluid was obtained by amniocentesis. All fluids were fractionated and the activity studied in both supernatant and sediment in a highly specific and sensitive test system. All samples tested showed a very high thromboplastin activity. No correlation to gestational age was found. Amniotic samples from 19 of the patients were incubated with endotoxin. Endotoxin did not further increase the procoagulant activity.

1,066 urine samples were assayed by β-human chorionic gonadotrophin (β-hCG) monoclonal pregnancy test with a sensitivity of 150 IU hCG/l Tests were performed in 5 independent laboratories and results were compared with those of pregnancy tests which were used routinely. Urine samples which showed discordant results under evaluation were reas-sayed for β-hCG using radioimmunoassay. 569 urine samples were from nonpregnant women, and a consistent result with comparison pregnancy tests was achieved in 97.19% of urine samples. The trial test from 389 urine samples of pregnant women was 99.49% in accordance with routine pregnancy tests. Results from 108 urine samples of women over 40 years of age indicate the high specificity of the new β-hCG monoclonal pregnancy test. Titration experiments showed 63% positive tests at 150 IU β-hCG/l Data presented combine to suggest that this new monoclonal pregnancy test is a valuable aid in diagnosing early pregnancy. It may also be used in the diagnosis of tubal pregnancy and in other ectopic processes.

Recovery of gonadotropin-secreting function of the pituitary during the early puerperium has been studied in women with postpartum hemorrhage of 500 g and more during the first 2 h after expulsion of the placenta but without circulatory collapse. On day 4 postpartum, the concentrations of erythrocytes, hemoglobin, and hematocrit were significantly lower in puerperal women with postpartum hemorrhage than in normal puerperal women. On day 20 postpartum, administration of 100 μg synthetic luteinizing hormone-releasing hormone (LHRH) significantly increased serum follicle-stimulating hormone (FSH), but not luteinizing hormone, in puerperal women with severe postpartum hemorrhage of more than 1,000 g, but the FSH response to LHRH was significantly lower in these women than in normal women on the same postpartum day. All puerperal women were lactating and there was no significant difference in serum prolactin levels between normal puerperal women and puerperal women with postpartum hemorrhage. These results indicate that blood loss in the amount of more than 1,000 g during the first 2 h after expulsion of the placenta may cause perturbation to some extent in recovery of gonadotropin-secreting function of the pituitary during the early puerperium, even if there are no complications by circulatory collapse and disabled lactation.

29 pregnancies occurred spontaneously in 28 women with mild to moderate hyperprolactinemia (prolactin levels up to 100 ng/ml). Only 4 of them showed tomographic evidence of microprolactinoma; 1 of the prolactinoma patients experienced a visual field defect at week 30, which was quickly controlled by bromocriptine treatment. Pregnancy was uneventful in the rest of the patients. Subjects who wished to lactate successfully breast-fed their babies. Of 22 women in whom prolactin levels were reevaluated at least 1 month after delivery and/or lactation, normal values were found in 9.

We studied 59 pregnancies in 48 female carriers of prosthetic heart valves. In 44 pregnancies the mother had a metal valve mostly with coumarin therapy and in 15 a bioprosthesis usually without hematologic treatment. In the patients with metal prostheses we had the following maternal complications: 1 maternal death, 3 valve thromboses, 2 heart failures and puerperal hemorrhage. There was also a significantly increased rate of preterm, low birth weight infants and intrauterine growth retardation. Concerning malformations we found only 2 cases of mild nasal hypoplasia. Regarding the complications of bioprosthesis, we had only 1 case of paroxysmal tachycardia and no fetal complications. Comparing the results we came to the conclusion that pregnancy is much better tolerated in patients with bioprostheses and that in patients with metal prostheses the risk is increased if the mother is older than 30 years, if time of replacement is more than 5 years or if there is a history of previous thromboembolism or gravidic heart failure.

90 women participated in a 4-month study. During the first 2 periods, they took no pain relievers whatsoever; during their last 2 periods they took 2 × 325 mg aspirin, acetaminophen or an identically packaged placebo every 4 h to total 8 tablets during the first 24 h of their periods beginning with spotting. For statistical analysis, periods 1 and 2 were combined and averaged, then compared with periods 3 and 4 combined and averaged. Total menstrual loss in grams, number of days of flow, and pain of cramps and headaches were analyzed by MANOVA for each of the three treatment groups. An ANOVA for each of these variables as well as for daily menstrual loss for the first 3 menstrual days was also performed. The MANOVA for all variables by the three treatment groups failed to show any significant differences. Similarly, ANOVAs for the individual variables failed to indicate significant differences except for the variable pain of cramps (p = 0.0072). The Duncan’s Multiple Range Test for pain of cramps showed that the average pain for the placebo group was higher than for either the aspirin or the acetaminophen group, although the means for these two groups were not significantly different. These results indicate that neither aspirin nor acetaminophen in the doses given alter either total menstrual loss or the pattern of loss during the first 3 menstrual days. However, both preparations were found to be more effective than placebo in reducing pain of cramps.

The number and shape of Langerhans’ cells (LC) were studied by determining cytoplasmic formaldehyde-resistant ATPase activity in whole mounts of normal and meta-plastic human cervical epithelium. In normal epithelium the number of LC per square millimeter was 52.75 ± 2.21. A similar number was found in completely differentiated metaplastic squamous epithelium (49.11 ± 2.42), but their shape was different with less branching processes. When metaplasia was still incomplete, and numerous mucous cells remained, no LC were present. On the basis of these results it is speculated that mucous cells provide a negative chemotactic stimulus which prevents migration of LC into metaplastic epithelium.
