A 30-year-old Japanese female with Turner’s syndrome had become pregnant and delivered a normal male infant by cesarean section. Her menarche was at age 14, and secondary sex characteristics developed normally. She had some features of Turner’s syndrome, and cytogenetic studies from peripheral blood lymphocytes and several tissues revealed only 45, XO karyotype without evidence of mosaicism. To our knowledge, she is only the thirteenth case of monosomy X to achieve pregnancy. A review of the literature indicates a high incidence of fetal wastage and increased chromosomal errors in the offspring of women with a 45, XO cell line with or without mosaicism.

Vasomotor effects of serotonin (5-hydroxytryptamine; 5-HT) were studied in isolated human placental chorionic veins in vitro. 5-HT produced marked contraction with a maximum of 89 ± 5.05% of the K+-induced contractions. This response was specifically and competitively antagonized by ketanserin at doses of 10––9 and 10––8M(pA2 = 9.65). Ketanserin, at concentrations up to 10––7M, induced noncompetitive inhibition of the contractions. These findings show a marked vasoconstrictor effect of 5-HT on the capacitance vessels of the human placenta, which suggests that this amine could play an important role as a mediator of fetal circulation. The possible therapeutic effect of ketanserin as a vasodilator to improve blood flow in the uteroplacental unit, is also discussed, based on the ability of this drug to block competitively 5-HT effects on chorionic veins.

One hundred forty-one amniotic fluid samples were analyzed for disaturated phosphatidylcholine (DSPC), phosphatidylglycerol (PG) and the lecithin/sphingomyelin (L/S) ratio. The L/S ratio was measured by two-dimensional thin layer chromatography. Mature levels (positive) were defined as an L/S ratio of 2:1 or more, a DSPC level of 100 μg/ml or more and a detectable level of PG. The DSPC value agreed with the L/S ratio in 114 samples (80.9%). Fourteen of 17 infants with respiratory distress syndrome (RDS) (82.4%) had immature L/S ratios and immature DSPC levels. RDS developed in 16 of 40 (40%) children with immature L/S ratios, and in 15 of 33 (45.5%) children with immature DSPC levels. The true-positive rates of the L/S ratio and DSPC levels were 99.0 and 98.1 %, respectively. PG had a low true-negative rate, as only 17 of 67 (25.4%) samples without detectable levels of PG were associated with RDS. However, when PG was present, it had a 100% predictive value of no-RDS. In conclusion, DSPC is nearly equal to the L/S ratio measured by two-dimensional as concerns diagnostic accuracy. PG is useful as an additional index for predicting lung maturation.

The effect of a newly developed tocolytic oxytocin analogue, 1-deamino-2-D-Tyr(OEt)-4-Thr-8-Orn-oxytocin on lipid and carbohydrate metabolism was investigated. Oxytocin and vasopressin both stimulated lipogenesis in isolated rat adipocytes, an effect which was dose-dependently inhibited by the oxytocin analogue. In vivo, intravenous injection of 10 nmol/kg body weight of the analogue to 11 healthy subjects caused an initial, but insignificant peak after 4 min in both plasma glucose and glycerol, which thereafter remained at basal level. It is concluded that although an antagonistic effect of the analogue on vasopressin- and oxytocin-stimulated lipogenesis could be demonstrated in vitro, the effect on lipid and carbohydrate metabolism in vivo in humans is insignificant.

The concentrations of atrial natriuretic peptide (ANP) and its presumed second messenger, cyclic guanosine-3’,5’-monophosphate (cGMP) were determined in maternal plasma and amniotic fluid. Samples were obtained from normal and pathological pregnancies revealing hydramnios or severe Rh incompatibility between week 16 of gestation and delivery. For analysis of ANP and cGMP, radioimmunoassays were used. ANP and cGMP concentrations in maternal plasma did not differ in normal and pathological pregnancies. In amniotic fluid, we found ANP levels of about 120 pg/ml at 16 weeks of gestation which then decreased below the detection limit of 15.6 pg/ml. In contrast, cGMP levels were low at the beginning of pregnancy (4pmol/ml) and rose significantly (14 pmol/ml), at the end of pregnancy. Pregnancies revealing severe Rh incompatibility exhibited the same levels as normal pregnancies when regular prenatal transfusions resulted in sufficient blood volume substitution. ANP, a volume-homeostasis-regulating hormone, is already produced in early pregnancy. The meaning of the presumed second messenger, cGMP, requires further investigation.

The serum pancreatic and catalytic phospholipase A2 level (PLA2) in human pregnancy is normal, and the increase of pancreatic enzyme before delivery is small. In patients with pruritus associated with obstetric hepatosis maternal serum had a slightly lower pancreatic PLA2 level if the cholic acid level was higher. Umbilical cord blood has twice as much pancreatic PLA2 as maternal blood. If the enzyme concentration was very high, pregnancies were postterm and the newborns had low Apgar scores. Amniotic fluid contains these enzymes, but there is no change in the enzymes during the course of pregnancy. The amount of pancreatic enzyme was not reflected in catalytic activity. A very high PLA2 activity was observed in 1 patient with suspected amniotic fluid embolism and in the meconium, but low in the first urine of neonates.

Human immunodeficiency virus (HIV) has been isolated from fetal tissues as early as 13 weeks and later from fetal blood. These findings have raised the possibility of prenatal diagnosis of infected fetuses by identification of the virus in the fetal compartment. Study of the fetal immune status has proved reliable in prenatal diagnosis of congenital immunodeficiency, and we have tested the possibility to diagnose acquired immunodeficiency in utero by this approach. We studied T lymphocyte subsets and their mitogenic response in fetal blood obtained after elective termination at midgestation in 8 cases and at delivery in 26 cases of maternal HIV infection. Results have been compared to appropriate normal controls. No significant difference was found in terms of total lymphocytes, CD4 and CD8 populations and phytohemagglutinin responses. This indicates either that immunological parameters currently used to assess postnatal immunodeficiency are not reliable during intrauterine life or that the intrauterine environment and the transplacental passage of maternal antibodies interfere with development of prenatal immunodeficiency.

The authors compared the serum DHAS and cortisol level in 53 vaginal deliveries with those of 21 caesarean sections prior to the onset of labour at the 28th to 36th weeks. They also compared 18 vaginal deliveries with 16 elective caesarean sections at the 40th week. The serum hormone concentrations were measured in the maternal vein, the umbilical vein and the umbilical artery. The results indicate that the serum DHAS and cortisol level was higher after vaginal delivery than after caesarean section in the maternal vein, umbilical vein and umbilical artery at the 28th to 36th and also at the 40th week. The authors suggest that, although the role of the fetal adrenal cortex is not so definitive as in the case of some species in the onset of labour, it may be presumed that the increase in adrenal cortical activity cannot be explained exclusively by maternal stress.

The correlation between arousal state and fetal habituation was tested. Four behavioral states were determined according to fetal heart activity and movement pattern in 52 fetuses. Repeated wide low-frequency sound was used as external stimulus. Ceasing of response to 5 consecutive stimuli indicated habituation. The time from response to habituation was defined as habituation time. Although fetuses in a lower arousal state habituate more rapidly, the difference is not statistically significant.

Pain experience and the amount of analgesics needed during labor were studied in 32 primiparous women who had received repeated treatment with acupuncture (AP) during the month prior to term and in 16 nontreated primiparous women. The women’s psychological profiles were evaluated by a psychiatric interview at week 38 of pregnancy. Treatment with AP did not reduce the need for analgesics in labor. During labor, all women experienced successively rising pain irrespective of whether or not they had been treated with AP prior to labor or delivered under local anesthesia. Experience of pain was not reduced in subjective assessments in women treated with AP. There was a strong correlation between assessments of pain made during labor and 6 months after delivery. In the group that did not receive AP, cerebrospinal fluid dynorphin A was significantly lower in parturients who chose epidural anesthesia.
