The role of the renin-angiotensin system in the development of hypertension was studied by infusing an angiotensin II analog, 1-Sar, 8-Ile angiotensin II, in women with toxemia and women on estrogen-containing oral contraceptives or estrogen treatment. In 3 of 4 women who developed hypertension on oral contraceptives or estrogen treatment, the blood pressure was reduced with the infusion of 1-Sar, 8-Ile angiotensin II. In 3 women who remained normotensive on oral contraceptives or estrogen treatment, the blood pressure was not reduced, although there was no significant difference in plasma renin activity (PRA) between the women who developed hypertension and those who remained normotensive on oral contraceptives. In 6 pregnant women who developed hypertension, the blood pressure increased remarkably with the infusion of 1-Sar, 8-Ile angiotensin II, although PRA was increased significantly in all of them. In these women, however, sodium excretion was less than that in normal pregnant women. In 4 normal pregnant women, the blood pressure remained unchanged or increased slightly, and PRA was almost similar to that of women who developed hypertension. The above results suggest that increased PRA in women who develop hypertension whilst on estrogen-containing oral contraceptives or estrogen treatment is more closely related to the development or maintenance of hypertension than that in pregnant women who develop hypertension, although there remains a possibility that the response of blood pressure to 1-Sar, 8-Ile angiotensin II in pregnant women who develop hypertension is influenced by sodium retention.

Immunological interrelations between placenta and kidney have become a matter of interest with regard to the pathogenesis of preeclampsia. Kidney morphology of Lewis rats was studied following repeated transplantations of heterologous or syngeneic placental tissue. In most of the heterologous placenta-recipient animals, diffuse severe glomerular lesions were found, similar to those seen in women with preeclampsia. In the strongly responding recipients, endothelial cell swelling, narrowing of capillary lumina and proliferation of mesangial matrix were observed. Patchy areas of fluorescence associated with dense IgG deposition, as well as fibrin deposits demonstrated in the affected glomeruli, were correlated with histologic changes. The use of syngeneic placenta as a transplant did not cause histological or immunohistological changes. These findings support the view that differences in histocompatibility antigens between mother and fetus may be involved in the pathogenesis of the nephropathy in preeclampsia.

A prospective, randomized, controlled study was carried out to investigate the dilating and softening effect on the cervix of prostaglandin F2-alpha gel administered intracervically and of prostaglandin E2 tablets administered orally. A total of 581 patients were included in the study: 149 received 2.5 mg prostaglandin F2-alpha in gel form intracervically, 150 received 1 mg prostaglandin E2 in tablet form orally, 138 were given placebo gel and 144 no medication at all. In the groupgiven prostaglandin F2-alphagelintracervically, success was achieved in 85.8% of patients and partial success in 8.8%. In the group given prostaglandin E2 tablets orally, the results were successful in 29.4% and partially successful in 40.6%. The incidence of side effects in the group given prostaglandin F2-alpha gel was rather high – 53%. The results show that prostaglandin can bring about a certain softening of the cervix, but side effects cannot be ruled out. Nevertheless, priming of the cervix is to be recommended in young primigravidas.

In order to establish early recognition of possible myocardial damage, the serum myoglobin level was determined by radioimmunological methods in 115 patients who were treated – due to anticipated premature birth – with tocolytically operating β-sympathomimetics druing a period of up to 10 weeks.
